The *Binhu Maijue* (Li Shi Zhen's Pulse Classification) contains songs describing the diseases associated with each pulse, but they only mention the general outline. I compiled the *Maixiang Tonglei* (Comprehensive Classification of Pulse Images), detailing the diseases governed by each pulse. However, the content is fragmented and lacks coherent literary flow, making it difficult to memorize and recognize. Therefore, imitating Li Shi Zhen's method, I composed poems on the diseases governed by twenty-seven pulses. Readers who study these poems and cross-reference them with the *Maixiang Tonglei* will gain a clear understanding of which pulse governs which disease and which disease corresponds to which pulse, thereby achieving profound insight.
(The floating pulse [Fu Mai] can be felt with light pressure but disappears with heavy pressure; its movement is located above the muscle layer.)
The floating pulse belongs to Yang and indicates exterior syndromes, which is definitive. A floating pulse combined with a slow [Chi] pulse suggests wind; combined with a rapid [Shu] pulse, heat; combined with a tight [Jin] pulse, cold. (This refers to the combination of the floating pulse with slow, rapid, or tight pulses; other combinations follow this pattern.) A floating pulse that is forceful indicates wind-heat; a floating pulse that is weak indicates blood deficiency in the person. (This stanza generally describes the diseases governed by the floating pulse.)
At the Cun position on the left: headache, dizziness (vertigo), and body heat are caused by wind; additionally, wind-phlegm affects the left Cun. At the Cun position on the right: coughing is caused by wind pathogen invading the lungs. At the Guan position: weakness of the Spleen on the right leads to abdominal fullness and anorexia; Qi stagnation on the left causes abdominal distension. At the Chi position: frequent urination with red, scanty, and painful urine at the left Chi indicates Bladder wind-heat; difficulty in defecation at the right Chi occurs because wind pathogen resides in the Lower Jiao, causing constipation.
A floating pulse that is forceful is the Surging pulse [Hong Mai] (also known as the Large pulse). Its image is extremely large and rapid; upon deep pressure, it feels full under the fingers, like waves surging up—strong in arrival and weak in departure, large in onset and small in decline.
The Surging pulse indicates Yang excess and relative Yin-blood deficiency. Flaring ministerial fire [Xiang Huo] is prevalent, commonly seen in heat syndromes. Abdominal fullness and rebellious Stomach Qi causing vomiting require early treatment; diarrhea and dysentery due to Yin deficiency must be urgently cleared. (This stanza generally describes the diseases governed by the Surging pulse.)
Excess fire in the Heart channel causes inner irritability (Left Cun syndrome, accompanied by red eyes, mouth ulcers, and headache). Lung heat causes dry hair and parched throat (Right Cun syndrome, accompanied by sticky saliva). Liver fire causes body pain (Left Guan syndrome, accompanied by floating heat in the four limbs). Stomach deficiency causes vomiting (Right Guan syndrome, accompanied by dry mouth and tongue). Kidney deficiency with Yin fire makes urination difficult (Left Chi: Bladder heat, red and scanty urine; Right Chi: abdominal fullness, difficult defecation, or hematochezia).
A floating pulse that is weak is the Scallion-Stalk pulse [Kou Mai]. Its image is floating, large, and soft; upon heavy pressure, the center is empty while the sides are firm, feeling like a hollow pit under the fingers. It is diagnosed between light lifting and heavy pressing.
Left Kou pulse: hematemesis and epistaxis accompanied by Heart blood deficiency (Left Cun syndrome). At the Guan position, it signifies true blood stasis causing hypochondriac pain (abdominal blood stasis, Qi and blood pain in the hypochondrium, hematemesis, dim vision). In men, Left Chi indicates hematuria; in women, it is the cause of menstrual disorders. (This stanza exclusively describes the diseases governed by the left hand Kou pulse.)
Right Kou pulse: accumulated blood in the chest (Right Cun syndrome, accompanied by epistaxis and hematemesis). Meeting the Kou pulse at the Guan position indicates intestinal abscess, accompanied by hematemesis, anorexia, and blood stasis. The Chi position often shows bloody dysentery and intestinal bleeding (hematochezia). (This stanza exclusively describes the diseases governed by the right hand Kou pulse.)
