一两。此本金匮肾气方中诸药,各减过半,惟桂、苓二味仍照原方,为宣布五阳、开发阴邪之专药;

Headache during pregnancy is mostly caused by blood deficiency with internal heat; treat with Siwu Tang (Four Substances Decoction) with modifications. If there is a history of recurrent migraine or frontal headache, Chuanxiong Chatiao San (Ligusticum Chuanxiong Tea-Pinginating Powder) is suitable.
For headache due to external contraction, Jiawei Xiongui Tang (Modified Ligusticum and Angelica Decoction) should be used.
Heart and abdominal pain during pregnancy may arise from pre-existing cold stagnation or phlegm-fluid accumulation, or from recent exposure to wind-cold. Pathogenic factors clash with healthy qi, affecting the movement of qi, which moves up and down. When rushing upward to the heart, it causes heart pain; when striking downward into the abdomen, it causes abdominal pain. Unrelenting pain may disturb the fetus. If the true heart is damaged causing Zhenxintong (True Heart Pain), death occurs within the same day (morning onset leads to evening death, evening onset leads to morning death). If the collateral branches are damaged, pain comes and goes intermittently. *Yitong* recommends Zhengqi San (Regular Qi Powder) with modifications for this. *Zunsheng* uses Dingtong Yanhu San (Pain-Relieving Corydalis Powder) for heart pain. *Yitong* also states: Fullness and distension in the chest and abdomen with tenderness upon pressure indicate food stagnation; treat with Renshen Yangwei Tang (Ginseng Stomach-Nourishing Decoction). Non-tenderness upon pressure indicates spleen-stomach injury; tonify with Liujunzi Tang (Six Gentlemen Decoction). Abdominal fullness and pain radiating to the heart with inability to eat calls for Qianjin Qinshu Shaoyao Tang (Thousand Gold Scutellaria, Atractylodes, and Peony Decoction). Intermittent abdominal pain is called Taipain (Fetal Pain), caused by either blood deficiency or qi stagnation, though blood deficiency is more common. Use Shudi (Prepared Rehmannia Root) 3 qian, Danggui (Angelica Sinensis) 1 qian, decocted in water. If ineffective, add Renshen (Ginseng), Baizhu (Atractylodes Macrocephala), and Chenpi (Tangerine Peel). Danxi uses Siwu Tang removing Chuanxiong and doubling Shudi. *Yitong* uses Siwu Tang added with Xiangfu (Cyperus Rotundus) powdered, taken with Zisu Tang (Perilla Decoction). For qi stagnation, Zisu Yin (Perilla Infusion) treats it. If pain is accompanied by a heavy, bearing-down sensation due to sinking of middle qi, use Buzhong Yiqi Tang (Center-Building and Qi-Augmenting Decoction).
Pain at the stomach opening is often due to spleen deficiency with retained undigested food; nourish the stomach and digest food. *Gangmu* suggests Pingwei San (Stomach-Calming Powder) added with Zhike (Bitter Orange) and Shanzha (Hawthorn Fruit). I believe Pingwei San might damage the fetus and is not suitable; instead, use Yangwei Tang with modifications.
Hypochondriac pain during pregnancy can result from crying, internal injury, or anger. Do not use qi-breaking herbs. It is suitable to take Tongbian (Child's Urine) mixed with wine, or use Zisu Yin removing Renshen, adding Baishao (White Peony Root), Danggui, Shenren (Amomum Villosum), and Tongbian.
Cold pain below the umbilicus, abdominal distension with empty pain, frequent urination, and loose stools are all caused by consuming raw and cold foods. Treat with Xiaojianzhong Tang (Minor Construct the Middle Decoction) added with Paojiang (Roasted Ginger) and Muxiang (Aucklandia Root). If ineffective, add Huixiang (Fennel Seed) and Liangjiang (Galangal Rhizome).
Lower abdominal pain during pregnancy: for deficiency-heat, use Zisu Yin; for deficiency-cold, use Jiaoai Tang (Ejiao and Mugwort Decoction). If swelling appears near the lower abdomen with thin, shiny skin, this is a gestational carbuncle (Yunyong); use Qianjin Tuoli San (Thousand Gold Supporting Interior Powder) or drink decoction of Yiyiren (Coix Seed). If there is acute heart-abdominal pain, restlessness, cyanotic face, cold sweat, collapse, continuous bleeding, and fetal rising, it is incurable.
