A person who suddenly vomits blood violently has invariably a root in fire. However, once the blood pours out like an overturned basin, the excess fire must transform into deficiency fire. Excess fire can be purged, but deficiency fire absolutely cannot be purged. Moreover, since blood has been lost and the body lacks blood nourishment, using fire-purging herbs would severely damage stomach qi. Not only can blood not regenerate rapidly, but qi also cannot recover immediately; often, this leads to death from qi collapse. The treatment method should not merely stop bleeding, but should invigorate blood; even more importantly, it should urgently consolidate qi. When qi is consolidated, lost blood can gradually regenerate, and unlost blood can become vigorous again. Use Guqi Shengxue Tang (Consolidate Qi and Generate Blood Decoction):
Renshen (Ginseng Radix) (30g) Danggui (Angelicae Sinensis Radix) (15g) Jingjie (Schizonepetae Herba, stir-fried until black) (6g) Decoct in water and take orally. After one dose, bleeding stops; after two doses, qi becomes vigorous; after four doses, blood returns to its meridians, preventing further vomiting.
This formula is a variation of Buxue Tang (Tonify Blood Decoction). Its efficacy lies entirely in Jingjie guiding blood into the qi aspect and guiding qi to generate within the blood aspect. Once the yin and yang of qi and blood interact, the yin and yang of water and fire naturally assist each other, ensuring that zang-fu organs and collaterals do not experience reversal, which would cause blood to surge and induce vomiting again. Generally, tangible blood cannot be generated quickly, whereas intangible qi is suitable for urgent consolidation. Treating hematemesis by regulating qi rather than blood has been discussed by predecessors, so I need not elaborate further. This formula is suitable for initial cases of violent hematemesis; if hematemesis has persisted for a long time, modification is necessary.
For this syndrome, Huangjing Tang (Rehmannia and Schizonepeta Decoction) is also miraculous.
Shengdi (Rehmanniae Radix) (120g) Chaohei Jingjie (Stir-fried black Schizonepetae Herba) (9g) Decoct and take orally to stop bleeding.
A person with chronic hematemesis that does not cease, or vomits every half month, every month, several times every three months, or frequently throughout the year, although without coughing, yet continues to expectorate phlegm and is extremely fatigued, this is hematemesis originating from the Kidney and Liver. Hematemesis is not necessarily always a disease of the Kidney and Liver, but frequent hematemesis over time inevitably damages the Kidney and Liver. Once damaged, Water fails to nourish Liver Wood, causing Liver Wood to dry up. The Dragon-Lei Fire (Hyperactive Fire) cannot remain within the Wood and attacks the Spleen and Stomach downwards, causing Cold-Deficiency in the Spleen and Stomach. Consequently, Dragon-Lei Fire reverses and rushes upwards, bullying the weakness of Lung Metal, carrying blood from the Stomach, leading to boiling due to excessive fire, and exiting through the mouth. The treatment must tonify the Kidney, Liver, and Lung meridians simultaneously. Use Santai Jiming Tang (Three-Terrace Life-Saving Decoction):
Shudi (Prepared Rehmanniae Radix) (240g) Maidong (Ophiopogonis Radix) (90g) Danpi (Moutan Cortex) (60g) Decoct in water to yield two bowls, consume within one day to stop vomiting blood.
Shudi tonifies the Kidney to nourish the Liver; Maidong clears the Lung to restrain the Liver; Danpi removes floating fire from the Liver and guides upper Jiao fire down to the Kidney, returning blood to its meridians. However, without heavy dosage, the fire spreads like a prairie fire, making it impossible to suppress its blazing intensity. Therefore, heavy dosage is required, like torrential rain extinguishing widespread heat. Once the fire subsides and blood calms, regulating with Dihuang Wan (Rehmannia Pill) for three years is the best strategy for longevity; I urge people to adhere to this regimen as a life-extending elixir.
For this syndrome, Tianjing Zhixue Tang (Fill Essence and Stop Bleeding Decoction) is excellent.
