谓目珠不正,人虽要转而目不能转。乃风热攻脑,筋络被其牵缩紧急,吊偏珠子,是以不能运转。

It is referred to as "misalignment of the eyeball," meaning that although the person attempts to turn their head, the eyeball cannot follow. This occurs when wind-heat attacks the brain, causing the meridians and collaterals to be pulled, contracted, and tense, thereby displacing the eyeball, which results in an inability to rotate freely.
In severe cases, one may hear a scraping sound within the eye, occurring intermittently. If there is blood stasis, the eye will also appear red and painful. If left untreated, there is a risk of the eyeball turning completely backward (fanbei). This condition resembles the initial stage of bilateral pupil abnormalities but differs in nature.
This is caused by the excess of one of the six qi, where wind-heat binds tightly, causing the eyeball to tilt and flip sideways, with the sclera facing outward and the iris/pupil facing inward. Medication cannot cure this; only manual manipulation (bo zhi) can be used. One must practice for a long time to become proficient, able to identify the direction of deviation or whether it is displaced upward or downward, before performing the manipulation. Then, the treatment can be completed easily ("in the turning of a palm"). Otherwise, the patient suffers pain in vain, and the physician wastes effort. Nowadays, people mistakenly call any case of blindness with internal cataracts, or damage to the wind-wheel and water-wheel (sclera and cornea/iris), where the tissues are ruined, mixed turbidity, and white covers black, as "pupil fanbei." This is erroneous. "Fanbei" actually refers to the dark part of the eye tilting inward; how can an eyeball facing outward normally be called "fanbei"?
"Fenglun破碎" (Shattering of the Wind Wheel) refers to the internal aqueous humor/juice of the cornea overflowing layer by layer. If untreated, in severe cases, the juice will be exhausted, leading to the collapse and atrophy of the eyeball. Some cases involve spontaneous rupture and discharge, allowing fire qi to dissipate; if neither internal nor external conditions are treated, the qi stagnates, causing swelling and protrusion that cannot retract. In some instances, physicians use cold-cooling herbs to drive away internal fire, neglecting external balance, resulting in stagnant qi becoming a fixed protrusion; this is also incurable. Initially, it arises from wind-heat attack or physical trauma damaging the true juice, causing extreme hyperactivity of blood and qi stasis, which shatters the divine pearl. The juice inside the divine pearl has all ulcerated and exited. Even with skilled hands, it cannot be saved; one can only prevent collapse and protrusion. The juice and fluid on the eyeball mix and form nebulae (yi), resembling a white cataract; southerners call this "white fruit." Even if Hua Yuanhua were reborn, what could he do?
Observing deep within the pupil, a stream of qi moves faintly and curlingly, resembling a wisp of clear smoke reflected in a distant bright mirror. Patients suffering from head wind-phlegm-fire diseases, where stagnation leads to excessive fire over time, agitate the true qi within the collaterals, causing it to scatter and dissipate. Urgent treatment is required. If, after the movement subsides, vision remains dim and obscure, it indicates that an internal syndrome has formed.
