Vitiligo refers to the appearance of purple or white patches on the body, face, neck, and skin, without pain or itching. This is caused by wind pathogen invading and knotting between the skin and interstices (skin pores and muscle texture), leading to disharmony of Qi and Blood circulation.
Purpura-like vitiligo often occurs on the four limbs or presents as purple nodules on the body; this results from the stagnation and accumulation of wind-heat pathogen. The treatment method allows for the same prescription for both purpura-like vitiligo and vitiligo.
Main Prescription: Prepared Atractylodis Rhizoma (Cangzhu), Sophorae Flavescentis Radix (Kushen), Polygoni Multiflori Preparata He Shou Wu), Schizonepetae Spica (Jingjie Sui) (all in equal parts). Process into pills. The administration method is identical to the main pill prescription for Leprosy (Lifeng); it can also be taken for Vitiligo.
Four Spirits Powder (Sishen San): Treats purple and white vitiligo.
Ingredients: Realgar (Xionghuang), Orpiment (Cihuang), Sulfur (Liuhuang), Alum (Baifan) (all in equal parts).
Usage: Grind into powder. First, take a hot bath until sweating, wash the affected area clean with soap, then apply the powder by rubbing with cut and crushed fresh ginger root. (Repeat washing and rubbing every three days; five treatments will result in cure.)
Simple Formula: For Vitiligo.
Grind Sulfur into powder, mix with egg white, spread on cloth, and dry in the sun. When the patient sweats, rub the affected area with this cloth.
Another method: Realgar, Sulfur, Minium (Huandan), Lead Monoxide (Miduoseng), Arisaema Cum Bile (Nanxing) (all in equal parts), grind into powder, and apply to cloth as described above (also effective for wiping sweat spots.)
Note: This condition has three distinct etiological categories. If a sore or ulcer persists for a long time, covered by a white scab that blocks the pores, causing Yang heat to stagnate internally, generating wind due to extreme heat, this is an internal cause. If various sores do not heal, and moxibustion is used, causing heat toxin to attack inward and potentially generate wind, this is a neither-internal-nor-external cause. If injured accidentally by knives or scissors, or bruised/cracked from falls, or suffering from toxic sores, and exposed to wind during washing or applying medication, this is an external cause. External causes are more common. If the injury is small, like a bean, and suddenly attacked by wind pathogen at the muscle-skin level, it spreads to the meridians, causing alternating chills and fever, convulsions of the hands and feet, trembling and restlessness; gradually progressing to opisthotonos (body arched backward like a bow), trismus (lockjaw), inability to eat, severe abdominal pain, and if the pathogen enters the zang organs, death occurs within seven days. If the medicine hits the disease accurately, two or three out of ten may survive. Patients often fail to consider this consequence. If physicians lack experience and misdiagnose it as other diseases, none of the ten will survive. There are many among the wealthy who die without knowing their illness. A floating pulse indicates Taiyang channel involvement; a long and forceful pulse indicates Yangming channel involvement; a wiry pulse indicates Shaoyang channel involvement; a deep and rapid pulse indicates interior syndrome. Treatment is similar to that for Stroke (Zhongfeng) and Cold Damage (Shanghan).

A case: A person had a small boil on the arm, which was lanced and bled, leading to wind invasion. He was warned of serious harm but did not believe it. At night, his arm felt like it was immersed in water, and the condition became critical. After administering large doses of harsh attacking medicines, his chest and back suddenly swelled like a bowl. Then, Ten Prescriptions of Zhenren Huoming Yin were prepared, combined with Calculus Bovis (Niuhuang), administered over three days, resulting in stabilization.
