Yan Zili said: The disease of scabies (Scabies) and crusts, although painful but not fatal, has many difficult-to-cure cases. The *Suwen* (Basic Questions) states: All pain, itching, and sores belong to the Heart (Zang organ governing blood and spirit). It is mostly caused by stagnation of Heart Qi, or irregular diet, leading to toxin accumulation in the intestines and stomach; manifesting on the skin. Ancient prescriptions mention types such as Horse Scabies, Water Scabies, Dry Scabies, and Wet Scabies, varying in kind, occurring on hands and feet or covering the whole body. Symptoms may include itching, pain, ulceration with pus, swelling, uneven skin texture, raised bumps upon scratching, or oozing pus. Treatment internally should regulate Heart blood and dispel Wind-Heat; externally apply compresses and washes, ensuring no case remains incurable. Scabies and crusts are all generated by Wind-Heat, causing whole-body itching, skin elevation upon scratching, bleeding or weeping fluid, forming dry crusts, containing insects which people often pick out with needle tips, resembling midges in water. This is due to severe invasion of Wind pathogen and Heat qi between muscles and flesh.
[Zhu Danxi] For sores in the upper body, frequently administer Fangfeng Tongsheng San (Saposhnikia Decoction for Unblocking the Supreme); for the lower body, use Cangyong Wan (Hidden Use Pill) to purge downwards; for swollen legs with bleeding, add herbs treating Damp-Heat in the Blood level.
[Xue Ji] Scabies belongs to Damp-Toxin and accumulated Heat in the Spleen Meridian, or Blood-Heat and Wind-Heat in the Liver Meridian, or Yin Deficiency Fever in the Kidney Meridian. Fatigue and poor appetite indicate Spleen Meridian Damp-Heat, treat with Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). Pain upon drinking cold water indicates Spleen Meridian Accumulated Heat, treat with Qingre Xiaodu San (Clear Heat and Detoxify Powder). Itching and fever indicate Spleen Deficiency with Wind-Heat, treat with Renshen Xiaofeng San (Ginseng Dispersing Wind Powder). Itching and pain indicate Wind-Heat, treat with Danggui Yinzi (Angelica Decoction). Constipation and pain indicate Heat Toxin, treat with Shengma Heqi Yin (Cimicifuga Harmonizing Qi Decoction). Heat, thirst, and normal bowel movements indicate Spleen-Lung Deficient Heat, treat with Zhuye Shigao Tang (Bamboo Leaf and Gypsum Decoction). Internal heat, afternoon tidal fever, or alternating chills and fever indicate Liver Meridian Blood Deficiency with Wind-Heat, treat with Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) and Liuwei Wan (Six-Ingredient Pill). Fatigue, reduced food intake, night sweats, and insomnia indicate Spleen Qi Stagnation, treat with Jiawei Guipi Tang (Modified Restore the Spleen Decoction), Xiaoyao San, and Dihuang Wan. If fever, night sweats, phlegm expectoration, and dry mouth occur, it indicates Kidney Meridian Deficient Heat, treat with Liuwei Dihuang Wan decoction. Li Longgang, a clerk, suffered from this all over his body, especially legs and feet, worsening at dusk, with dry mouth, thirst, frequent red urine; this was Kidney Meridian Deficient Heat, cured with Buzhong Yiqi Tang and Liuwei Dihuang Wan. A scholar prone to sneezing and itching: I believed internal Damp-Heat and loose pores allowed external pathogens to strike, prescribed Buzhong Yiqi Tang plus Bai Zhi (Angelicae Dahuricae Radix) and Chuan Xiong (Ligustici Chuanxiong Rhizoma). He refused, self-administered Jingfang Baidu San (Schizonepeta and Saposhnikia Detoxifying Powder), resulting in night sweats, fever, thirst, pain, and profuse pus; still treated with the previous decoction doubled Ren Shen (Ginseng Radix et Rhizoma), Huang Qi (Astragali Radix), and Wu Wei Zi (Schisandrae Fructus), cured. Another scholar had lesions on arms and legs, itching or distension at dusk, fatigue, self-medicated with Baidu San, causing swelling at painful sites and red scanty urine; this was Lung-Kidney Yin Deficiency, treated with Buzhong Yiqi Tang plus Wu Wei Zi and Maidong (Ophiopogonis Radix), cured. Another scholar misused purgatives, depleting Yuan Qi, unable to heal; treated with Buzhong Yiqi Tang plus Fu Ling (Poria), lesions healed suddenly. Due to improper care, worsened at dusk, treated with Bazhen Tang (Eight-Treasure Decoction) plus Wu Wei Zi and Maidong for over fifty doses, cured. A man with dark complexion, itching, black blood discharge, worsening at dusk, legs swollen by day and subsiding by night; I considered Qi-Blood Deficiency with Heat, morning Buzhong Yiqi Tang; evening Jiawei Xiaoyao San, cured. A man after recovering from epidemic disease suffered similarly, cured with tonification. One patient using Bian Fa (Bloodletting) died; this was Yin Deficiency with Blood-Heat and dark itching, why harm Yin Blood? A woman with chronic non-healing sores, poor appetite and fatigue; Liver-Spleen Deficiency with Deficient Heat, first Buzhong Yiqi Tang plus Chuan Xiong and Chao Shan Zhi (Gardeniae Fructus fried), Yuan Qi gradually recovered, then Xiaoyao San, sores gradually healed. If delirium at night, Heat entering Blood Level, treat with Xiao Chaihu Tang (Minor Blue-Green Decoction) plus Sheng Di Huang (Rehmanniae Radix). For Blood Deficiency, Siwu Tang combined with Xiao Chaihu Tang; after heat subsides, use Xiaoyao San to tonify Spleen-Stomach and generate Yin Blood. Also chills and fever like malaria, treated with previous medicines.
