During pregnancy, Zangzao (Visceral Agitation) manifests as sadness and a desire to cry, with mental states resembling possession by spirits, accompanied by frequent yawning and stretching. This is because the emotional expression of the Lung is grief; fetal heat flares upward, causing the Lung Qi to lose self-control, resulting in sadness. This belongs to the category of Lung Dryness disease.
Before and during pregnancy, Qi and Blood gather to nourish the Fetal Origin, preventing Body Fluids from adequately moistening, which leads to Dryness of the Lung. Treatment should tonify the Mother Organ (Spleen). Zhang Zhongjing used Gancao Xiaomai Dazao Tang (Licorice, Wheat, and Jujube Decoction) as the primary formula. Since Licorice and Jujube tonify Spleen Qi, treating with sweet and緩 (gentle/moderating) methods, assisted by cooling and purging methods, ensures recovery. Li Zhongzi (Zi Xian) used Danzhuru Tang (Bamboo Shavings Decoction) combined with Bazhen Tang (Eight Treasures Decoction). The Eight Treasures Decoction tonifies Qi and Blood, expanding upon theories not previously expounded by earlier scholars. It also treats Zangzao-induced weeping, as well as involuntary laughter or crying. The method involves burning red jujubes until carbonized (preserving their nature), and administering them with rice water.
Gancao Xiaomai Dazao Tang (Licorice, Wheat, and Jujube Decoction): Treats women suffering from persistent Zangzao and sadness.
Gancao (Licorice Root, 3 liang), Xiaomai (Wheat, 1 sheng), Dazao (Jujube, 10 pieces).
Boil the above ingredients in 6 sheng of water, reduce to 3 sheng, and take warm in three divided doses. This formula is sweet, which generates dampness; once dampness is generated, dryness is resolved.
Danzhuru Tang (Bamboo Shavings Decoction): Treats pregnant women with Heart deficiency palpitations and persistent Zangzao sadness. It is also highly effective for severe deficiency heat irritability.
Maidong (Ophiopogon Root, heart removed), Xiaomai (Wheat), Zhibanxia (Prepared Pinellia Tuber, each 1 qian), Renshen (Ginseng Root), Fuling (Poria, each 1 qian), Gancao (Licorice Root, 5 fen).
Add 3 slices of Shengjiang (Fresh Ginger) and 1 Jujube to the decoction, along with a ball-sized portion of Danshuzhu (Light Bamboo Shavings), boil in water, and take orally.
Bazhen Tang (Eight Treasures Decoction) (See Chapter on Various Pains in this volume).
Constipation and dry stools during pregnancy. This occurs because Blood is utilized to nourish the Fetal Origin, leading to Blood deficiency in the Large Intestine. The stool becomes dry and difficult to pass. Although it causes no major harm, defecation is very painful. Runzao Tang (Moistening Dryness Decoction) is the primary treatment.

Ejiao (Donkey-Hide Gelatin, 1.5 qian), Tiaoqin (Scutellaria Baicalensis Root), Sugeng (Perilla Stem), Fangfeng (Saposhnikovia Root, each 1 qian), Danggui (Angelica Sinensis Root), Mahren (Hemp Seed), Zhima (Sesame Seed, each 2 qian).
Boil in water and take orally.
The Classics state: "When a woman is heavily pregnant, she may become aphonic at nine months due to the obstruction of the collateral vessels of the Bao (Uterus)." No treatment is needed; speech will return naturally after ten months. This is because by the ninth month, the fetus has grown large, and the Bao is connected to the Kidney. The Shaoyin (Lesser Yin) Kidney meridian ascends to connect with the root of the tongue. If the meridian pathway is obstructed and blocked, speech impairment may occasionally occur. After delivery in the tenth month, speech returns naturally, requiring no intervention. Forcing treatment to restore voice would be erroneous. "Aphonia" (Yin) refers to having sound but lacking volume/clarity. The Classic's mention of "unable to speak" does not mean total muteness. Sound originates from the throat and emanates from the tongue root. Due to the bulkiness of fetal Qi, the ascent of Kidney Qi is blocked. The Kidney meridian enters the Lung, follows the throat, and connects to the tongue root; the throat is the domain of the Lung, which governs sound. The patient can only whisper; although the mind intends to speak, others cannot hear, hence it is called "Yin." The Lung and Kidney are mother-son organs; therefore, it is stated that no treatment is necessary. Interpreting this as Heart disease (as suggested in *Daquan* because the Heart governs vocalization) is unrelated to fetal aphonia. Suggestions by Zhang Zihe to descend Heart Fire or Ma Yuantai to tonify Heart and Kidney are flawed; if the Kidney collaterals were truly severed, the condition would be incurable, contradicting the natural recovery postpartum. Thus, the term "severed" (Jue) in "obstruction of Bao collateral vessels" should be interpreted as "blocked" (Zu). For wealthy families or those unfamiliar with medical principles who insist on forced treatment, one may decoct Shenmai Wuwei Yin (Ginseng, Ophiopogon, and Schisandra Decoction) thickly and administer it on an empty stomach, along with Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill), to assist Lung and Kidney Qi to nourish the fetus. One must absolutely avoid diagnosing it as Phlegm blocking the Heart Orifices and using phlegm-resolving, orifice-opening, or voice-restoring herbs, which could endanger both mother and child.