Lower back pain during pregnancy: if caused by kidney deficiency, the pulse is large and pain persists; urgently administer Bazhen Tang (Eight Treasures Decoction) added with Ejiao (Donkey-Hide Gelatin), Aiye (Mugwort Leaf), and Huangqi (Astragalus Root), or modify Zisu Yin. If caused by overexertion and heavy lifting damaging the uterine ligaments threatening prolapse, use Antai Yin (Fetus-Securing Infusion) added with Duzhong (Eucommia Bark) and Xuduan (Dipsacus Root). If caused by excessive sexual activity damaging the uterine ligaments, use Shudi, Danggui, Ejiao, Shenren, Zhuru (Bamboo Shavings), and Zhigancao (Honey-fried Licorice Root), mixed with 1 qian of ash from male scrotum skin; strict abstinence from sex for 100 days is crucial. For damp-heat with a slow pulse, pain worsening on cloudy days or after prolonged sitting, with discharge of white matter, use Ermiao San (Two Marvels Powder) added with Chaihu (Bupleurum Root), Fangfeng (Saposhnikovia Root), Fuling (Poria), and Banxia (Pinellia Tuber). For cold-damp causing heavy, cold sensation in the waist like wearing a weight, modify Zisu Yin with spleen-fortifying and warming-drying herbs. For qi-blood stasis with a choppy pulse, pain worse at night preventing sleep, general stiffness and discomfort, and visual floaters, use Zisu Yin added with Zhike and Jiegeng (Platycodon Root). For wind-pathogen in the bladder causing neck stiffness, use Duohuo Jisheng Tang (Angelica Pubescens and Taxillus Decoction). For sprain/trauma with a solid pulse, use Jiaoai Xiongui Tang or Foshou San (Buddha's Hand Powder), or take charred brown sugar dissolved in child's urine and wine. For toxicity-induced issues, select Ejiao San, Kuaiejifang (Quick Method Formula), or Sanwu Jiedu Tang (Three-Item Detoxifying Decoction). For wandering pain indicating stagnant blood entering meridians, use Siwu Tang added with Bohe (Mint), Rougui (Cinnamon Bark), Duzhong, and Xuduan. For severe lower back pain resembling kidney dislocation at term, this indicates impending labor; do not treat as ordinary back pain, it will resolve after delivery without medication.
Back pain during pregnancy is due to qi stagnation; treat with Zisu Yin.
Pain involving the lower back, abdomen, and back during pregnancy: The kidney governs the lower back; exhaustion and channel deficiency allow wind-cold to invade, causing back pain. Entering the abdomen via deficiency causes abdominal pain, leading to mutual牵引 (traction) pain between waist and abdomen. Unrelenting pain risks miscarriage. For trauma-induced back/abdominal pain, use Buzhong Yiqi Tang added with Duzhong and Xuduan. However, many cases involve pain due to strangury (Lin syndrome); examine carefully. Treat strangury with Jiawei Daochi Tang (Modified Guiding Out Red Decoction); once strangury resolves, pain ceases. For yuanqi injury, use Bazhen Tang added with Duzhong, Shenren, Ejiao, and Aiye. For spleen-kidney insufficiency, add Baizhu and Buguzhi (Psoralea Fruit). For external pathogen invasion, use Duohuo Jisheng Tang. For qi-blood depression/stagnation, use Zisu Yin added with Zhike and Jiegeng. For liver deficiency with fire, use Xiao Chaihu Tang (Minor Bupleurum Decoction) added with Baizhu, Zhike, and Shanzhi (Gardenia Fruit). For liver-spleen depression/stagnation, use Guipi Tang (Restore the Spleen Decoction) added with Chaihu and Zhike.
Pain at the Huantiao point (GB30) during pregnancy belongs to kidney deficiency; use Liuwei Dihuang Tang (Six-Ingredient Rehmannia Pill Decoction) added with Duzhong and Xuduan, or Qianjin Baoyun Wan (Thousand Gold Pregnancy-Preserving Pill).
Red, painful eyes, sore throat, and sores/pain in mouth, nose, lips, and tongue during pregnancy are all treated with Liangge San (Cooling the Diaphragm Powder) with modifications.
