Patient Zhu, aged eighty, arrived in the capital by carriage. During the journey, he was extremely exhausted. After ten days, he suffered from constipation. The pulse was surging and large (Hong Da), but the right Chi position was deficient (Xu). I stated that the Large Intestine governs fluids; this is a case of fluid exhaustion in the Yangming channel, presenting as heat-type constipation (Re Mi). It is suitable to use Run Chang Wan (Moisten the Intestines Pill). Because the patient is advanced in age with dry-heat in the blood and yin, I modeled the prescription after Li Dongyuan's Run Zao Tang (Dryness-Moistening Decoction). It used Sheng Di Huang (Raw Rehmannia Root), Shu Di Huang (Prepared Rehmannia Root), Huo Ma Ren (Hemp Seed), Tao Ren (Peach Kernel), Dang Gui (Angelica Sinensis), and Hong Hua (Safflower), taken with honey. It was effective.
Brother Fang, suffering from post-illness constipation with a deficient pulse, was treated with moistening and tonifying agents combined with methods to regulate ascending and descending Qi. Lu Shen (Codonopsis Pilosula), Shu Di Huang (Prepared Rehmannia Root), Dang Gui (Angelica Sinensis), Sheng Ma (Cimicifugae Rhizoma), and Xing Ren (Apricot Seed) were used. He recovered after taking it. (Shu Di Huang can be doubled to approximately one liang.)
Member of the Shi family, elderly with wind-type constipation (Feng Mi), complicated by hemorrhoidal bleeding, swelling, and pain. The pulse was surging yet deficient. Zi Zao Yang Ying Tang (Nourish Dryness and Nourish Blood Decoction) was used, added with Jing Jie (Schizonepetae Herba, stir-fried with vinegar), Di Yu (Sanguisorbae Radix, stir-fried with wine), Hu Ma (Sesamum Indicum Seed), Sheng Ma (Cimicifugae Rhizoma), and Cong Rong (Cynomorii Herba, steamed). Honey pills were made and taken, showing effectiveness. (Zi Zao Yang Ying Tang is seen under "Dryness Syndromes".)
Member of the Li family, the Fu organs lost their transmission function, causing hypochondriac pain, epigastric distension, and difficult defecation. All were due to obstruction of Qi mechanism. It was appropriate to first guide the Fu organ Qi. Xing Ren (Apricot Seed), Su Geng (Perilla Stem), Hou Po (Magnolia Bark), Yu Jin (Curcumae Radix), Ju Bai (Citri Reticulatae Pericarpium Album), Yu Li Ren (Prunus Lasiocarpa Seed), and Dang Gui (Angelica Sinensis) were used. After four doses, pain and distension ceased. He was also instructed to drink cow's milk, which facilitated smooth defecation. Later, he experienced drilling pain in the left hypochondrium, which stopped after hot soup baths. This indicated Liver Qi stagnation spreading from the Zang to the Fu. Wheat bran was stir-fried and applied as a warm compress. Yan Hu Suo (Corydalis Rhizoma, stir-fried with wine), Bai Shao (White Peony Root, stir-fried), Dang Gui (Angelica Sinensis), and Jin Ju Pi (Kumquat Peel) were decocted. Jiang Xiang (Aquilariae Lignum Resinatum) and Mu Xiang (Aucklandiae Radix), both ground into juice, were taken orally, and the condition subsided.
Ancestral relative, having bowel movements only once every ten-plus days. Someone advised her to take Da Huang (Rhei Radix et Rhizoma), making the constipation worse. Both Chi pulses were deep and large. This was sinking of clear Qi. Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) was used, removing Chai Hu (Bupleuri Radix) and Bai Zhu (Atractylodis Macrocephalae Rhizoma), adding Tao Ren (Peach Kernel) and Xing Ren (Apricot Seed). After several doses, she returned to normal.
Member of the Deng family, Yin deficiency with Yang hyperactivity leads to Ben Beng (profuse uterine bleeding); prolonged Ben Beng becomes Lou (persistent spotting), indicating deficiency of the Chong and Ren vessels. According to the history, in May, grief and overthinking caused blood to flow out in clots. Thereafter, red and white discharge alternated. By mid-winter, the spotting had not stopped, and medication was ineffective. Recently, there was a heavy bearing-down sensation in the lower abdomen, and pulling pain in both flanks like being bound. Urination became scanty or absent at night. Some attributed it to rough Qi and obstructive pain. Qian Cao (Rubiae Cordifoliae Radix), Gui Xu (Angelica Sinensis Root Tip), and Tao Ren (Peach Kernel) were used to unblock collaterals. Without effect, others attributed it to blood deficiency with slipping leakage. Pu Huang (Typhae Pollen), Shi Zhi (Halloysitum Rubrum), and Shi Ying (Quartzum) were used to stabilize and secure. The strangury and pain worsened, and the pulse was deep and wiry. I stated that this syndrome is due to bladder Qi sinking from prolonged leakage. Unblocking collaterals increases leakage through the pores; stabilizing causes the uterus to sag further. The method should be warming and lifting, securing and containing, which aligns with the principles. Sheng Ma (Cimicifugae Rhizoma, 6 fen), Tu Si Bing (Cuscutae Semen Cake), Chi Ling (Polyporus Umbellatus, each 3 qian), Yan Hu Suo (Corydalis Rhizoma), and Dang Gui (Angelica Sinensis, all stir-fried with vinegar, each 2 qian), E Jiao (Donkey-Hide Gelatin), Zong Hui (Ligum Palmare, each 1.5 qian), Hui Xiang (Foeniculum Vulgare Fruit), Bu Gu Zhi (Psoraleae Fructus, all stir-fried with wine, each 1 qian), and Sha Yuan Zi (Astragali Complanati Semen, 2 qian) were used. After one dose, urination occurred and pulling pain stopped. After several doses, strangury and leakage were eliminated.
