Qian Yi stated: Patients presenting with wheezing and expectoration of pus and blood are suffering from Lung heat; Gan Jie Tang (Licorice and Platycodon Decoction) should be administered after meals. For chronic cough, which depletes the fluids of the Lung, E Jiao San (Donkey-Hide Gelatin Powder) is indicated. (This is the same as Bu Fei San [Lung-Supplementing Powder] mentioned previously.) If the patient coughs with phlegm accumulation and excess syndrome, but without severe wheezing, accompanied by a red face and thirst, Pian Yin Wan (Narrow Silver Pill) should be used to purge the pathogen. (Formula see previous section.)
The son of Duan Zhaolang, aged four, suffered from cough, fever, and expectoration of phlegm, which progressed to hemoptysis after several days. Previous physicians treated him with Jie Geng Tang (Platycodon Decoction) and Fang Ji Wan (Stephania Pill), but the condition did not improve. Instead, saliva surged upward, causing incessant vomiting and wheezing. Qian Yi was consulted and first administered a large dose of Pian Yin Wan for purgation, followed by treatment with Bu Fei Tang (Lung-Supplementing Decoction) and Bu Fei San. Someone asked, "Since the boy's hemoptysis indicates Lung deficiency, why use a purgative method?" Qian Yi replied, "Although there is hemoptysis due to the Lung, it is caused by heat. If left untreated for a long time, it will lead to deficiency and atrophy. Currently, the surging saliva causes vomiting; one must first purge the saliva. If this is not done, the vomiting of saliva will further weaken the body and may induce fright convulsions; excessive phlegm attacking upwards can also trigger spasms. Therefore, according to medical principles, one should first purge the phlegm and subsequently tonify the Spleen and Lung. Only when the saliva stops and vomiting ceases does this constitute a favorable treatment sequence. Tonifying the Lung first would be an adverse approach." This illustrates the principle of recognizing the severity and urgency of the disease to determine the order of treatment.
Gan Jie San (Licorice and Platycodon Powder)
Jie Geng (Platycodonis Radix, soaked in rice-washing water overnight, then dried by baking) and Gan Cao (Licorici Radix et Rhizoma, stir-fried). Each two liang.
Grind the above into a fine powder. Take one da qian (large coin measure) per dose, add one da zhan (large cup measure) of water and half a liang of processed E Jiao (Donkey-Hide Gelatin), decoct until five fen remain, and take warm after meals.
[Hai Zang] Gan Jie Tang is a formula from Zhang Zhongjing for treating Shaoyin throat pain. Sun Simiao used raw Gan Cao with modifications in over twenty prescriptions for treating lung abscess with expectoration of pus and blood.
Jie Ji Wan (Muscle-Relieving Pill) from *Wang Shi Shou Ji*: Treats external wind invasion combined with internal phlegm-fluid retention, alternating chills and fever, irritability and thirst, flushed cheeks, palpitations, reduced appetite, heat in the Upper Jiao, and cough with blood.
Fang Feng (Saposhnikoviae Radix) and Di Gu Pi (Lycii Cortex), each one fen.
Make pills using burnt rock sugar. Take one pill per dose after meals, washed down with Zi Su Tang (Perilla Leaf Decoction).
Tuan Shen Wan (Ginseng Round Pill): Treats cough with hemoptysis.
E Jiao (Donkey-Hide Gelatin), Zao Zi Huang (Yellow of Soapberry Seed), and Ren Shen (Ginseng Radix et Rhizoma), each half a liang.
Except for the gelatin, grind the others into a fine powder. Use a small amount of soup to dissolve the gelatin and mix with the powder to form pills the size of a chicken head. Dissolve and take with plain boiled water.
Ji Qing San (Chicken White Powder) by Zhu Shi: Treats cough with bleeding and reduces salivation.
Yu Jin (Curcumae Radix, half a liang, boiled in one zhan of soapberry juice or sour vegetable juice until dry), Hua Shi (Talcum, half a liang, raw), and Xiong Huang (Realgar, half a liang, boiled in vinegar until semi-dry).
Grind the above into a fine powder. Take one zi (a very small measure) per dose. For regular use, mix with Bo He Tang (Mint Decoction). To stop cough, take with Luo Fen Shui (Conch Shell Powder Water). To treat cough with blood, mix with Ji Zi Qing (Egg White).
