According to the *Taiping Shenghui Fang*, Lung Qi connects with the nose, and the nose is the external manifestation of the Lung. If an infant's diet of milk and food is irregular, causing stagnation in the Upper Jiao (Upper Burner), it leads to "Gan" insects (parasites associated with malnutrition) ascending to erode the nasal cavity. The symptoms include redness and itching in the nose, high fever with excessive crying, dry and scorched hair and skin, emaciation, coughing with rebellious Qi, irregular diarrhea, and sores with redness and ulceration extending from under the nose to the lips; hence it is called "Bi Gan" (Nasal Gan).
Formula for urgently treating infants with redness and ulceration under the nose, irritability, sores in the nose that gradually spread over the body, restlessness, and day-and-night pain:
*Hezi* (Two fruits) *Doudou* (Three seeds) *Huanglian* (Six fen) *Fangfeng* (Half liang) *Zhusha* (One fen)
Grind the above ingredients into powder, form pills the size of hemp seeds using rice paste. Administer three pills per dose, washed down with *Jingjie* Decoction.
*[Tang Shi]* Wufu Huadu Dan: Treats nasal Gan sores; clears the diaphragm and cools the blood.
*Xuanshen* *Jiegeng* (remove rhizome, each one liang) *Chifuling* *Renshen* *Mayaxiao* *Qingdai* *Gancao* (each one fen) *Shexiang* (Five fen); another version adds *Longnao* (Five fen).
Except for *Shexiang* and *Mayaxiao* which are ground separately, grind the remaining ingredients together into a fine powder, then mix with *Qingdai* etc., and bind with refined honey to form pills the size of Chinese chestnut (*Qianshi*), coated with gold and silver foil. Dissolve and administer with *Bohe* Decoction. For residual toxins from exanthematous diseases, grind fresh *Xijiaojiao* (Rhino Horn) and take with water. For heat stagnation in the Upper Jiao causing bleeding gums, exposed teeth, and foul odor, dissolve and administer with juice of *Shengdihuang*. Take after meals.
*Zexie San*: Treats infant lung accumulation, sores inside the nose, and redness/ulceration under the nose.
*Chuan Zexie* *Chuan Yujin* (raw) *Gancao* (honey-fried) *Shanzhizi Ren* (stir-fried, each one fen).
Grind the above into powder. Dosage: one "yi" character measure for infants, half "qian" for ages 2-3, one "qian" for ages 5-7; mix with *Gancao* Decoction and administer. Twice daily. It is advisable to additionally apply *Qingjin San* externally.
*Pipaye San* (Self-made): Treats Bi Gan with redness and ulceration.
*Pipaye* (remove hairs, shade-dry, one liang) *Shanzhizi* (Half liang) *Bai* *Binglang* (each two qian and half).
Grind into fine powder. Dosage: three "qian" per dose, two "qian" for small children, one "qian" for very small children; mix with warm water and administer.
*Tian Shi*'s treatment method: First wash the area with a decoction of *Gancao* and *Baifan*, then dust with powders of *Luhui*, *Huanglian*, and *Huangbai*, applying three times daily. Another method: wash with rice washing water, apply *Huanglian* powder, three to four times daily, which is also effective.
*Qingjin San*
*Tongqing* *Baifan* (raw, each one qian).
Grind into powder. Apply a small amount to under the nose each time.
*[Qian Yi]* Lanxiang San
*Lanxiang Ye* (vegetable name, burnt to ash, two qian) *Tongqing* (Half qian) *Qingfen* (Two zi).
Grind into a uniform fine powder. Apply directly as a dry powder according to the size of the sore.
*[Zhang Huan]* Shidan San: Treats Bi Gan disease, where Gan insects ascend to erode the nose causing redness and itching, and extend to the lips causing redness and ulceration.
*Shidan* (One liang) *Dilong* (One fen, washed clean) *Toufa* (burnt to ash) *Langdang Zi* (raw use, each half liang).
Pulverize the above ingredients into a fine powder, add one "qian" of *Shexiang* and grind uniformly. Apply one "yi" character measure to the sore each time.
*Ji Shi Jiachuan* Formula for treating redness and ulceration of Bi Gan under the nose:
*Qingdai* (One qian) *Shexiang* (A little) *Xiongdan* (powder, half qian).
Grind into powder. Apply a small amount to under the nose before sleep.
*Zhu Shi Jiachuan* Formula for treating damp, itchy Gan sores under the nose in infants:
Take one date, remove the pit, insert a piece of *Baifan* into the date, calcine over mild-to-strong fire until the substance remains (preserve nature), then grind finely. Apply to the sore; if the sore is dry, mix with sesame oil for application.
*Taiping Shenghui Fang* Chuibi Chanke San: Treats infant nasal Gan itching.
*Chanke* (lightly stir-fried) *Qingdai* (finely ground) *Shetuipi* (ash) *Huashi* *Shexiang* (finely ground, equal parts).
Pound the above herbs, sieve into a fine powder, and grind thoroughly to mix evenly. Blow in an amount the size of a mung bean into the nose each time. Use three times daily; this expels all Gan insects.
Qian Shi Baifen San: Treats various sores.
*Wzeiyu Gu Mo* (Three bi) *Baiji Mo* (Two bi) *Qingfen* (One bi).
Grind into powder. First wash and wipe with clear fermented grain liquid, then apply.
