For children under five years of age, it is not feasible to palpate the Cun (Inch), Guan (Bar), and Chi (Cubit) positions; instead, one should observe the fingerprint on the tiger's mouth area, distinguishing between males (left hand) and females (right hand).
The first segment of the index finger at the Yin position is called the Wind Gate (Feng Guan); if the fingerprint is visible here, the condition is easy to treat. The second segment at the Mao position is called the Qi Gate (Qi Guan); if visible, the disease is deep-seated. The third segment at the Chen position is called the Life Gate (Ming Guan); if visible, the life is in danger.
A purple fingerprint indicates Heat. A red fingerprint indicates Shanghan (Cold Damage). A cyan fingerprint indicates Jing Feng (Wind-Stroke/Convulsions). A white fingerprint indicates Gan Ji (Malnutrition/Digestive Disorder). A yellowish fingerprint that is faintly visible represents a normal pulse pattern. A black fingerprint generally indicates a critical condition.
If the fingerprint extends straight into the palm, it is termed "Inner Hook" (Nei Gou). If the fingerprint curves inward, it indicates Wind-Cold. If it curves outward, it indicates Food Stagnation.
For children over five years of age, use one finger to palpate the three positions: Cun, Guan, and Chi. Seven beats per respiration indicate a harmonious state; eight or more beats indicate Heat; four to five beats indicate Cold.
For infants under half a year old, examine the area between the forehead, the eyebrows, and the hairline using the ring finger and middle finger (and potentially the index finger as per traditional methods described). If the infant's head is to the left, the physician raises the right hand to examine; if the head is to the right, the physician raises the left hand. The part of the index finger near the hairline is considered Upper; the part of the ring finger near the eyebrows is Lower; the middle finger is Middle. If all three areas are hot, it indicates external contraction of Wind, manifesting as nasal congestion and cough. If all three areas are cold, it indicates external contraction of Cold combined with internal injury from diet, manifesting as fever, vomiting, and diarrhea. Heat in the proximal parts of the index and middle fingers indicates upper heat and lower cold. Heat in the ring and middle fingers indicates complication with Jing (convulsions/fright). Heat in the index finger alone indicates food stagnation.
In Shanghan (Cold Damage), before sweating, Yang pulses are favorable, while Yin pulses are contraindicated. After sweating, Yin pulses are favorable, while Yang pulses are contraindicated.
