Saozi San (Soap Nut Powder): Treats swelling and pain caused by water-toxin entering sores, or foreign objects penetrating into the bone.
Ingredients: Gleditsiae Sinensis Semen (Zaojiazi, 7 seeds), large dried toad (1 piece), Piperis Nigrum Fructus (Hujiao, 50 grains).
Preparation and Administration: Place the ingredients in a dry pot, cover the mouth of the pot with a tile, and heat slowly over a low fire until the smoke is exhausted. The *Suwen* states: When a person falls from a height, evil blood remains inside, causing abdominal fullness and distension, and inability to pass stool or urine. One should first drink purgative medicines. This injury damages the Jueyin meridian above and the Shaoyin collateral below. Acupuncture should be performed at the area below the medial malleolus of the foot, anterior to Ran Gu (BL64), to bleed the vessels. If needling the arterial pulse on the dorsum of the foot is ineffective, needle each of the three hairs on the big toe (near Yinbai SP1), one pricking per site, until bleeding occurs. Treat left-sided symptoms by needling the right side, and right-sided symptoms by needling the left. For symptoms of sadness, fright, and unhappiness, follow the same needling method. (From *Miao Ci Lun*)
The *Lingshu* states: Falling causes evil blood to remain inside. Great anger causes Qi to rise but not descend, accumulating under the hypochondrium, thereby injuring the Liver. Additionally, wind-stroke, being struck or fallen, or having intercourse while drunk with sweat exposed to wind, injures the Spleen. Regarding headache where acupuncture at a specific point is contraindicated due to being struck or fallen, with evil blood remaining inside, if muscle injury causes persistent pain, lateral needling should be used rather than distant points. (From *Xieqi Zangfu Bingxing Pian* and *Juebing Pian*)
Li Dongyuan discusses in *Yixue Faming*: When falling from a height, evil blood remains inside. Regardless of which of the twelve meridians is injured, physicians often treat it as a Wind-stroke affecting the Liver meridian, assuming blood accumulates under the hypochondrium and treating it as Wind-stroke. Blood is governed by the Liver; evil blood inevitably returns to the Liver. Regardless of the initial meridian injury, evil blood must accumulate under the hypochondrium because the Liver governs blood. Severe pain leads to spontaneous sweating. Since everyone sweats, these symptoms are attributed to Wind patterns, and all Wind patterns belong to Liver Wood. Moreover, congealed bad blood reverses its flow and enters the Liver. Falling from a height reverses the ascending Qi and Blood; who else but the Liver? This differs from Cold Damage where there is no sweating. Since it is stated that "sweat is transformed by Wind," therefore, blood-breaking and meridian-moving herbs are used for treatment.
