The *Neijing* (Classic of Internal Medicine) states: If there is a fall from height causing injury, leading to the retention of evil blood (stasis) within the body, abdominal distension, and obstruction of urination and defecation, one should first administer purgative medicines to guide their expulsion. In cases of falling from a horse or stumbling, dropping from a high place, being beaten or struck causing internal trauma, or suffering severe swelling and pain from beating without skin rupture or bleeding, one should initially use the Main Formula to promote qi circulation, remove blood stasis, and eliminate stagnant blood. For metal blade injuries where the skin is broken and bleeding is excessive, only blood-invigorating and harmonizing medicines for pain relief are suitable; subsequently, qi-regulating, blood-nourishing, and stomach-qi-tonifying medicines should be added. Strong blood-breaking and stasis-expelling drugs must be strictly avoided. For sprains and contusions resulting in fractures or dislocations, immediately dissolve alum in water, dip cloth into the solution, and apply hot compresses to the injured area until pain subsides. Reduction should be performed promptly; if delayed, muscle swelling and blood coagulation will cause severe pain. One must carefully examine the alignment of bones, either gently palpating and kneading, or assessing the position of left and right bones to determine whether they require straightening or angling, ensuring the joints are level and aligned. Based on the condition of the bone fissures, push the bone back into the joint socket. Alternatively, several people may use strong force to pull and align the bone, restoring it to its original position, then bind it with ramie rope or fix it with fir wood boards. Wash and change dressings every three to five days. Avoid exposure to wind, as this renders the injury incurable. For arm dislocations, traction should be applied laterally. For skull fractures, cut the hair and gently rub the area to flatten it. If the skin is intact, apply Wulong San around the injury; if broken, fill the wound with Fengliu San and cover externally with oiled paper or cotton paper. For foreign bodies embedded in bone, surround the swelling with Wulong San, leaving the wound open, and fill it with Fengliu San. In situations lacking medical supplies, simply incise the wound, introduce urine into the wound and have the patient drink it to survive.
Main Formula: Dahuang (Rhei Radix et Rhizoma), Zhike (Aurantii Fructus Immaturus) (each 2 qian); Dangguiwei (Angelicae Sinensis Radix caudex), Houpo (Magnoliae Officinalis Cortex), Chenpi (Citri Reticulatae Pericarpium), Gancaoshao (Glycyrrhizae Radix et Rhizoma tip), Honghua (Carthami Flos), Sumu (Sappan Lignum), Mutong (Akebiae Caulis) (each 1 qian). Decoct half a zhong of water for a few boils, filter, and take orally.
If bowel movement remains unobstructed, add Mangxiao (Natrii Sulfas) (1 qian); if already patent, remove Dahuang. If blood stasis does not descend, add Taoren (Persicae Semen), Rougui (Cinnamomi Cortex), Heiqiu (Momordicae Fructus), Liuji Nu (Artemisiae Annuae Herba), Tu Niuxi (Cyathulae Radix), etc. For mild cases without stasis, or those with significant blood loss, or for the elderly and young, reduce the dosage of Dahuang, Zhike, Sumu, Mangxiao, etc., and increase Xuanhusuo (Corydalis Rhizoma), Chishao (Paeoniae Radix Rubra), Danpi (Moutan Cortex), Zelan (Lycopus Herba), Gusuibu (Drynariae Rhizoma), etc. To relieve pain, grind real Rushiang (Boswelliae Resina) and Moyao (Commiphora Myrrha Resina) into fine powder and mix into the decoction; subsequently add Danggui (Angelicae Sinensis Radix), Chuanxiong (Chuanxiong Rhizoma), Baizhi (Angelicae Dahuricae Radix), Muxiang (Aucklandiae Radix), etc. For weak individuals, add Renshen (Ginseng Radix et Rhizoma)... [text missing]. Strictly avoid raw/cold herbs. Drinking pure grain wine and eating beef will exacerbate pain.
Wulong San: Used for falls causing bone fractures, comminution, or dislocation. First wash with decocted green onion soup. After reducing the bone to proper alignment, spread on soft silk or leather paper and apply externally, binding with wooden splints. Also applicable for insect and beast bites. Apply Fengliu San to open wounds.
Shanjiapia (Manis Squama, stir-fried yellow), Dingpi (Zanthoxyli Pericarpium; if genuine unavailable, use Dingxiang/Syzygium Aromaticum Flos, each 6 liang), Tudanggui (Herba Actaeae Spicatae, 2 liang), Baicaoshuang (Carbonis Cytisi, separately ground), Pipaye (Eriobotryae Folium, de-hair, each 5 qian).
Grind all into fine powder. Mix with ginger juice water or Shengdihuang juice (Rehmanniae Radix) and ginger juice to form a paste for external application.
Fengliu San: Treats soft tissue injuries with continuous bleeding, and head wounds infected by wind pathogen. Fill and apply medicine to the wound.
Xuejie (Dragon's Blood), Rushiang (Boswelliae Resina, ground together with Dengxincao/Junci Medulla), Moyao (Commiphora Myrrha Resina) (each 5 qian), Jiangxiangjie (Sappan Lignum node), Guiwei (Angelicae Sinensis Radix caudex) (3 qian), Longgu (Fossilia Ossis Mastodi, five-flower variety), Honghua (Carthami Flos) (each 2 qian), Sumu (Sappan Lignum) (1 qian), Dengxincao (Junci Medulla) (one bundle).
Grind each into fine powder and mix evenly. Take a small amount, sprinkle dry onto the wound; apply more if bleeding persists. If the mixture dries and hardens, mix with clear sesame oil for external application.
Linxi Fang: For severe injuries, decoct and wash before applying other medicines.
Jingjiesui (Schizonepetae Spica), Tudanggui (Herba Actaeae Spicatae) (each 3 qian), Dacong (Allii Fistulosi Bulbus, several stalks) pounded together with Shengjiang (Zingiberis Rhizoma Recens) until mashed. Decoct in water and wash the affected area while hot.
Xiaohong Wan: For skin rupture, bone protrusion, tendon severance, and post-injury joint contracture. Frequent administration nourishes sinews, bones, qi, and blood.
Gusuibu (Drynariae Rhizoma, processed with ginger), Tudanggui (Herba Actaeae Spicatae), Chuanwu (Aconiti Radix Praeparata), Yangbapi (Celtis Sinensis Cortex), Shaoyao (Paeoniae Radix Alba) (each 6 qian), Chuanxiong (Chuanxiong Rhizoma), Pengzhu (Curcumae Rhizoma, roasted) (each 3 qian), Rougui (Cinnamomi Cortex), Xixin (Asari Radix et Rhizoma) (each 4 qian), Ganjiang (Zingiberis Rhizoma, processed), Dingxiang (Syzygium Aromaticum Flos) (each 2 qian), Xuduan (Dipsaci Radix) (1 qian), Fuzi (Aconiti Lateralis Radix Praeparata, semi-processed) (3 qian), Rushiang (Boswelliae Resina, separately ground), Moyao (Commiphora Myrrha Resina, ground) (each 5 fen).
