A stream of blood flows from the hair follicles, shooting out like an arrow; hence it is named "Blood Arrow" (Xue Jian). This condition arises from excessive fire in the Heart and Lung organs, forcing blood to exit through the pores. Treatment should focus on clearing Heart Fire to eliminate the source of bleeding, and purging Lung Fire to consolidate the Qi of the skin and hair follicles, preventing leakage so that bleeding stops naturally. The recommended formula is Liang Xue Di Huang Tang (Cooling Blood Rehmannia Decoction) with added Puhuang (Corydalis Rhizome Powder). Alternatively, one may purge Lung Fire using Xie Bai San (Draining White Powder) supplemented with Shengdi Huang (Rehmanniae Radix), Chan Tui (Cicadae Periostracum), Bai He (Lilii Bulbus), Wu Bei Zi (Galla Chinensis), Huang Qin (Scutellariae Radix), Puhuang (Corydalis Rhizome Powder), Xing Ren (Armeniacae Semen Amarum), and Bai Ji (Bletillae Rhizoma). To treat both Heart and Lung simultaneously, Sheng Di Huang San (Raw Rehmannia Powder) is suitable.
If excessive bleeding leads to stupor and loss of consciousness, it is identical to the syndrome of Qi and Blood collapse seen in hematemesis or epistaxis. Dandu Tang (Single Ginseng Decoction) with Fupian (Prepared Aconiti Lateralis Radix) and Puhuang (Corydalis Rhizome Powder) is indicated. Dang Gui Bu Xue Tang (Angelica Sinensis Tonifying Blood Decoction) and Shiquan Dabu Tang (Complete Ten Great Tonification Decoction) are also appropriate options.
For external treatment, apply Taohua San (Peach Blossom Powder) mixed with water to the bleeding pores to stop hemorrhage. Alternatively, apply vinegar-ground Jingmo (Beijing Ink) or sprinkle dry Duanshi San (Calcined Stone Powder) or Huaruishi San (Flower Stalactite Powder); all are effective.
Blood Nevus (Xue Zhi) initially appears as a mole-like spot, gradually enlarging to the size of a bean. If ruptured, it bleeds and oozes fluid for a long time. This is caused by Liver Fire stagnation leading to blood coagulation. Internal treatment involves Danzhi Xiaoyao San (Moutan and Gardenia Free and Easy Wanderer Powder) and Liang Xue Di Huang Tang (Cooling Blood Rehmannia Decoction).
If ruptured and bleeding, apply Huaruishi San (Flower Stalactite Powder). After bleeding stops, use Tianluo San (Snail Powder) to cauterize the root of the nevus, and apply other flesh-regenerating medicines to close the wound. For unruptured nevi without bleeding, there were no ancient treatments. I propose using Mengchong (Tachina Larva) powder mixed with ginger juice and vinegar for topical application; or applying vinegar-grind mixtures of Yujin (Curcumae Radix) and Sanleng (Sparganii Rhizoma); or rubbing heated real Hupo (Amber) several times daily. Internal medications remain as above.
Blood spots and nodules within tinea and scabies, along with all forms of red and itchy skin lesions, vary in name and color but are collectively termed "Blood Itch" (Xue Sao). These result from blood being disturbed by Wind and Fire. Excessive Fire causes red spots and nodules; excessive Wind causes parasitic sensations and itching. For severe Fire presenting with redness and pain, treat with Liang Xue Di Huang Tang (Cooling Blood Rehmannia Decoction) plus Jingjie (Schizonepetae Herba), Chan Tui (Cicadae Periostracum), Honghua (Carthami Flos), and Xing Ren (Armeniacae Semen Amarum). For severe Wind causing itching, treat with He Xiao Feng San (Harmonizing Blood Eliminating Wind Powder).
Understanding the pathology of Blood Itch reveals that treatments for conditions such as Fire Roaming Redness (Huo You Dan), Lacquer Dermatitis (Qi Chuang), and Wind Urticaria (Feng Dan) generally follow these principles. Cases with concurrent Heat present with white discoloration or yellow exudate; refer to the aforementioned formulas and add Cang Zhu (Atractylodis Rhizoma) and Chi Ling Shen (Polyporus with Red Host). Cases with concurrent Cold present with cyanotic, dark, hard, or swollen appearances; add Guijian (Cinnamomi Ramulus Tip).
Externally, wash with a decoction of Yinhua (Lonicerae Japonicae Flos), Chen Ai (Aged Mugwort), Chuanjiao (Zanthoxyli Pericarpium), and Shiyan (Sodium Chloride). Additionally, apply Dafeng Dan (Large Maple Elixir), ideally prepared with oil.
