In cases of metallic trauma penetrating the interior, with significant internal hemorrhage, if abdominal distension and fullness occur, accompanied by distension in both hypochondriac regions and an inability to eat, the prognosis is fatal. If blood stasis remains within, causing abdominal distension, and the pulse is firm and large (indicating severe stagnation), the condition is critical.
For injuries caused by bows and crossbows, if the arrowhead is poisoned and penetrates the skin and vessels, it causes shortness of breath, leading to death within moments. Symptoms include lockjaw, dry lips, cessation of blood circulation, abdominal fullness, muteness, and a delirious state resembling drunkenness. Before complete death occurs, such cases are considered incurable. If Ying and Wei (Nutritive and Defensive Qi) show cyanotic stasis, and blood should flow out immediately, with the wound edges being warm, this indicates a venomous arrow injury.
There are three types of venomous arrows: In the Lingnan region, ethnic minorities use scorched copper for arrowheads; secondly, in various places north of the岭 (Ling Mountains), juices from various snakes and insects are applied inside the arrow shafts to soak the arrowheads. These two types cause immediate severe swelling, boiling ulceration, and death upon skin penetration. However, when shooting pigs or dogs, although they may become weak, they often survive because they consume feces. If humans are struck by these arrows, they must immediately consume feces or drink fecal juice, and apply it to the wound to recover. Otherwise, rescue is impossible after a short while. If the arrow strikes a wider area, although the condition is critical, gradual treatment can lead to improvement, and death is not inevitable. If the arrow is near the chest or abdomen, rapid treatment is essential; any delay allows the toxin to enter the interior, making it incurable.
This refers to injuries from spears or arrows. If the abdomen is struck, Qi agitation occurs, causing the intestines to protrude through the wound opening due to the agitation of Qi.
Regarding metallic trauma resulting in intestinal rupture, survival depends on the depth of the injury. If only one end of the intestine is visible at the break, reconnection is not feasible. If abdominal pain, shortness of breath, and inability to eat occur, death follows in one and a half days for large intestine rupture, and three days for small intestine rupture. If both ends of the ruptured intestine are visible, rapid suturing is required. First, suture the broken ends using needles and thread according to proper methods. Then, apply chicken blood to the junction to prevent gas leakage, and push the intestine back into the abdominal cavity. If the intestine is merely prolapsed but not ruptured, prepare millet porridge, extract its juice to wash the intestine, moisten it with water, and reduce it into the cavity. The patient should also drink rice gruel. After more than twenty days, gradually introduce thick liquid food. Only after one hundred days can regular rice be consumed. Overeating leads to intestinal pain, rupture, and internal leakage. Regularly administer Qianxie San (Coin Scrap Powder).
If abdominal organs protrude from the wound, some patients die while others survive; outcomes depend on specific conditions. If the protruding tissue is hand-sized, with a hard mass below, accompanied by irritability, fullness, and shortness of breath occurring intermittently, death will occur within three days. If the protruding tissue does not remain stuck, the patient is calm without irritability, breathing is normal as before, and only the wound hurts, tie off the blood vessels supplying the protruding part with raw silk thread. Leave it tied overnight, then amputate it. Do not close the wound completely; use ointment to facilitate drainage.
