Yang poison disease presents with heavy body, pain in the waist and back, mental restlessness, delirious speech, or running about, or seeing ghosts, or hematemesis and dysentery. The pulse is floating, large, and rapid. The face is red with mottled spots like brocade patterns, accompanied by sore throat and expectoration of pus and blood. It is treatable within five days; after seven days, it becomes untreatable. Some cases of cold damage transform into yang poison within one or two days. Alternatively, after taking medicine that induces vomiting or purgation, the condition may transform into yang poison, which is primarily treated with Shengma Tang (Cimicifugae Decoction).
Yin poison disease presents with heavy body, stiff back, colicky abdominal pain, difficulty in the throat, toxic qi attacking the heart, hardness below the heart, shortness of breath with inability to breathe deeply, vomiting and regurgitation, cyanotic lips, blackish complexion, and cold extremities. The pulse is deep, fine, tight, and rapid. The body feels as if beaten. It is treatable within five to six days; after seven days, it becomes untreatable. Some cases of initial cold damage form yin poison within one or two days. Alternatively, after taking medicine for six to seven days up to ten days, the condition may transform into yin poison, which is primarily treated with Gancao Tang (Licorice Decoction).
The manifestation of Baihe Bing (Lily Disease) is characterized by a desire to remain silent and lie down, yet an inability to do so; sometimes resembling a healthy person who wishes to walk but cannot; desiring food but unable to eat; at times finding food palatable, at other times repulsed by the smell of food; appearing neither cold nor hot; bitter taste in the mouth upon waking, dark yellow urine, while the physical appearance seems normal. The pulse is slightly rapid. Since all vessels originate from one source, they are all diseased; treatment should follow the specific syndrome. When Baihe Bing manifests with yin characteristics, it should be rescued by yang methods; when manifesting with yang characteristics, it should be rescued by yin methods. If yang manifestations are present but yin is attacked, followed by sweating, this is contrary to principle, and the disease is difficult to treat. If yin manifestations are present but yang is attacked, followed by purgation, this is also contrary to principle, and the disease is difficult to treat. (*Qianjin Fang* states: If the disease is in yin but yang is attacked, yin cannot be resolved, and inducing sweat is contrary to principle. If the disease is in yang but yin is attacked, yang cannot be resolved, and purgation will not cure the disease.)
Huhuo Bing (Fox-Spook Disease) presents with symptoms similar to cold damage: silence, desire to sleep, inability to close the eyes, and restlessness in lying down and rising. Erosion in the throat is called "huo" (spook); erosion in the genitalia is called "hu" (fox). Patients with huhuo Bing have no appetite and are repulsed by the smell of food. The facial color changes alternately between red, white, and black. Toxic erosion in the upper part causes hoarseness; toxic erosion in the lower part causes dry throat. Erosion in the upper part is primarily treated with Xixin Tang (Pinellia and Gentian Decoction). Erosion in the lower part is washed with Kushen Tang (Sophora Root Decoction); erosion in the anus is fumigated with Xionghuang (Realgar). The patient has a rapid pulse, no fever, slight irritability, silence, desire to sleep, and sweating. Within three to four days of onset, the eyes become red like those of a turtledove; after seven to eight days, the corners of the eyes turn yellow-black. If the patient can eat, pus has formed, and Chixiaodou Danggui San (Adzuki Bean and Angelica Sinensis Powder) is primarily used. The disease may start from respiration, eroding upward into the pharynx, or from the lower jiao, eroding downward into the anus and genitals. Erosion in the upper part is huo; erosion in the lower part is hu. Huhuo Bing is primarily treated with Zhuling San (Polyporus Powder).
