Two-Yang concurrent disease refers to the simultaneous existence of Taiyang (Greater Yang) and Yangming (Yang Brightness) diseases. When Taiyang disease is in its initial stage, if diaphoresis (sweating therapy) is applied, and sweat is released initially but not thoroughly, the pathogen may transfer to Yangming. The patient will present with continuous slight sweating and aversion to cold ceases. If the Taiyang syndrome has not subsided, purgation (downward-draining method) is contraindicated; using it would be an incorrect treatment. In such cases, a mild diaphoretic method should be employed.
If the patient's face appears persistently red, this indicates Yang Qi stagnation at the exterior. It should be treated by resolving the exterior or using fumigation therapy. If diaphoresis is insufficient, Yang Qi stagnates and cannot vent outward. The patient becomes restless and irritable, unable to locate specific pain points, which may shift between the abdomen and limbs, and cannot be found upon palpation. The patient experiences shortness of breath and can only sit, due to incomplete sweating. Re-administering diaphoresis will lead to recovery. How do we know the sweat was incomplete? This is known through a choppy pulse (Se Mai).
Before taking the pulse, observe the patient crossing their hands over the heart region. If the physician instructs the patient to cough and they do not respond immediately, it indicates bilateral hearing loss. This occurs because excessive diaphoresis has caused deficiency of healthy Qi.
After diaphoresis, drinking excessive amounts of water will inevitably cause wheezing. Washing the body with water will also cause wheezing. After diaphoresis, if fluids and medicine cannot be retained in the mouth, it is considered a counter-productive state. If diaphoresis is repeated, it will inevitably lead to incessant vomiting and diarrhea.
In Yangming disease, spontaneous sweating is common. If the physician repeats diaphoresis, although the illness may seem alleviated, the patient remains slightly irritable and mentally unclear due to hard stool. This results from fluid exhaustion and dryness in the Stomach. One should inquire about the frequency of urination. If the patient usually urinates three to four times a day but only twice today, it indicates that stool will soon pass. Since urinary frequency has decreased, fluids are returning to the Stomach, indicating bowel movement is imminent.
Excessive diaphoresis followed by repeated diaphoresis leads to collapse of Yang (Wang Yang). If delirium (Chan Yu) and a short pulse occur, the prognosis is fatal; if the pulse is harmonious, the patient will survive.
In Shanghan (Cold Damage), after diaphoresis, jaundice of the body and eyes may appear because cold-dampness combines internally and fails to resolve. If a patient with cold symptoms undergoes repeated diaphoresis, causing coldness in the Stomach, vomiting of roundworms will occur.
In Taiyang disease, if diaphoresis causes leakage of sweat that does not stop, accompanied by aversion to wind, difficulty in urination, slight stiffness in the four limbs making flexion and extension difficult, this belongs to the Guizhi Jia Fuzi Tang (Cinnamon Twig Decoction plus Aconite Root) pattern.
After taking Guizhi Tang, if heavy sweating occurs and the pulse is merely surging and large (Hong Da), continue administering Guizhi Tang. If the condition resembles malaria, occurring two or three times a day, resolving with sweating, this belongs to the Guizhi Er Mahuang Yi Tang (Cinnamon Twig Decoction twice, Ephedra Decoction once) pattern.
After taking Guizhi Tang, if heavy sweating leads to severe irritability and unquenchable thirst, and the pulse is surging and large, this belongs to the Baihu Tang (White Tiger Decoction) pattern.
In Shanghan, if the pulse is floating, there is spontaneous sweating, frequent urination, mental restlessness (Fan Xin), slight aversion to cold, and cramping in the feet, administering Guizhi Tang to attack the exterior is erroneous. This will result in cold extremities, dry throat, irritability, and vomiting. One should administer Gancao Ganjiang Tang (Licorice and Dried Ginger Decoction) to restore Yang. Once extremities warm up, administer Shaoyao Gancao Tang (Peony and Licorice Decoction) to extend the legs. If Stomach Qi disharmony causes delirium, Chengqi Tang (Purgative Decoction) may be given. If heavy diaphoresis is combined with moxibustion on needles, this belongs to the Sini Tang (Frigid Extremities Decoction) pattern.
