[General Principle]
The head and face are the convergence point of all Yang meridians. The twelve primary meridians all ascend to the face and connect with the orifices (from the Classic texts). Therefore, diseases of the face belong to the three Yang meridians (from *Zhengzhi Huibu*).
[Facial Pain]
Facial pain is generally attributed to Heat patterns. However, acute onset often indicates Excess patterns, while chronic conditions often indicate Deficiency patterns. Obstruction in eating is due to wind-toxin accumulation from excessive consumption of rich and sweet foods. Difficulty in eating and sleeping is nothing more than Stomach Deficiency with Fire (from *Zhengzhi Huibu*).
[Facial Swelling]
Facial swelling is termed "Wind" (from *Neijing*). It occurs when Wind and Heat pathogens struggle against each other after exposure to wind following a meal, attacking upward to the head and face. If the patient can eat, it is Wind-Deficiency, accompanied by numbness; if the patient cannot eat, it is Wind-Heat, necessarily presenting with facial redness (from *Yizong Rumen*).
[Facial Edema]
Excessive physical labor and irregular diet lead to insufficient Spleen Qi transformation and distribution, and impaired Lung Metal dissemination. Thus, edema under the eyes resembles a silkworm lying down, indicating a potential risk of generalized edema. If the pulse is floating, deficient, and weak, it indicates Spleen Qi insufficiency, requiring tonification of the Spleen to strengthen Earth. If the pulse is deep, solid, and forceful, it indicates obstructed Lung Qi, requiring descending Qi and draining water (from *Zhengzhi Huibu*).
[Facial Heat and Cold]
When the Qi of the Hand and Foot Yangming meridians is vigorous, the anterior aspect of the body feels hot (from *Yizong Rumen*). When the Qi of the Hand and Foot Yangming meridians is deficient, the anterior aspect of the body feels cold (from *Ling Shu*). Therefore, when Wind-Heat rushes upward, only the face becomes hot; when Cold-Dampness rebels upward, the face becomes unable to tolerate cold (from *Yizong Rumen*).
[Facial Sores]
When Wind pathogens soak the skin and Phlegm lodges in the Zang-Fu organs, the complexion becomes dark. When the Spleen is affected by Damp-Heat and the Lung by Wind pathogens, sores appear on the face. These may be painful or itchy, red and swollen, millet-grain-like, or change colors. In cases where the Lung is affected by Fire pathogens, causing coughing up of pus and blood, and red sores appear on the face, this is due to Kidney Water failing to ascend while Lung Fire flares independently (from *Zhengzhi Huibu*).
[Treatment Principles]
For high-seated pathologies, suppress them; for external invaders, disperse them. For Blood Heat, use cooling methods; for Qi Deficiency, use tonifying methods. Do not rely exclusively on bitter-cold herbs to descend Fire. If clearing Fire and tonifying Qi proves ineffective, one should resolve stagnation.
[Prescription Usage]
For Stomach Fire: Qingwei San (Stomach-Clearing Powder), add Huangqin (Scutellaria Root) and Shanzhi (Gardenia Fruit). For Wind Pathogens: Gegen Tang (Kudzu Decoction), add Bohe (Mint) and Jingjie (Schizonepeta). To regulate the Spleen and Stomach: Liujunzi Tang (Six Gentlemen Decoction), add Shengma (Cimicifuga Rhizome) and Gange (Kudzu Root).
[Appendix: Cheek Disorders]
If swelling extends to the tooth roots, it is Stomach Fire. If swelling involves both cheeks, it is Gallbladder pathogen influence. As before, ability to eat indicates Wind-Deficiency, inability to eat indicates Wind-Heat (from *Yizong Rumen*). If swelling is caused by Damp-Heat and pain by excessive Fire: Generalized swelling around both ears is named "Fayi" (Parotitis). Treat with Ganji Tang (Licorice and Platycodon Decoction) plus Bohe (Mint), Jingjie (Schizonepeta), Lianqiao (Forsythia), Fangfeng (Saposhnikovia), Huangqin (Scutellaria), and Niubangzi (Arctium Seed).