A floating pulse that is straight and rigid is the Bowstring pulse [Xian Mai]. Its image does not shift upon pressing, responds to lifting, and is straight like a zither string.
Left Xian pulse: headache and cardiac palpitations/fear, night sweats, and fatigue/laziness due to overwork. At the Guan position on the left, hypochondriac pain accompanies accumulations [Pi]. Pain at the Chi position involves the lower abdomen and leg spasms. (This stanza exclusively describes the diseases governed by the left hand Xian pulse.)
At the Right Cun position, excessive phlegm in the diaphragm causes wheezing and coughing (due to Wind-Cold invading the Lungs). At the Right Guan position, Cold Stomach causes abdominal and heart pain (Spleen and Stomach injured by cold, undigested food retention, excessive drinking). Water retention in the Lower Jiao presents with a Xian pulse at the Chi position; Yin hernias [Yin Han] often invade from below the navel. (This stanza exclusively describes the diseases governed by the right hand Xian pulse.)
A floating, slow, and large pulse is the Deficient pulse [Xu Mai]. Its image is slow, soft, and scattered/large; both light and heavy pressures feel weak, hollow and empty, unable to maintain itself.
The Xu pulse (due to Qi and Blood deficiency, the pulse is also deficient); body heat indicates Summerheat injury. Deficiency, fatigue, and irritability with spontaneous sweating. Fever due to Yin deficiency should be treated early; nourishing Ying and benefiting Qi should not be delayed. (This stanza generally describes the diseases governed by the Deficient pulse.)
Palpitations [Zheng Chong] and fright/panic [Jing Ji] are commonly deficient at the Cun position; if Blood fails to nourish the Heart, what can be done? Abdominal distension at the Guan position (undigested food); Bi syndrome and Wei syndrome at the Chi position; damage to Essence and Blood leads to bone-steaming tidal fever. (This stanza collectively describes the diseases governed by the Deficient pulse on both hands.)
A floating, slow, and fine pulse is the Soggy pulse [Ru Mai] (also known as the Soft pulse). Its image is deficient, soft, and weak; responding to the fingers, it is fine and scattered, like cotton wool in water. It is felt with light pressure but disappears with heavy pressure.
The Ru pulse indicates loss of Blood and Yin deficiency. The Sea of Marrow and Dantian are secretly depleted. Night sweats fall like rain, steaming into the bones; blood collapse falls down, resembling dampness soaking the Spleen. (This stanza generally describes the diseases governed by the Soggy pulse.)
Left Cun Heart deficiency (thus palpitations and night sweats) is accompanied by shortness of breath. Left Guan Ru pulse (accompanied by disharmony of Ying and Wei, physical weakness and lack of strength). Left Chi in men indicates essence depletion; in women, blood loss. Spontaneous sweating profusely and frequent urination are all present. (This stanza exclusively describes the diseases governed by the left hand Ru pulse.)
Right Cun aversion to cold and sensation of internal heat; how can one be at ease with Qi deficiency and physical fatigue? Right Guan Stomach deficiency (anorexia) and weaker Spleen (indigestion). Right Chi intestinal deficiency causes diarrhea, and the Lower Yuan is cold. (This stanza exclusively describes the diseases governed by the right hand Ru pulse.)
A floating and continuous pulse is the Long pulse [Chang Mai]. Its image is neither too large nor too small, continuous and natural, feeling abundant under the fingers, extending beyond its normal position.
When Qi and Blood are in excess, the Long pulse appears (the Long pulse governs excess syndromes). The heat intensity of the Yangming channel is naturally deep. If not Yang Toxin [Yang Du] (accumulated internally), then Epilepsy [Dian He Xian], or scorching irritability and high fever invasion.
A floating, deficient, and large pulse is the Scattered pulse [San Mai]. Its image has surface but no depth, Yin but no Yang; although it feels full under the fingers, it is scattered and not gathered, coming and going unclearly, without root.
Left Cun palpitations; Right Cun excessive sweating. Overflowing fluid [Yi Yin] at the Left Guan should manifest as soft and scattered. Soft and scattered at the Right Guan indicates swelling. If the Scattered pulse resides at both Chi positions, the Soul is destined to break (poor prognosis).