In Shanghan, if the condition resolves after diaphoresis but returns with irritability after half a day, and the pulse is floating and rapid, diaphoresis can be repeated using Guizhi Tang.
After diaphoresis, if body pain persists with a deep and slow pulse, this belongs to the Guizhi Jia Shaoyao Shengjiang Renshen Tang (Cinnamon Twig Decoction plus Peony, Fresh Ginger, and Ginseng) pattern.
After diaphoresis, Guizhi Tang is contraindicated. If sweating and wheezing occur without significant heat, Mahuang Xingren Gancao Shigao Tang (Ephedra, Apricot Kernel, Licorice, and Gypsum Decoction) should be used.
After excessive diaphoresis, if the patient crosses hands over the heart, experiences palpitations in the epigastrium, and desires pressure, this belongs to the Guizhi Gancao Tang (Cinnamon Twig and Licorice Decoction) pattern.
After diaphoresis, if the patient feels palpitations below the navel and sensation of Ben Tun (Qi rushing upward like a piglet), this belongs to the Fuling Guizhi Gancao Dazao Tang (Poria, Cinnamon Twig, Licorice, and Jujube Decoction) pattern.
After diaphoresis, if abdominal fullness occurs, this belongs to the Nongpo Shengjiang Banxia Gancao Renshen Tang (Magnolia Bark, Fresh Ginger, Pinellia, Licorice, and Ginseng Decoction) pattern. If the disease does not resolve after diaphoresis and aversion to cold increases, it is a deficiency pattern belonging to Shaoyao Gancao Fuzi Tang (Peony, Licorice, and Aconite Root Decoction). If there is no aversion to cold but only heat, it is an excess pattern; one should harmonize Stomach Qi using Xiao Chengqi Tang (Minor Purgative Decoction).
In Taiyang disease, if heavy sweating causes dryness in the Stomach, irritability, insomnia, and desire for water, small sips of water should be given until Stomach Qi harmonizes and recovery occurs.
After diaphoresis, if the pulse is floating and rapid, accompanied by irritability and thirst, this belongs to the Wuling San (Five-Ingredient Powder with Poria) pattern.
In Shanghan, if sweating is accompanied by thirst, it belongs to the Wuling San pattern; if there is no thirst, it belongs to the Fuling Gancao Tang (Poria and Licorice Decoction) pattern.
In Taiyang disease, if sweating does not resolve the disease, and the patient presents with fever, palpitations, dizziness, muscle twitching, and trembling as if falling to the ground, this belongs to the Zhenwu Tang (True Warrior Decoction) pattern.
After Shanghan resolves following sweating, if Stomach Qi is disharmonious, presenting with epigastric fullness and hardness, dry belching with food odor, water accumulation under the ribs, borborygmus, and diarrhea, this belongs to the Shengjiang Xiexin Tang (Fresh Ginger Drain the Epigastrium Decoction) pattern.
In Shanghan, if fever persists after sweating, with epigastric fullness, vomiting, and diarrhea, this belongs to the Da Chaihu Tang (Major Bupleurum Decoction) pattern.
In Taiyang disease, if after three days of treatment, sweating fails to resolve the condition, and steaming heat occurs, the pathology is in the Stomach, belonging to Chengqi Tang patterns.
If heavy sweating leaves heat unresolved, accompanied by internal cramping, limb pain, diarrhea, cold extremities, and aversion to cold, this belongs to Sini Tang.
If excessive diaphoresis leads to Yang collapse and delirium, purgation is contraindicated. Instead, Chaihu Guizhi Tang (Bupleurum and Cinnamon Twig Decoction) should be administered to harmonize Ying and Wei (Nutritive and Defensive levels) and promote fluid circulation, leading to natural recovery.
