The eyelid margin turns inward and adheres to the outer eyelid, resembling the shape of a tongue licking the lips. This is due to Qi stagnation and blood surging internally, causing the skin to become tense and tethered externally, thus preventing it from returning to its original position. Some cases involve inversion caused by internal obstruction of pathology; others occur when the inverted eyelid becomes diseased, where wind-heat binds and obstructs, preventing return and causing inversion. Generally, this is mostly caused by the obstruction of wind-dampness. Therefore, patients with wind diseases are more common, and treatment is difficult to cure. Non-wind cases are easier to treat. The method of picking open and guiding (Ti Kai Dao) should be used.
This refers to the eyelid pulling, twitching, and trembling spontaneously without human intervention to open or close it. It is a disease of the Qi level, belonging to the disorders of pulling and trembling in the Liver and Spleen meridians. People all call it "wind," but they do not realize that it is blood deficiency leading to unsmooth Qi, not pure wind. Only if there is damp rot or head wind disease is it due to wind pathogen. If untreated for a long time, it becomes a disease of pulling and disfigurement.
This refers to the flesh inside the eyelid appearing purple, stagnant, and floating, resembling the appearance of foul blood rotting and floating. The color shows purple halos rising, and in severe cases, it becomes fine like bubbles, countless and connected in patches. This is because the collateral vessels of the eyelid have blood stasis, and one does not avoid fire toxins and dry/greasy substances, leading to accumulation without dispersion. The blood loses its clarity and luster, becoming stagnant and floating, causing the eyelid flesh to decay. Or, broken eyelids bleed, thereby exposing to wind; wind damages the blood, making the blood sluggish and preventing the eyelid flesh from being moistened. This is a chronic accumulated disease, unlike acute sudden illness. Treatment prioritizes invigorating blood; for severe cases, only then use drastic purgative methods; for mild cases, simple dispelling methods suffice.
This refers to the eyelid swelling like a ball. There are no other eye diseases initially, but over time, red veins and chaotic pulses may appear. If fire is heavy, the skin may turn red, but the eyes do not hurt. If damp-phlegm and fire clump together, there will be tearing and signs of ulceration. This is a deficiency syndrome of fire at the Qi level; it must not be mistaken for a fullness syndrome of the Blood level, such as swelling like an inverted cup. Rubbing it with warm palms provides slight relief temporarily, but it returns to normal shortly after, showing blood insufficiency and deficient fire congesting at the Qi level.
Zhu Danxi said: Wind along the eye system above the diaphragm has accumulated heat. From diet combined with anger, stubborn phlegm and obstruction form. Turbid Qi fails to descend, clear Qi fails to ascend. Thus, fire becomes more blazing while water descends further. Over time, the eyelid margins swell due to pus exudation, and tiny worm-like filaments grow inside, resulting in chronic non-healing conditions with much itching. Use Zijin Ointment (Purple Gold Ointment); wipe off grease with the foot of a silver hairpin before applying. Ask yourself: if it is indeed itchy, you must remove worms to cut off the root. For Zijin Ointment merely removes dampness, wind, and cools the blood. If the aforementioned condition of diet and anger forming phlegm exists, one must additionally use Fangfeng Tongsheng San (Saposhnikovia Decoction for Unblocking the Master), removing Rhubarb (Da Huang) and Mirabilite (Mang Xiao), grinding into fine powder, mixing evenly with wine, drying, and taking according to the method. Prohibit all rich flavors and strong spices to fully achieve the essence of these treatments.
[Wind Margin Red Rot Syndrome] refers to the redness, rotting, and greasy dirt along the eyelid margin. This is because blood deficiency and fluid scarcity fail to nourish the eyelid flesh, leading to damp-heat stagnating in the Spleen collaterals. Chronic redness and rotting here differ from wind attacking due to deficiency. If untreated for a long time, eyelashes invert inward; if damage is severe, redness, rotting, wet dirt, and lash destruction occur. If nebulae (cataracts/opacities) exist prior to red rot, it is due to meridian sluggishness and unclear Shen Water (aqueous humor) causing rot. Treating the nebula and unblocking the meridians leads to self-healing. Some cases arise from plucking or trimming inverted lashes, damaging essence fluids and introducing wind pathogens, leading to decay. Treat each according to its source. One method involves drastic treatment using a small soldering iron wrapped in paper, dipped in tung oil, heated red, and cauterized; this is quite effective for rotting, wet, and itchy conditions. Neglecting internal treatment makes eradication difficult.
[Wind-Induced Red Rot Syndrome] refers to the eyes becoming red and rotting upon exposure to any wind, but remaining normal without wind. This differs from Wind Margin Red Rot which penetrates deeply into the Spleen collaterals. Wind belongs to Wood; Wood is strong and Earth is weak. Weakness makes it susceptible to invasion. Pathogens attract pathogens, attacking from inside and outside. Earth is overcome by Wood, so disease occurs with wind and heals without wind. Red rot is the primary disease of Wood and Earth. Redness indicates Fire within Wood; rotting indicates Dampness within Earth. If Phlegm and Dampness are abundant, rotting exceeds redness. If Fire and Dryness are abundant, redness exceeds rotting. If Heart receives Lung Qi, the eyeball also hurts and turns red. This specifically discusses the affliction of seeing wind causing red rot, which differs from the subsequent chapters on facing East/West or wind-induced cold/tears penetrating deeply internally.
[Canthus Red Rot Syndrome] refers to red rot occurring only at the canthus (eye corners), with no other eye diseases. If other eye diseases cause red rot, it is due to other fire pathogens injuring the canthus, which is different. Redness exceeding rotting indicates excess Fire, caused by mental labor, depression, anger, and injury from formless fire. Rotting exceeding redness indicates excess Dampness, caused by indulgence in dry foods and alcohol, excessive crying, exposure to fire/smoke, and steaming by wind-heat, representing injury from tangible pathogens. The disease belongs to the Heart collaterals. In severe cases, excessive fire and unclear water lead to sores at the margins. One must distinguish between inner and outer canthi, and the concepts of Ministerial Fire vs. Sovereign Fire, and deficiency vs. fullness. Take Xidao San (Scalpel Powder) and Juhua Tongsheng San (Chrysanthemum Decoction for Unblocking the Master) internally. Wash with Huanglian San (Coptis Powder). Apply Luganshi San (Galena Powder). Apply Er Can Sha (Bombyx Batryticatus) soaked in sesame oil for over a month, filtered through thick cotton. Apply Zijin Gao (Purple Gold Ointment) made from water-classified Galena, Guo Dan (Lead Oxide), honey, and little water, boiled on gentle/fire heat, stored in a vessel, and applied. To treat eye redness and blindness, clean green mud maggots thoroughly, dry and powder them. Lie supine with eyes closed, place one qian of medicine on the eye. After a moment when the medicine acts, remove it after a short time, and redness/blindness will naturally disappear. Li Dongyuan said: Chronic red rot of the eye socket is commonly called Chi Xia (Red Blindness). Often prick the outer eye with a three-edged needle to drain damp-heat, leading to immediate recovery. Secret acupoints for treating wind-margin rotting eyes: Da Gu Kong (Great Bone Emptiness), located at the tip of the second joint of the thumb. Moxibustion 9 cones, extinguished by blowing. Xiao Gu Kong (Small Bone Emptiness), located at the tip of the second joint of the little finger. Moxibustion 7 cones, also extinguished by blowing.
