During pregnancy, if there is pain in the heart and abdomen, spasmodic twitching of the hands and feet, a bluish-cold complexion, profuse sweating like rain, and breath so faint it seems about to cease, this condition is named *Chou* (convulsions/spasms during pregnancy). This arises from excessive labor or exertion that injures the fetal palace, causing Liver Wind and Heart Fire to blaze mutually. The Heart governs the blood vessels and belongs to Fire; the Liver governs the sinews and belongs to Wind. Treatment should aim to calm the pulse, relax the sinews, and nourish Qi and Blood simultaneously. The appropriate formula is Gouteng Tang (Uncaria Decoction). If caused by deficiency of Qi and Blood, Bazhen Tang (Eight-Treasure Decoction) [formula seen previously under the section on Fetal Dizziness] with added Gouteng (Uncariae Ramulus cum Uncis) and Shanzhiren (Gardeniae Semen, stir-fried), each one qian, is appropriate.
Gouteng Tang
Gouteng (Uncariae Ramulus cum Uncis), Danggui (Angelicae Sinensis Radix), Renshen (Panacis Ginseng Radix), Fushen (Poria Cum Radix Pini), Jiegeng (Platycodonis Radix) (each 1.5 qian); Sangjisheng (Taxilli Caulis) (0.5 qian).
Decoct in water and take orally.
If wind-heat is present, add Chaihu (Bupleuri Radix), Huangqin (Scutellariae Radix), Baizhu (Atractylodis Macrocephalae Rhizoma), and Shanzhiren (Gardeniae Semen, stir-fried). If wind-phlegm surges upward, add Zhuli (Bambusae Sap), Dansheng (Arisaematis Cum Bile Rhizoma), and Banxia (Pinelliae Rhizoma, processed, stir-fried until yellow). If acute twitching due to wind-pathogen occurs, add Quanxie (Scorpio) and Jiangcan (Bombyx Batryticatus). If vexation and heat are severe, add Shigao (Gypsum Fibrosum) 1.5 qian. Near delivery, add Guixin (Cinnamomi Cortex) 0.5 qian.
If sudden pain in the heart and abdomen occurs during pregnancy, Danggui San (Angelica Powder), Muxiang San (Costus Powder), or Kujie San (Platycodon Powder) may be selected according to the syndrome. If the pain in the heart and abdomen is colicky and severe, resembling a ghostly strike, massage should not be performed. If the patient feels stifled and nearly dies, or suffers from epistaxis or hematemesis, treatment should focus on regulating and tonifying Zhengqi (Upright Qi). Using Rendongteng (Lonicerae Japonicae Caulis, i.e., Honeysuckle Vine) decocted in water for oral administration yields miraculous effects. Alternatively, boiling Shuai Ai (Artemisiae Argyi Folium, prepared) in water and taking it frequently is also effective.
Danggui San
Danggui (Angelicae Sinensis Radix), Chuanxiong (Ligustici Chuanxiong Rhizoma), Dingxiang (Caryophylli Flos) (each 3 liang); Qingpi (Citri Reticulatae Pericarpium Viride) (2 liang); Wuzhuyu (Evodiae Fructus) (5 qian, soaked in Jiegeng Tang, stir-fried until black).
Grind into fine powder. Take 1 qian per dose, mixed with warm wine.
Muxiang San
Muxiang (Aucklandiae Radix), Zhiqiao (Aurantii Fructus, bran-fried) (each 7.5 qian); Shengdihuang (Rehmanniae Radix) (2 qian).
Process the above into powder. Take 1 qian per dose, mixed with warm wine.
Kujie San
Kujiegeng (Platycodonis Radix, 1 liang, slightly stir-fried); Shengjiang (Zingiberis Rhizoma Recens) (5 qian).
Decoct in water and take orally.
For heatstroke during pregnancy, characterized by vexation, thirst, mental confusion, and fetal instability, Xiangru Yin (Elsholtzia Decoction) is appropriate.
Xiangru Yin
Xiangru (Elsholtziae Herba) (2 qian); Nongpo (Magnoliae Officinalis Cortex, ginger-processed); Bai Biandou (Phaseoli Semens Album, stir-fried) (each 1 qian).
Decoct in water and take warm. If vexation and heat are severe with excessive drinking, add Maidong (Ophiopogonis Radix), Huangqin (Scutellariae Radix), Huafen (Puerariae Radix), Wuweizi (Schisandrae Chinensis Fructus), and Shanzhiren (Gardeniae Semen, stir-fried until black), each 1 qian.
