When menstruation is about to commence, colicky pain occurs in the umbilical and abdominal regions; this is due to sluggish blood flow causing obstruction. It is appropriate to administer Tongjing Tang (Unblocking Menses Decoction).
Tongjing Tang (Unblocking Menses Decoction)
Prepared Rehmanniae Radix (Shu Dihuang), Angelicae Sinensis Radix (Danggui), Chuanxiong Rhizoma (Chuanxiong), White Peony Root (Baishao), Fructus Toosendan (Chuanlianzi, stir-fried), Foeniculum Vulgare Fruit (Xiaohuixiang), Areca Seed (Binglang), Corydalis Rhizoma (Yanhusuo), Aucklandiae Radix (Muxiang) (each 0.21 g).
Decoct in water and take before meals.
If lower back and abdominal pain occur during menstruation accompanied by qi stagnation and blood excess/stasis, it is appropriate to administer Tao Ren Tang (Peach Kernel Decoction).
Tao Ren Tang (Peach Kernel Decoction)
Tail of Angelicae Sinensis Radix (Dangguiwei), Red Peony Root (Chishao), Raw Rehmanniae Radix (Sheng Dihuang), Cyperi Rhizoma (Xiangfu, processed with child's urine), Moutan Cortex (Mudanpi), Safflower (Honghua), Corydalis Rhizoma (Yanhusuo), Peach Kernel (Taoren, pounded separately into a paste and taken with the decoction).
Decoct in water. Just before taking, add the peach kernel paste. Take on an empty stomach. For patients who are thin with internal heat, add Scutellariae Radix (Huangqin) and Coptidis Rhizoma (Huanglian); for patients who are obese with excessive phlegm-dampness, add Aurantii Fructus (Zhike), Atractylodis Macrocephalae Rhizoma (Cangzhu), and Pinelliae Tuber (Banxia).
If hard masses form in the lower abdomen during menstruation, or if one feels as though a strip like a soap bean crosses horizontally, with unbearable pain, bluish-yellow complexion, and loss of appetite, it is appropriate to administer Yanhu San (Corydalis Powder). Recovery can be expected within half a month.
Yanhu San (Corydalis Powder)
Corydalis Rhizoma (Yanhusuo, 120 g), Ash of Human Hair (Toufa Hui, 12 g). Grind into powder, mix with wine, and take orally.
If menstruation stops halfway with residual blood remaining, causing abdominal pain, sometimes with fever and sometimes without, this indicates that both qi and blood are full and obstructed. It is appropriate to administer Honghua Danggui Tang (Safflower and Angelica Decoction) to break up the residual blood, after which the heat will subside naturally.
Honghua Danggui Tang (Safflower and Angelica Decoction)
Safflower (Honghua), Angelicae Sinensis Radix (Danggui), Achyranthis Bidentatae Radix (Niuxi), Calamus Wood (Sumu) (each 3 g), Chuanxiong Rhizoma (Chuanxiong, 1.5 g), Aurantii Fructus (Zhike, 1.8 g, stir-fried with bran), Curcuma Zedoaria Rhizoma (Ezhu), Red Peony Root (Chishao), Sparganii Rhizoma (Sanleng), Genkwa Flos (Yuanhua) (each 2.4 g).
Decoct in water and take before going to bed.
If menstruation stops halfway, accompanied by tidal fever over the whole body, headache, thirst, and painful urination, this is caused by injury from consuming raw and cold foods, leading to blood stasis and obstruction. Since there is residual blood internally, tonifying agents are contraindicated; it is appropriate to administer Ezhu Tang (Curcuma Zedoaria Decoction).
Ezhu Tang (Curcuma Zedoaria Decoction)
Curcuma Zedoaria Rhizoma (Ezhu), Sparganii Rhizoma (Sanleng), Safflower (Honghua), Calamus Wood (Sumu), Achyranthis Bidentatae Radix (Niuxi).
Decoct in water and take on an empty stomach.
If pain occurs after menstruation has completely ceased, accompanied by numbness of the hands and feet, this is due to deficiency-cold in the abdomen. In cases of severe blood deficiency, it is appropriate to administer Siwu Tang (Four Substances Decoction) plus Evodiae Fructus (Wuzhuyu, 3 g, soaked in boiling water and stir-fried).
Siwu Tang (Four Substances Decoction)
Prepared Rehmanniae Radix (Shu Dihuang), Angelicae Sinensis Radix (Danggui) (each 9 g, stir-fried with wine), White Peony Root (Baishao, 6 g, stir-fried with wine), Chuanxiong Rhizoma (Chuanxiong, 3 g).
Use fresh ginger and jujube as guides, decoct in water, and take orally.
