(Note: Chancre)
Those suffering from hernia must not consume waxberries (Yangmei) together with distilled liquor; doing so will cause immediate recurrence, which is incurable.
The seven types of hernias present different symptoms and require distinct treatments. For example, Cold Hernia: the scrotum is cold, hard as a stone, the penis does not erect, or there is pain radiating to the testicles. This results from sitting or lying in damp places or wading through water; it should be treated with purgatives that dispel dampness. Water Hernia: the scrotum swells like crystal, sweating occurs in the genital area, itching leads to yellow fluid discharge upon scratching, or pressing the lower abdomen produces sound. This results from sexual intercourse while drunk, exposure to wind after sweating, and the accumulation of cold-dampness in the scrotum; it should be treated with purgatives that drain water. Sinew Hernia: swelling of the penis, ulceration, pus discharge, or pain; tenesmus and sinew contraction; or penile pain. Extreme pain may lead to itching, protrusion that cannot retract, and white discharge resembling semen during urination. This is caused by evil arts in sexual activity; it should be treated with agents that descend Heart Fire. Blood Hernia: shaped like a cucumber, located on both sides of the lower abdomen at the ends of the transverse bone, commonly known as chancre. This results from repeated exposure to the intense heat of spring and summer, exertion in sexual activity, overflow of Qi, infiltration into the bladder and scrotum, forming an abscess; it should be treated with blood-regulating purgatives. Qi Hernia: extends from the kidney region to the scrotum, causing distension due to crying, anger, or Qi stagnation; it should be treated with Qi-dispersing purgatives. Fox Hernia: shaped like a tile, entering the abdomen when lying down and descending when walking, moving up and down unpredictably, confusing like a fox, hence the name; it should be treated with agents that expel Qi and unblock meridians. [Missing Character] Hernia: the scrotum swells large as a measuring vessel, without itching or pain, arising from dampness; it should be treated with dampness-dispelling purgatives.
Hernia originates in the Liver Meridian; it should be unblocked rather than obstructed, and has absolutely no connection to the Kidney Meridian. Some are formless and soundless, some have form like melons, some make sounds like frogs; these are hernia diseases. In the initial stage, damp-heat stagnates in the meridians for a long time, followed by external constriction by cold air preventing dispersion, thus causing pain. One must not rigidly diagnose this as a cold syndrome. It requires a combination of cold and hot properties, using Shenxiao Tang with modifications. Chuanwu (Aconiti Radix Preparata) disperses cold air, Zhizi (Gardeniae Fructus) clears damp-heat; both are primary drugs for the Lower Jiao, acting very quickly.
Wood Hernia (also known as Wood Kidney)
When the Heart Fire descends, the Kidney Water is assured to be warm; when True Yang descends, the Kidney Water is assured to be harmonious. Warm and harmonious, how can there be "Wood Excess"? Wood excess injures the Kidney! Therefore, dry herbs should not be used exclusively. Instead, methods of warming, harmonizing, dispersing, and draining should be employed to expel pathogens, thereby eliminating them and promoting the flow of Ying and Wei.
