Baozhuan (Bladder Displacement) is caused by the compression and twisting of the bladder, resulting in the inability to urinate; this condition is named Baozhuan. Its clinical manifestations include: urgent pain below the umbilicus and urinary retention. This disease may occur due to forcibly holding urine when one should void, or due to being compelled by cold-heat pathogens. Both scenarios cause water qi to press back against the bladder, preventing it from filling and expanding normally. External fluids cannot enter, and internal urine cannot exit, leading to mutual obstruction inside and outside, thus causing blockage. If this condition persists for four to five days, it may lead to death. Eating until full and then forcibly holding urine, or running on horseback immediately after a full meal, or walking rapidly when the urge to urinate is strong, or engaging in sexual intercourse while holding urine, can all cause Baozhuan or prolapse of the bladder, potentially resulting in death.
Nan Da Bian (Difficulty in Defecation) arises from the disharmony of the Five Zang Organs, where Yin and Yang are biased towards excess or deficiency, specifically indicating that the San Jiao (Triple Burner) is not harmonious, causing cold and heat to knot together.
The Stomach is the sea of water and grain; the essence of water and grain transforms into Ying Qi (Nutritive Qi) and Wei Qi (Defensive Qi), while the dregs are transported through the Large Intestine for excretion. Since the Five Zang Organs and San Jiao are not harmonized, cold and heat stagnate and obstruct, accumulating between the stomach and intestines. The stomach and intestines are inherently solid, and when combined with the invasion of cold and heat qi, the accumulation cannot be dispersed, thus causing difficulty in defecation.
It is also stated: Pathogens in the Kidney can also cause difficulty in defecation. This is because when the Kidney is affected by pathogens, its deficiency fails to control urination, leading to frequent urination. Consequently, body fluids become dry and depleted, the stomach and intestines become dry and astringent, resulting in difficult defecation.
Furthermore, individuals suffering from Ke Li (Wasting-Thirst Syndrome/Diabetes Mellitus) also experience difficulty in defecation. This is because body fluids are exhausted, causing dryness in the stomach and intestines.
Palpating the pulse at the Cun Kou (Inch Port) before the Ren Ying (Human Welcome) point on the left hand corresponds to the Hand-Shao-Yin Heart Meridian. A deep pulse indicates Yin; if Yin is Excess, the patient suffers from constipation, difficult defecation, abdominal distension, heaviness of the four limbs, fever, and gastric distension discomfort. If the pulse at Guan (Carpal) on the right hand is Yin Excess, it indicates Spleen Excess, characterized by a sensation of hardness like a prison within the intestines and difficult defecation. A tight and slippery pulse, with a slightly wiry Fu Yang (Foot-Yang) pulse, typically suggests abdominal distension; if there is no distension, there must be difficult defecation and foot pain, which is the downward transmission of deficiency-cold from above. Methods involving decoctions, moxibustion/ironing, acupuncture, and stone therapy have separate specific formulas. Nourishing and guiding exercises are attached hereafter.
The "Yang Sheng Fang · Dao Yin Fa" (Prescriptions for Health Preservation · Guiding and Pulling Method) states: Lie supine, extend both hands straight, and knead the left and right hypochondrium. This eliminates difficulty in defecation, abdominal pain, and cold in the abdomen. Inhale through the mouth, exhale through the nose, swallow the warm qi dozens of times, and the illness will heal.
