If a pregnant woman experiences hematemesis (vomiting blood) due to contracting wind-heat pathogenic factors, she will inevitably have coughing. If prolonged coughing leads to expectoration of blood, Xiangsu Yin (Fragrant Perilla Decoction) with added Huangqin (Scutellariae Radix, 黄芩) and Xiangbei (Fritillariae Thunbergii Bulbus, 象贝) should be used to clear and disperse wind-heat, stop the cough and bleeding, thereby resolving the hematemesis.
Alternatively, if hematemesis is caused by fetal heat and stomach fire rushing upward, An Tai Yin (Fetus-Calming Decoction) with added Huangqin (Scutellariae Radix, 黄芩) and Maidong (Ophiopogonis Radix, 麦冬) should be used exclusively. Shengdihuang (Rehmanniae Radix, 生地黄) should be small-sized only, so that fetal heat is cooled and blood returns to its normal course. If hematemesis results from consuming fried, roasted, or grilled foods, causing stomach fire to rebel upward, Qingwei San (Stomach-Clearing Powder) without Mudanpi (Moutan Cortex, 牡丹皮), but with added Maidong (Ophiopogonis Radix, 麦冬) and Huangqin (Scutellariae Radix, 黄芩), is the primary treatment. For those with inherent weakness and a history of hemorrhagic disorders, Liuwei Dihuang Tang (Six-Ingredient Rehmannia Pill) with added Digupi (Lycii Cortex, 地骨皮), Chai Shihu (Dendrobii Caulis, 钗石斛), Maidong (Ophiopogonis Radix, 麦冬), and Niuxi (Achyranthis Bidentatae Radix, 牛膝) should be used to settle the deficient fire naturally.

Hematochezia (blood in stool) in pregnant women is also caused by reckless movement of blood due to blood heat. Generally speaking, for hemorrhagic diseases, upward flow is considered adverse (ni), while downward flow is considered favorable (shun). It is appropriate to administer Shengdi Siwu Tang (Rehmannia Four Substances Decoction) with added Maidong (Ophiopogonis Radix, 麦冬), Huaimi (Sophorae Flavescentis Semen, 槐米), Chi Shao (Paeoniae Radix Rubra, 赤芍), and Gancao (Glycyrrhizae Radix et Rhizoma, 甘草). If it is Zangdu (visceral toxin/colonic dysentery with heat-toxin), adding Jinyinhua (Lonicerae Japonicae Flos, 金银花) will lead to self-healing.
While the fetus is in the womb, there is always fetal heat. Dryness of the stool is common. One must absolutely avoid straining during defecation; many miscarriages occur due to this, as excessive straining damages the fetal qi, causing the fetus to descend. There are also cases where straining injures the amniotic sac, leading to abortion. This occurs because the fetus is pressured downward by qi; the more urgent the pressure, the harder it is to pass stool. In such cases, one should only take Shengdi Siwu Tang (Rehmannia Four Substances Decoction) with added Songzi (Pinus Seed, 松子) and Hei Zhima (Sesami Nigris Semen, 黑芝麻), or Maidong (Ophiopogonis Radix, 麦冬) and Gualou (Trichosanthis Fructus, 栝蒌), mixed with vegetable oil or raw sesame oil and taken with the decoction. These methods aim to moisten the intestines. Medications with slippery and purgative properties, such as Dahuang (Rhei Radix et Rhizoma, 大黄), are strictly contraindicated.
