The nephew of Vice Minister Liang suffered a leg injury from a fall. He applied external poultices containing Rhizoma Polygoni Cuspidati (Dahuang) and other herbs, and took internal blood-breaking decoctions, which subsequently led to internal tissue ulceration. Xue Ji punctured the area, draining approximately three bowls of foul pus. The patient presented with comprehensive deficiency symptoms. After more than two months of treatment with major tonifying formulas, he could barely walk. Due to excessive mental exertion, he developed symptoms of hand relaxation, eye closure, and profuse sweating like water. Some suggested using wind-dispelling agents. Xue Ji stated: "This is due to insufficient Qi and Blood." He urgently had mugwort leaves (Artemisiae Argyi Folium) stir-fried until hot and frequently moxibusted the navel (Shenque, CV8) and Qihai (CV6) acupoints. He administered a decoction of four taels of Radix Ginseng (Renshen) and five qian of Prepared Radix Aconiti Lateralis Preparata (Paofuzi). After a long while, the arm showed slight movement. After administering another dose, the eyes opened and speech resumed, but the breath was unable to connect continuously. Therefore, he prescribed a formula of Radix Ginseng (Renshen), Rhizoma Atractylodis Macrocephalae (Baizhu), Radix Angelicae Sinensis (Danggui), and Radix Paeoniae Alba (Baishao)—totaling one jin—along with five qian of Radix Aconiti Lateralis Preparata (Fuzi), decocted in water and taken slowly. Gradually, the primordial Qi recovered, food intake resumed, and upon removing Fuzi and continuing the remaining prescription, the wound healed.
Chest and abdominal pain and distension caused by jumping, chest pounding, sprains, lifting heavy objects, labor, or anger, where the patient prefers being touched, indicates Liver Fire damaging the Spleen; treat with Four Gentlemen Decoction (Sijunzi Tang), Bupleuri Radix (Chaihu), and Gardenia Fruit (Shanzhi). If the patient fears being touched, it indicates Blood stasis in the Liver Meridian; treat with Four Substances Decoction (Siwu Tang), Chaihu, Shanzhi, Peach Kernel (Taoren), and Safflower (Honghua). If there is pain in the chest and hypochondrium accompanied by fever and afternoon tidal fever, it indicates damage to Liver Meridian Blood; treat with Modified Free and Easy Wanderer Powder (Jiawei Xiaoyao San). If there is chest/hypochondriac pain with poor appetite and lack of desire for food, it indicates damage to Liver and Spleen Qi; treat with Sijunzi Tang, Ligusticum Chuanxiong Rhizome (Chuanxiong), Danggui, Chaihu, Shanzhi, and Moutan Cortex (Danpi). If there is chest/abdominal fullness with poor appetite, it indicates Qi stagnation in the Liver and Spleen; treat with Six Gentlemen Decoction (Liujunzi Tang) plus Chaihu, Chuanxiong, and Danggui. If there is discomfort in the chest/abdomen, poor appetite, and insomnia, it indicates Spleen Qi depression; treat with Modified Guipi Decoction (Jiawei Guipi Tang). If there is difficulty in Phlegm-Qi movement indicating Spleen and Lung Qi stagnation, treat with Two-Chen Decoction (Erchen Tang), Baizhu, Chuanxiong, Danggui, Zhi Zi, and Green Tangerina Peel (Qingpi). If there is teeth grinding and convulsions, it indicates Liver Excess and Spleen Deficiency; treat with Minor Bupleurum Decoction (Xiaochaihu Tang) plus Chuanxiong, Shanzhi, Gastrodia Tuber (Tianma), and Uncaria Ramulus cum Uncis (Gouteng). Using wind-dispersing herbs will further injure Liver Blood and intensify Liver Fire. Drinking sweet wine will further deplete Kidney Water and exacerbate Liver Fire. Using cold herbs like Rhizoma Polygoni Cuspidati (Dahuang) will damage the Yin collaterals internally, leading to lower bleeding; young and strong individuals will likely develop chronic diseases, while the old and weak may die.
