[Commentary] This clause also belongs to the example of treating a standard disease with standard methods.
Menstruation arriving late (delayed menstrual cycle), with pale color and abundant flow, or mixed with white sticky or yellow saliva-like discharge, indicates excessive dampness causing stagnation of Spleen Qi. It is appropriate to strengthen the Spleen and warm the Uterus.

When menstruation is significantly delayed, occurring only once every two months, or accompanied by the passage of clots, this indicates extreme cold in the Lower Yuan (Kidney and Mingmen), leading to a deficiency pattern where blood stasis may eventually form a mass (Zhao). It is appropriate to warm the Blood Sea (Chong Mai) and simultaneously tonify the Fire of Mingmen. The patient will necessarily have cold feet year-round,
with a deep and slow pulse and loose stools.

[Commentary] This is also a major variation of delayed menstruation, just as early menstruation is a precursor to flooding (Beng). Whenever delayed menstruation occurs, one should prevent what it might develop into? Planning ahead allows for less effort and greater success.
Menstruation arrives only once every two months, and the volume is extremely scant, stopping after only three or four drops. The color may be pale, normal red, or purple. The patient has slight chills and fever, or none at all; they may be thirsty or not thirsty.
The body feels no numbness or pain, and there are no masses in the lower abdomen, or if present, they are old, pre-existing masses. The pulse may be floating and large, or fine, choppy, soggy, and weak. There is slight hair loss, the skin seems to gradually lose luster, and the complexion and tongue root lack vibrancy and nourishment.
This is a sign that the blood vessels are about to dry up. Urgently administer Guipi Tang (Spleen-Invigorating Decoction) with added Deer Antler (Lurong) and Donkey-hide Gelatin (Ejiao) to remedy the situation; once dried up, it will be difficult to act.

For extreme cold in the Lower Yuan, add Prepared Aconite Lateralis Radix Preparata (Dan Fuzi) and Cinnamon Twig (Rougui). For floating deficient Yang, add Walnut Kernel (Hutaorou). For spontaneous sweating, add Dragon Bone (Longgu) and Floating Wheat (Fuxiaomai). For night sweats, add Raw Oyster Shell (Sheng Muli) ground into powder, and Ephedra Root (Mahuanggen).
For bone-steaming fever, add Bark of Ground Cherry Root (Digupi) and Raw Rehmannia Root (Sheng Dihuang). For severe thirst, add Ophiopogon Tuber (Maidong Rou) and Dendrobium Stem (Chuan Shihu). For loose stools, add Alpinia Oxyphylla Fruit (Yizhiren) and Poppy Shell (Yuke). For dampness overwhelming the Middle Jiao (Spleen/Stomach) causing chest stuffiness, add Six-Herb Medicated Leaven (Liushenqu), Tangerine Peel (Chenpi), and Cardamom Seed (Kouren).
For rebellious Liver Yang, add Abalone Shell (Shijueming), White Peony Root (Baishao), Fritillaria Bulb (Xiang Beimu), and Black Gardenia Fruit (Hei Shanzhi). For cough, add Adenophora Root (Nanshashen) and Sweet Apricot Seed Paste (Tian Xingni). When encountering concurrent symptoms, add ingredients as appropriate; they cannot all be listed. The general principle is to avoid herbs that consume blood or break Qi.
