The Phlegm-Fire Syndrome is what is commonly referred to as Acute Convulsive Fever (Ji Jing Feng) in popular terms. In children, contracting Wind-Cold or having accumulated milk and food can both generate Phlegm. When Phlegm accumulates, it transforms into Fire; alternatively, contracting Summer-Heat also generates Fire. If failing to clear heat and resolve the pathogen promptly, Fire will ascend, carrying Phlegm with it. The obstruction of Phlegm and Fire at the upper jiao closes the Lung orifices, thereby closing all orifices. Symptoms include staring eyes, panting, coma, and unresponsiveness. Moreover, excessive Fire causes Liver dryness and tendon tension, leading to spasms, convulsions, tremors, opisthotonos, and upward deviation of the eyes—these constitute the "Eight Signs." This condition arises fundamentally from the stagnation of Phlegm and Fire stirring up internal Liver Wind. During episodes of rigidity and arching of the back, one should only support the child gently by hand; do not hold tightly with force, as this may damage the sinews and meridians, resulting in permanent disability. At the onset, use Tongguan San (Passing Through Obstruction Powder) to induce sneezing; awakening occurs upon sneezing. For mild cases, utilize Lihuo Jiangtan Tang (Fire-Reducing and Phlegm-Lowering Decoction); for severe cases, use Qingge Jian (Clear Diaphragm Decoction) added with Shichangpu (Acorus Rhizome) and Zhuru (Bamboo Shavings), or Baolong Wan (Embracing Dragon Pill). After regaining consciousness, administer Qingre Yangxue Tang (Heat-Clearing and Blood-Nourishing Decoction).
The Wood-Rebelling-Earth Syndrome corresponds to Chronic Convulsive Fever (Man Jing Feng) in common parlance. Children contracting Summer-Heat or Cold, or suffering from dietary injury to milk and food, may experience vomiting and diarrhea, or simultaneous vomiting and purging. Prolonged occurrence weakens Spleen Earth, allowing Liver Wood to overact upon it. Stool gradually turns greenish, the face appears sallow with a bluish tint, hands and feet exhibit slight twitching and weakness, and spirit is listless and depressed, marking the formation of Slow Spleen Wind. At the initial stage, Yigong San (Benefiting Work Powder) is appropriate. If vomiting is present, add Huoxiang (Patchouli Herb) and Wei Jiang (Baked Ginger). If the illness has persisted for several days and stool shows green color, add Muxiang (Costus Root) or Rougui (Cinnamon Bark). If hands and feet are cold, pulse is faint and fine, and lips and tongue are pale and flaccid, this indicates impending collapse of Yang Qi. Urgent administration of Fuzi Lizhong Tang (Aconite Warming Middle Decoction) is required to warm the middle jiao and restore Yang Qi, offering a chance of rescue. Pathological changes in the five Zang organs are generally slow, but alterations in Spleen disease are extremely rapid; some cases progress to collapse within a day and night of vomiting and diarrhea. Caution is imperative; do not overlook this.
Sudden Great Fear represents true fright. Children have insufficient Qi and Blood, and their Heart Spirit is timid and weak. Upon encountering a startling event, the Soul and Spirit are shaken and terrified, leading to abnormal behavior. They may startle awake jumping at night and remain easily startled during the day. Treatment should focus on calming the Soul and Spirit, and consolidating Heart Qi. Qifu Yin (Seven Blessings Decoction), Mizhi Anshen Wan (Secret Purpose Calming Spirit Pill), and Anshen Dingzhi Tang (Calming Spirit Stabilizing Will Decoction) are all suitable. For palpitations and fright due to latent Heat in the Heart, use Qiwei Anshen Wan (Seven-Ingredient Calming Spirit Pill). After the spirit stabilizes, if Qi deficiency exists, employ Sijunzi Tang (Four Gentlemen Decoction) to tonify Yang; if Blood deficiency exists, use Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) to nourish Yin. Incorrectly administering Zhusha Zhenjing Wan (Cinnabar Calming Convulsion Pill) consumes Heart Blood, causing the condition to worsen progressively, allowing Liver Wind to hyperactive due to deficiency, making the situation uncontrollable. Exercise extreme caution.
