The *Suwen* (Basic Questions) states: The East corresponds to the color green, internally connecting with the Liver. Its disease manifests as fright and alarm. If heat from the Spleen transfers to the Liver, it results in fright and epistaxis. When the Yangming (Stomach/Large Intestine) and Jueyin (Liver/Pericardium) channels are affected simultaneously, the primary symptoms are fright, back pain, frequent belching, and frequent yawning; this condition is named Fengjue (Wind Jue). When Taiyang (Bladder) and Yinyin (Liver) are involved, if the Taiyang pulse prevails and the Yinyin cannot restrain it, internal disorder of the five Zang organs occurs, manifesting externally as fright. The Foot-Yangming Stomach meridian, when disturbed, causes illness where hearing the sound of wood induces sudden fright and palpitations. In the *Lingshu*, it is stated that "when severe, one dislikes people and fire, and hears the sound of wood with sudden fright." This is because Yang Qi and Yin Qi contend with each other, and Water and Fire are antagonistic, thus causing sudden fright. The Yellow Emperor asked: When the Foot-Yangming meridian is diseased, why does one dislike people and fire, and feel sudden fright at the sound of wood, yet remain unmoved by bells and drums? Qibo replied: The Yangming is the meridian of the Stomach, and the Stomach belongs to Earth in the Five Elements. Therefore, one feels fright at the sound of wood because Earth fears Wood (Wood overcomes Earth). From this, we can see that the Liver, Gallbladder, Heart, Spleen, and Stomach all have manifestations of fright.
There are three types of fright palpitations caused by climatic factors (Yunqi): First, deficiency of Liver Wood allows Metal to invade. The classics state: "Deficiency of Wood is called Weihe. In the era of Weihe, its manifestation is fright." It also states: "When Yangming reverts, it severely affects the Liver, causing fright and tendon spasms." Second, Fire pathogen assists the Heart. The classics say: "Arrival of Shaoyang brings fear and fright." Also: "Shaoyang arrival brings fright and restlessness." And: "Victory of Shaoyang leads to easy fright." Third, Cold pathogen damages the Heart. The classics state: "Excess of Yearly Water leads to the prevalence of Cold Qi, causing heart vexation, irritability, and palpitations." Li Dongyuan said: If the six pulses are large but empty upon deep pressure, accompanied by a red face, easy fright, and upper body heat, this is the pulse of the Hand-Shaoyin Heart meridian. This indicates excess Qi and deficient Blood. One should use sweet-cold descending agents to clear Heat and descend floating Qi. Then, use sweet-pungent-warm slightly bitter strong tonics to replenish Blood, using Shu Dihuang (Prepared Rehmannia Root), Sheng Dihuang (Raw Rehmannia Root), Chaihu (Bupleurum Root), Shengma (Cimicifuga Rhizome), Baishaoyao (White Peony Root), Mudanpi (Moutan Cortex), Chuanxiong (Szechwan Lovage Rhizome), Huangqin (Scutellaria Root), etc., to prevent blood from overflowing and exhausting upwards. (Sweet-cold descending agents refer to substances like Zhusha/Cinnabar.) The *San Yin Fang Lun* states: Accumulation of the five types of fluid retention obstructing the middle Jiao most easily causes fright; this belongs to the category of Fluid Retention diseases (use Wuyin Tang or pills). Deficiency and timidity of the Heart and Gallbladder, leading to easy fright upon encountering events, or dreaming of ominous signs, resulting in a frightened heart and gallbladder, Qi stagnation generating phlegm-fluid, which contends with Qi, transforming into various syndromes such as shortness of breath, fatigue, or spontaneous sweating, are all treated primarily with Wendan Tang (Warm the Gallbladder Decoction). If vomiting occurs, replace Zhuru (Bamboo Shavings) with Renshen (Ginseng). For palpitations, excessive dreaming, and immediate awakening from fright, use Wendan Tang plus Suanzaoren (Ziziphus Seed) and Lianrou (Lotus Seed Flesh), each 1 qian, taken with Yinjin (Silver foil) decoction, along with Shisiyou Wan (Fourteen Friends Pill), Zhenxin Dan (Heart-Calming Pill), Yuanzhi Wan (Polygala Pill), Miao Xiang San (Wonderful Fragrance Powder) mixed with wine, Hupo Yangxin Dan (Amber Nourishing Heart Pill), Dingzhi Wan (Will-Firming Pill), or Ningzhi Wan (Mind-Calm Pill). For sleep disturbances with many nightmares, use Zhenzhumu Wan (Mother-of-Pearl Pill) or Duhuo Tang (Angelica Pubescens Decoction). Qianghuo Shengshi Tang (Notopterygium Decoction for Overcoming Dampness) treats sleep disturbances with fright palpitations, nightmares, and urinary abnormalities, indicating pathogen in the Shaoyang and Jueyin meridians. Add Chaihu 5 fen. If dysuria is present, add Zexie (Alisma Rhizome) 5 fen. This is a combined disease of Wind and Cold in the Lower Jiao. The classics state: Diseases of the Kidney and Liver share the same treatment because they are both in the Lower Jiao; wind-dispelling herbs must be used to unblock the meridians. Zhu Danxi said: When illness arises from fright, the Shen (Spirit) leaves its residence. An empty residence allows fluids to condense into Phlegm. When Qi and Blood enter the residence, Phlegm repels the Shen, preventing its return. (Use Shouxing Wan or Kongxian Dan, added with Chen Sha/Cinnabar and Yuanzhi/Polygala.) For palpitations caused by sudden great fright, the pulse is greatly agitated. The characteristic of the Dong Pulse (Agitated Pulse) is like a bean, trembling violently, without head or tail. Li Dongyuan said: Fright from external objects should be calmed with sedatives, such as Huanglian Anshen Wan (Coptis Calming Spirit Pill). Miduo Seng (Lead Monoxide) ground into fine powder, mixed with tea water and taken as 1 qian, treats inability to speak due to fright Qi entering the Heart collaterals. Historically, individuals frightened by wild wolves or large snakes were calmed by this remedy. Since fright causes Qi to ascend, heavy sedative agents are used to descend it. For Heat stagnation with Phlegm, use Hanshuishi San (Calcitum and Arsenolite Powder). For Qi stagnation with Phlegm, use Jiawei Siqi Tang (Modified Four-Seven Decoction). For deficiency with Phlegm, use Shiwei Wendan Tang (Ten-Ingredient Warm the Gallbladder Decoction) or Yangxin Tang (Nourish Heart Decoction). The *Jinkui Yaolue* states: There are conditions named Ben Tun (Rushing Pig), pus expectoration, fright terror, and Fire Pathogen; these four diseases all arise from fright. The classics state: When the opening and closing of Yang Qi fail, Cold Qi enters, causing severe curvature. If it sinks into the vessels and lingers in the muscle layers, affecting the Shu points, it transmits as fearfulness and fright. This is fright caused by carbuncles. Because the Shu point is the aperture of the carbuncle, painful Qi lingers in the muscle layers, fearing touch and pain, thus generating a fearful mind, which internally presses and transmits into diseases of fearfulness and fright.
[Diagnosis] The Cun Kou (Radial) pulse is agitated and weak; agitation indicates Fright, weakness indicates Palpitations. The Fu Yang (Dorsalis Pedis) pulse is faint and floating; floating indicates Stomach Qi deficiency, faint indicates inability to eat. Such a fearful pulse is produced by worry and distress. If the Cun Kou is tight and the Fu Yang is floating and slippery, Qi deficiency leads to palpitations. The main pulse for Fright disease is one that stops and then returns; the patient's eyes do not move, and they cannot exhale.
