When a carbuncle or furuncle (a severe purulent infection) first appears as one or two small spots within one or two days, immediately take a head of garlic, slice it horizontally to the thickness of a coin, and apply it to the center of the lesion. Place small moxa cones on top for moxibustion, stopping after five *zhuang* (units of moxa burning). If the lesion is slightly larger, soak yellow stalk paper in wine and apply it completely over the area. Identify the place that dries first as the "jin jiao" (the root where pathogenic toxins gather), and perform moxibustion there; if two places dry first, moxicate both, but limit the total to five to seven *zhuang*. Another method involves using fingers to palpate from the periphery inward; the point that elicits pain indicates the root of the toxin. Press deeply at this point until the patient feels relief, then perform moxibustion here, and also moxicate other areas. If swelling is extensive, crush garlic into a cake, bake it dry, dip it in vinegar, heat it, and apply frequently with replacement. Alternatively, use an ironing pot to warm the garlic cake on the skin, replacing with hot cakes frequently. If it feels like the pathogenic qi is dispersing from the affected area, cover it with silk gauze to prevent heat loss, and apply medicinal paste after a short while. Whenever a carbuncle spreads and enlarges resembling the shape of a turtle, observe the direction of the "head." First moxicate the two front "legs," then the "tail." If red meridians are tight and long, start moxication from the distal end, but must preserve the "head" and the two rear "legs" without moxication. If all parts are moxicated, fire qi will become stagnant, toxins will have no outlet to disperse, and may instead penetrate inward. If identification is unclear, decoct Baizhi (3 fen), Hanjiao, Sangbaipi (1 fen each), and Lianxu Congbai (10 slices) in fresh water, add half a cup of sour vinegar, and wash the area. After a moment, the meridians will naturally appear, allowing differentiation of the head and tail.
Shenxian Ge Suan Jiu Fa (Immortal's Garlic-Partitioned Moxibustion Method): Treats various carbuncles, sores, swellings, and toxic masses, as discussed in detail previously. For conditions involving severe pain, no pain, or numbness with pain, continue moxication until pain ceases, or until no pain becomes painful. The toxin dissipates with the fire; this is a treatment method that guides stagnant toxins outward, truly possessing the merit of restoring life. The method uses garlic slices 3 fen thick placed on the sore. Moxa cones are applied on the garlic; after five cones, replace the garlic and continue. For unformed abscesses, this can resolve them; for formed ones, it reduces their severity. For large lesions, crush garlic and spread it on the affected area, place moxa on top, and moxicate. Replace the garlic when it spoils. For lesions with purple coloration, or white coloration without elevation, suppuration, or significant pain, frequent moxication regardless of days is most suitable.
Shenxiao Cong Yi Fa (Miraculous Scallion-Ironing Method): Treats deep-seated abscesses (Liuzhu), tubercles, bone carbuncles, and knee-joint deformities (Hexi Feng) characterized by swelling and hardness. It is particularly suitable when residual swelling persists after garlic-partitioned moxibustion. Ironing helps promote Qi and Blood circulation and resolves stagnation, yielding great efficacy. It is also an excellent method for treating traumatic injuries, relieving pain, dispelling blood stasis, and reducing swelling. Crush scallion whites, stir-fry until hot, and frequently iron the affected area, replacing when cool.
Shenxiao Sang Zhi Jiu (Miraculous Mulberry Branch Moxibustion): Treats back carbuncles (Fabei) that fail to suppurate or necrotic tissue that does not ulcerate, caused by deficiency of Yang Qi. Ignite mulberry branches, extinguish the open flame, and use the smoldering branch to fumigate/moxicate the affected area for a short period, three to five times daily, to assist in swelling resolution and suppuration. If necrotic tissue has been removed but new flesh grows slowly, moxicate the periphery of the wound. This method is also suitable for yin sores, scrofula (Luoli), Liuzhu, and chronic malignant sores that do not heal for a long time. Generally, for unulcerated lesions, this method resolves heat toxins, stops pain, and reduces stasis and swelling; for ulcerated lesions, it tonifies Yang Qi, disperses residual toxins, and promotes flesh and bone regeneration. For Yang-pattern swelling and pain, even those heavy as carrying a stone, using this method at the onset to draw out toxic fluid leads to internal resolution. For chronic cases, even if ulcerated, the depth is shallow and suffering is minimal. Regrettably, patients are unaware of this method, and practitioners are reluctant to use it.
