Carbuncles and sores are all caused by heat-toxin; distinguishing their Yin and Yang is correct. But why is it necessary to further distinguish between deficiency (Xu) and excess (Shi)? One must not fail to realize that within both Yin and Yang, there are respectively deficiency and excess conditions. If these are not clearly distinguished, whether using tonification or purgation methods will be in vain. Therefore, it is essential to discern deficiency within Yin and excess within Yin, as well as deficiency within Yang and excess within Yang; this is the subtle approach.
What constitutes deficiency within Yin? It is when the sore mouth is flat and not elevated, and the blood color becomes dark and blackish. This is deficiency within Yin. What constitutes excess within Yin? It is when the sore mouth is initially flat but later becomes solid, with ruddy red blood color. This is excess within Yin. Although Yin syndromes generally belong to deficiency categories, the application of tonification requires differentiation of severity. I now establish a formula that can treat such cases: if dark blackish blood color is observed, this indicates extreme deficiency and extreme cold. Add one qian of Fuzi (Aconiti Lateralis Radix Preparata) and three qian of Rougui (Cinnamomi Cortex) to the formula. After taking several doses continuously, the black color will inevitably turn red. Then remove Fuzi and continue taking the remaining prescription; naturally, flesh will grow over the sore, leading to recovery. If the sore color is red from the beginning, there is no need to add Fuzi and Rougui. Taking the original prescription continuously will lead to complete recovery. However, if one does not understand the deficiency and excess of Yin syndromes and indiscriminately uses Fuzi and Rougui, it will only lead to failure.
What constitutes deficiency within Yang? It is when the sore mouth is high and elevated, but suddenly changes color and loses its redness. This is a sign that a Yang syndrome is transforming into a Yin syndrome. Urgently use Sanliang (3 liang) of Jinyinhua (Lonicerae Japonicae Flos), Yiliang (1 liang) of Guishen (Angelicae Sinensis Radix), one pian (slice, weighing two fen) of Fuzi (Aconiti Lateralis Radix Preparata), and Sanqian (3 qian) of Sheng Gancao (Glycyrrhizae Radix et Rhizoma). Decoct and drink the soup; the color will immediately turn red. This formula is named Zhuan Yang Hua Du Dan (Yang-Transforming Toxin-Dissolving Pill). This condition arises because the patient was originally not very robust, or due to excessive sexual activity or anger, causing an sudden transformation of symptoms. Delay is not permissible. Upon observing the color change, use this formula immediately to turn danger into safety.
What constitutes excess within Yang? It is when the sore mouth is highly protruding and towering, with bright red and lustrous color. This is excess within Yang. Use the formula Xie Yang Qu Du Dan (Yang-Purging Toxin-Expelling Pill). This formula is best for treating toxins in Yang syndromes. One dose expels the toxin; two doses clear the toxin completely; three doses result in full recovery. If the sore is protruding at the initial stage, one dose causes immediate subsidence. It is truly a miraculous formula! This knowledge is also indispensable.
