(Hand-Shaoyin Heart Meridian belongs to the Zang organs)
The substance of the Heart belongs to Fire. It is located in the South. Its color manifests as red. Its anatomical location is at the depression below the chest, between the bifurcation of the costal cartilages. The forehead, palms of the hands, and soles of the feet are all under its jurisdiction. The Heart requires blood to nourish it, enabling it to operate wisdom and utilize talent. The Heart has no Exterior syndromes; all conditions belong to the Interior.
Heart Deficiency refers to insufficient Blood. The pulse at the Left Cun position must be weak. Symptoms include Jingji (Palpitations with fright), inability to lie down, amnesia, deficiency pain, Zhengchong (Severe palpitations), and spermatorrhea.
Jingji refers to a state of constant fearfulness, where the Shen (Spirit) loses its residence. Treatment is primarily with Qifu Yin (Seven Blessings Decoction) or Mizhi Anshen Wan (Secret Purpose Calming Spirit Pill). Inability to lie down results from excessive overthinking, causing the Shen to fail to store itself. Treatment is primarily with Guipi Tang (Restore the Spleen Decoction) or Anshen Dingzhi Wan (Calming Spirit Stabilizing Will Pill). Amnesia results from the failure of communication between the Heart and Kidney, leading to insufficient充盈 (fullness) of the Shen. Treatment is primarily with Guipi Tang or Shibu Wan (Ten Tonics Pill). Deficiency pain feels like hunger without actual need for food, as if one needs to press on the heart, and relief is found by pressing with the hand. Treatment is primarily with Yangshen Maidong Tang (American Ginseng and Ophiopogon Decoction). Zhengchong refers to Qi counterflowing upward from below, causing palpitations and unrest. Treatment is primarily with Guipi Tang. Spermatorrhea may occur with dreams or without dreams, resulting from insecurity of the Heart and Kidney. Treatment is primarily with Qingxin Wan (Clear Heart Pill) or Shibu Wan.
Heart Excess refers to the invasion of pathogenic factors. The Heart itself does not accept pathogens; rather, the Pericardium (Bao Luo) accepts them. The pulse at the Left Cun position must be Wiry and Large. Symptoms include Qi stagnation, Blood pain, Fluid retention, Phlegm confusion, Summer-heat closure, and Worm biting.
Qi stagnation may result from food distension or anger striking upward, causing vexation, oppression, and pain. Treatment is primarily with Chenxiang Jiangqi San (Aquilaria Regulating Qi Powder). Blood pain involves Blood coagulation within, with fixed pain locations that feel like knife stabs or needle pricks upon turning or moving. Treatment is primarily with Shounian San (Hand-Picking Powder). Fluid retention presents as dry heaving, spitting saliva, pain, and water sounds in the abdomen. Treatment is primarily with Xiaobanxia Jia Fuling Tang (Minor Pinellia plus Poria Decoction). If a fluid cyst forms, Cangzhu (Atractylodis Rhizoma) is added, known as Daochang Fa (Stomach Emptying Method). Phlegm confusion involves stubborn Phlegm obstructing and closing the orifices, leading to loss of consciousness. Treatment involves administering Qingge Jian (Clear Diaphragm Decoction). Summer-heat closure presents with sweating, panting, and coma/oppression. Initially administer Xiaoshu Wan (Summer-Heat Dispelling Pill), followed by Xiangru Yin (Elsholtzia Decoction) plus Yiyuan San. Worm biting presents as pain occurring when hungry, with a pale face and red lips. Treatment is primarily with Huachong Wan (Expel Worms Pill).
Heart Cold: The pulse at the Left Cun position must be Slow. Symptoms include sudden severe pain.
Sudden severe pain involves cold limbs and cold internal organs, continuous and unceasing. Treatment is primarily with Jiangfu Tang (Ginger and Aconite Decoction) plus Rougui (Cinnamon Bark).
Heart Heat: Caused by Fire forcing. The pulse at the Left Cun position must be Rapid. The tip of the tongue is red. Symptoms include eye pain, Chongshe (Double Tongue), Mushe (Wooden Tongue), irritability, inability to lie down, Diankuang (Manic-depressive psychosis), Zhanyu (Delirious speech), Chizhuo (Red turbid urine), and Hematuria.
Eye pain presents with redness, swelling, and photophobia. Treatment is primarily with Daochi San (Guide Out Red Powder) plus Lianqiao (Forsythiae Fructus), Juhua (Chrysanthemi Flos), and Chantui (Cicadae Periostracum). Chongshe and Mushe are treated primarily with Xiexin Wan (Drain Heart Pill). Irritability is treated primarily with Xiexin Wan plus Zhujuanxin (Bamboo Heart). Inability to lie down due to Summer-heat invading the Heart is treated primarily with Daochi San plus Yiyuan San. Diankuang presents with discarding clothes, cursing, and shouting. Treatment is primarily with Shengtieluo Yin (Raw Iron Filings Decoction). Zhanyu results from Pathogenic Heat attacking the Heart. Treatment is primarily with Xiexin Wan. Chizhuo is treated primarily with Bixie Fenqing Yin (Dioscoreae Hypoglaucae Separating Turbidity Decoction) plus Dengxincao (Junci Medulla) and Danshen (Salviae Miltiorrhizae Radix et Rhizoma).