Trauma from carrying heavy loads causing blood stasis pain: Using blood-invigorating herbs fears fetal damage; not using them leaves injured blood unresolved. Treatment must first distinguish fetal life or death. Without other symptoms, use charred brown sugar mixed with aged wine and child's urine, chewed with whole walnuts. If the fetus is dead, blood passes; if alive, pain stops immediately. For trauma induced by anger or convulsions, use Zisu Yin added with Gouteng (Uncaria Rhynchophylla). For excessive blood loss, use Bazhen Tang removing Fuling, adding Ejiao, Aiye, and Huangqi. For residual trauma pain, use Siwu Tang added with Yanhusuo (Corydalis Rhizome) and Muxiang. If external injury is healed but fetal qi is unsettled, use Zisu Yin added with Tongbian and Shenren. For severe injuries, use Xiangke San (Costus and Bitter Orange Powder) added with Shudi and Danggui to protect the fetus. For critical conditions, use Xiayuxue Tang (Eliminate Stasis Blood Decoction) with Chuanxiong and Danggui decocted in wine to remove blood, thereby securing the fetus. This formula is harsh; use with extreme caution. For lethargy and delirious speech indicating blood forcing into the pericardium, use Danggui Huoxue Tang (Angelica Invigorating Blood Decoction). For heavy bearing-down sensation in abdomen with coldness on pressure, fetal qi is damaged; urgently use Xiangui San (Costus and Cinnamon Powder) added with Jiuhuanglian (Wine-processed Rhubarb) and Shengfuzi (Raw Aconite Root) to purge. If foul breath is detected, urgently use Pingwei San added with Mangxiao (Mirabilite) to purge. I believe Foshou San is best for expelling the fetus; even Xiangui San can expel it, so why use Rhubarb and Aconite? However, formulas like Siwu Tang, Xiangke San, Xiayuxue Tang, Danggui Huoxue Tang, Xiangui San, and Pingwei San should not be used lightly. It is better to abstain and simply use charred brown sugar with wine and urine, or chew walnuts. These formulas are recorded below for reference. Do not slight Foshou San or Xiangui San, nor recklessly use harsh life-damaging drugs.
Generalized body pain during pregnancy, potentially reaching the heart causing death-like distress, and inability to eat, is caused by fetal fluid retention. Use Baizhu (Atractylodes Macrocephala) 5 qian, Huangqin (Scutellaria Root) 2 qian, Baishao (White Peony Root) 4 qian, boiled with one live carp (approx. half jin) replacing water for decoction. This is the Fetal Fluid Syndrome, usually categorized separately, but included here due to generalized pain.

Decoct in water. For pills: Shudi 8 liang, Mudanpi/Fuling 3 liang each, Shanyuzhi/Shanyao 4 liang each, Zexie 1 liang (ancient formula used 3 liang Zexie). Honey-pill size of wutong seeds. Add Rougui (Cinnamon Bark) 1 liang, Zhi Fuzi (Processed Aconite Root) 5 qian, named Guifu Bawei Dihuang Wan or Jin Gui Shen Qi Wan. One version uses 1 liang each of Gui and Fu. Adding Niuxi (Achyranthes Root) and Cheqiancao (Plantago Leaf) 1 liang each to the eight ingredients is named Jiawei Shen Qi Wan. One version substitutes Rougui with Guizhi (Cinnamon Twig). This addresses yin consolidation internally requiring dispersion externally to circulate kidney qi throughout the body. Only the formula remains, rarely modified by others. I note Jisheng Shen Qi Wan in medical texts treats kidney qi failure, difficult/frequent urination, postpartum leg/abdominal swelling, or drum-like bloating. It is based on the eight-ingredient formula: Fuling 3 liang, Shudi 4 liang, Shanyao, Shanyuzhi, Mudanpi, Zexie, Rougui 1 liang each, Fuzi 5 qian, plus Niuxi and Cheqian 1 liang each. Most herbs in Jin Gui Shen Qi formula are halved, except Gui and Ling remain original, acting specifically to disseminate five yangs and dispel yin evil. Niuxi and Cheqian act as guides for Taiyang and Jueyin. Liver is wind-wood organ; herbs traversing its channels rise, but Niuxi directs fluids downwards to the lowest yin, while Cheqian drains fluids without harming righteous qi. With Niuxi guiding to lowest yin, Gui and Fu vaporize/sanitize the Triple Burner, empowering water passages and bladder transformation. Thus, double Rougui hides the subtle utility of Guiling Wan, ten times superior to Wuling San. However, Niuxi causes seminal emission; avoid in those with unstable essence/qi. Postpartum essence is naturally unstable; users must be cautious. Therefore, for miscellaneous disorders with urinary obstruction, Jisheng Shen Qi Wan is best. Modern dosages of Shen Qi Wan largely follow this. Later generations confused the distinct weights of Bawei, Jiawei, and Jisheng,统称 (collectively naming) them Jin Gui Shen Qi, which is confusing. I highlight this distinction now.
