[General Idea]
Lifeng (Leprosy/Malignant Wind) is a type of wind qi in heaven and earth that can kill all things, belonging to the Dry Metal qi. When it invades the human body, it causes sores that do not suppurate properly, appearing dry rather than moist. The circulation of Ying and Wei (Nutritive and Defensive Qi) becomes stagnant, leading to numbness and insensitivity. Hair falls out, eyebrows are lost, and this disease is extremely severe. Patients often neglect it due to laxity, failing to realize they must abstain from rich flavors and regulate desires, ultimately resulting in untreatable conditions. This is deeply regrettable. (From *Zhengzhi Huibu*)
[Etiology]
Lifeng was anciently called "Lei" (Leper). It is mostly caused by blood heat combined with cold exposure. For instance, bathing in water after strenuous labor in summer, or treading on ice while drunk in winter. This leads to damp-heat stagnating internally without dispersing, and wind pathogens lodging externally without moving. Internal and external heat become constrained. (Zhu Danxi's view) Over time, blood becomes turbid and qi becomes chaotic. Pathogenic qi interferes with Wei Qi, preventing normal transformation. Since qi cannot move freely, blood aggregates. Blood aggregation leads to tissue decay and rotting. (From *Yizong Jinjian*) This corresponds to the *Neijing* statement: "When heat prevails, flesh and blood rot." (Zhu Danxi's view) It may also result from violating taboos after suffering from carbuncles and sores, or from excessive alcohol consumption and rich foods causing extreme heat to generate wind. Here, deficiency is the root, and heat is the branch. (Sanxi's view)
[External Manifestations]
Severe damp-heat generates wind; excessive wind generates parasites, similar to the principle that decaying grass transforms into fireflies. (Sanxi's view) If there are parasites internally, there must be corresponding manifestations externally. Suddenly, skin and hair fall off, muscles swell and turn purplish. If the Liver is affected, eyebrows fall; if the Spleen is affected, the nose collapses; if the Lungs are affected, the voice becomes hoarse; if the Heart is affected, the feet ulcerate; if the Kidneys are affected, tinnitus occurs. There may be sores on the ears, or a sensation like needle pricks all over the body, feeling as if insects are crawling, with nodules in the muscles, ulceration, and sore formation. These are all external signs. (From *Zhengzhi Huibu*) If Qi is affected, symptoms appear in the upper body; if Blood is affected, symptoms appear in the lower body; if both Qi and Blood are affected, symptoms appear in both upper and lower parts. (Zhu Danxi's view)
[Treatment Focus on Yangming Meridian]
This disease primarily involves the Hand and Foot Yangming meridians because Yangming is where Qi and Blood are abundant, governed by the Stomach and Large Intestine, which are the Fu organs of the Lung and Spleen meridians. The Spleen governs muscles, and the Lungs govern skin and hair. Disease in the Fu organs affects the Zang organs, manifesting in the skin and muscles. Moreover, the intestines and stomach are like a market, containing and accepting everything, so heat toxins accumulate internally and manifest externally. Treatment must first target the Yangming meridians and then extend to the Taiyin meridians, representing the principle of treating the root while addressing the branch. (Zhu Danxi's view)
[Therapeutic Methods]
The treatment method must first eliminate the parasites and purge the fire, then nourish and cool the blood, dispel wind, guide out stagnation, descend Yang, and ascend Yin. These are urgent priorities in treatment. (Zhu Danxi's view)
