Leukorrhea is caused by the flow of damp-heat and turbid qi into the Dai Mai (Belt Vessel), flowing continuously downward, hence named leukorrhea. It is common in women. Red leukorrhea belongs to heat syndrome, caused by blood deficiency generating internal fire. White leukorrhea belongs to dampness syndrome, caused by Qi deficiency generating phlegm. There are also cases of mixed five-color discharge, often injured by the Six Evils (Wind, Cold, Summerheat, Dampness, Dryness, Fire) and Seven Emotions (Joy, Anger, Worry, Pensiveness, Sorrow, Fear, Fright). If sliding and uncontrolled, it leads to soreness and weakness of the waist and knees. Treatment should regulate the Spleen and Kidney, or use lifting methods, or astringent methods. Furthermore, one must distinguish between three categories: white leukorrhea, white turbidity, and white spermatorrhea (Bai Yin). White leukorrhea refers to discharge that is thick, sticky, clear, and cold; this originates from the Uterus (Bao Gong) and is the residual essence. White turbidity refers to turbid qi from the Stomach seeping into the Bladder; it is the turbidity of water. White spermatorrhea refers to slippery ejaculation after urination, with little discharge; this occurs when male essence cannot be retained after sexual intercourse.
According to Zhang Jingyue, there are six causes for leukorrhea syndromes. First, if the mind is agitated and Heart Fire is restless causing leukorrhea, one should first clear the fire. Use Zhusha Anshen Wan (Cinnabar Calming the Spirit Pill) or Qingxin Lianzi Yin (Heart-Clearing Lotus Seed Decoction). If there is no pathogenic fire, but only Heart deficiency causing leukorrhea, use Miyuan Jian (Secret Origin Decoction) or Renshen Wan (Ginseng Pill). Second, for excessive sexual activity leading to slippery and uncontrolled leakage, use Miyuan Jian, Gujing Wan (Essence-Securing Pill), or Suojing Wan (Essence-Locking Pill). Third, for obstructed sexual relations where the essence pathway reverses causing turbid discharge, initially use Weixi Wan (Victory and Blessing Pill), and for chronic cases, use Guyin Jian (Nourishing Yin Decoction). Fourth, for damp-heat flowing downward causing turbid discharge, the pulse will likely be slippery and rapid, accompanied by irritability, thirst, and heat. Use Baoyin Jian (Protecting Yin Decoction) or Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder); if heat is severe with stranguria and red color, use Longdan Xiegan Tang (Gardenia and Gentian Liver-Draining Decoction). Fifth, for Yuan Qi (Primary Qi) deficiency causing leukorrhea, use Shoupi Jian (Longevity Spleen Decoction), Qifu Yin (Seven Blessings Decoction), or Shiquan Dabu Tang (Ten Complete Great Tonifying Decoction). If Yang Qi is deficient and cold, with a faint and choppy pulse, abdominal pain, and clear cold white discharge, use Jiajiu Zi Wan (Chinese Chive Seed Pill). For Spleen and Kidney Qi deficiency with sinking, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) or Guipi Tang (Restore the Spleen Decoction). For initial turbid discharge with heat and stranguria, use Dali Qingyin (Great Clearing Decoction); for initial cases without fire but with stranguria, use Xiaoli Qingyin (Small Clearing Decoction) or Wuling San (Five-Ling Powder). For Liver Meridian furious fire flowing downward, use Jiawei Xiaoyao San, or Longdan Xiegan Tang for severe cases. If excessive use of cold-cooling herbs leads to lower Jiao insecurity, use Bixie Fenqing Yin (Grass Leaf and Separating Turbidity Decoction - *Note: Text says "Bi Li Qing Yin", likely referring to Bixie Fenqing Yin*). For Yuan Qi deficiency and cold with sinking, use Buzhong Yiqi Tang. For Spleen dampness flowing downward, use Liujunzi Tang (Six-Gentleman Decoction) or Guipi Tang. For chronic non-healing cases with deficiency, slipping, and sinking, use Miyuan Jian or Lingzhu Tusizi Jian (Poria, Atractylodes, and Dodder Seed Decoction). Generally, obese patients with leukorrhea have damp-phlegm; use Yueju Wan (Resolve Depression Pill) added with Huashi (Talcum), Haishi (Floating Quartz), Gafen (Clam Shell Powder), Fuling (Poria), Banxia (Pinellia), and Chunpi (Toona Bark) made into pills. Thin patients have heat-phlegm; use Dabu Wan (Great Tonifying Pill) added with Huashi, Baiguiban (Tortoise Plastron), and Chunpi. Additionally, for postpartum excessive blood loss with dripping white leukorrhea, use Weisheng Tang (Health-Preserving Decoction). For long-standing unceasing cases with weak and powerless pulses, use Guzhen Wan (Solidify True Pill), Yuguan Wan (Jade Gate Pill), Shenshen Tang (Ginseng Decoction - *Text implies Ginseng related*), Keying Wan, or Mizhen Dan (Secret True Elixir). Any of these can be selected according to circumstances.
