Women at the age of forty-six or forty-seven experience a decline in Qi and Blood in the Liver and Kidney meridians, presenting with distension and pain in the hypochondrium, dizziness and vertigo, aversion to cold with high fever, body-wide pain accompanied by amenorrhea, night sweats, which may eventually develop into consumptive disease (Lao Zhai). Bu Gan Jian (Tonify the Liver Decoction) is the primary formula.

At the age of forty-seven or forty-eight, women are in the year when menstruation is about to cease, often gradually experiencing obstruction of menstrual flow. When the period approaches but menstruation does not arrive, one must be most vigilant and observe carefully. If Qi and Blood are harmonious and there are no other chronic diseases, this gradual cessation is normal and causes no concern. However, if the patient has a history of depression and irregular menstruation, such delayed obstruction indicates an impending sign of metrorrhagia and metrostaxis (Beng Lou). If the interval between periods shortens daily, the bleeding will be mild; if the interval lengthens daily, the bleeding will be severe. It is advisable to pre-administer Ba Zhen Tang (Eight Treasure Decoction) to regulate the condition. Otherwise, prolonged stagnation may lead to severe complications from metrorrhagia.

Menstruation begins at around fourteen years old (Two Sevens) when Tian Gui arrives, and ceases at around forty-nine years old (Seven Sevens) when Tian Gui exhausts; this is the normal physiological course. However, if at forty-nine or fifty years old, Tian Gui has not yet exhausted and menstruation continues regularly without other symptoms, it indicates an excess of Blood, and medication should not be used to forcibly stop it. If Tian Gui has passed its time, menstruation becomes irregular, such as missing for three or four months or occurring twice in one month accompanied by lower back and abdominal pain, Dang Gui San (Angelica Powder) is appropriate.