Shudi (Prepared Rehmanniae Radix) (60g) Shanzhuyu (Corni Fructus) (12g) Maidong (Ophiopogonis Radix) (15g) Bei Wuweizi (Schisandrae Fructus, Northern) (3g) Chaohei Jingjie (Stir-fried black Schizonepetae Herba) (9g) Baishao (Paeoniae Radix Alba) (30g)
Decoct in water and take orally. After ten doses, bleeding ceases.
A person vomiting black blood, though not profuse, suffers severe phlegm-cough and thirst with desire to drink. This is Excess Fire of the Kidney Meridian. The fire within the Kidney rises together with the Ministerial Fire of the Pericardium. While Pericardium fire can be purged, Kidney fire ultimately cannot; purging Pericardium fire harms the Kidney. Thus, I purge the Liver, so that both Pericardium and Kidney fires are purged indirectly. The Liver is the Mother of the Pericardium and the Child of the Kidney; if the Mother is weak, the Child cannot be strong, and if the Child is deficient, the Mother becomes weak. Use Liangxie Tang (Double-Purging Decoction):
Baishao (Paeoniae Radix Alba) (30g) Danpi (Moutan Cortex) (30g) Digupi (Lycii Cortex) (30g) Chaohei Zhizi (Stir-fried black Gardeniae Fructus) (9g) Xuanshen (Scrophulariae Radix) (30g) Decoct in water and take orally. After two doses, black blood turns red; after another two doses, cough ceases and bleeding stops spontaneously—miraculous effect.
Black is the color of the North; black blood belongs to the Kidney but also involves Heart Fire, similar to extreme heat turning into charcoal when thrown into water. When Pericardium fire enters the Kidney, extreme fire resembles water, leaving no doubt. Although Liangxie Tang purges Liver Wood, it effectively purges both Pericardium and Kidney meridians. Fire resolves with Water; blood transforms with Cold. This explains why black blood changes easily and hematemesis stops readily.
For this syndrome, Sanxian Sanhuo Tang (Three-Immortal Fire-Scattering Decoction) is also usable:
Xuanshen (Scrophulariae Radix) (90g) Shengdi (Raw Rehmanniae Radix) (60g) Baishao (Paeoniae Radix Alba) (30g) Decoct in water and take orally. Stops bleeding after two doses.
A person affected by Summerheat experiences sudden stagnation of Qi, vomiting blood clots violently without cessation. This is Summerheat pathogen invading the Stomach. Symptoms include headache as if splitting, sweating like rain, extreme thirst, maniacal shouting. If treated as Deficiency, symptoms worsen; Danggui Buxue Tang (Danggui Tonify Blood Decoction) should not be used lightly. The method is to dissipate Summerheat Qi and assist with herbs that descend Qi and return blood to meridians, so Qi does not reverse and bleeding stops.
Use:
Qinghao (Artemisiae Annuae Herba) (30g) Danggui (Angelicae Sinensis Radix) (15g) Chaohei Jingjie (Stir-fried black Schizonepetae Herba) (9g) Shigao (Gypsum Fibrosum) (30g) Maidong (Ophiopogonis Radix) (15g) Xuanshen (Scrophulariae Radix) (15g)
Dahuang (Rhei Radix et Rhizoma) (3g) Decoct in water and take orally. After one dose, Summerheat dissipates and thirst stops; after two doses, blood returns to meridians and all symptoms resolve. Do not use beyond two doses.
This formula is named Jieshu Zhixue Tang (Resolve Summerheat and Stop Bleeding Decoction). Qinghao clears Summerheat and retreats Yin Fire, achieving Yin-Yang balance and removing stagnant Qi. Shigao clears Stomach Fire; Maidong clears Lung Fire; Xuanshen clears Kidney Fire; Jingjie guides fire downward from Upper Jiao; Dahuang eliminates stasis preventing retention in the Stomach; Danggui’s slipperiness aids rapid movement, fearing constipation from upward blood flow. Thus, the mechanism rotates smoothly, yielding swift results.
For this syndrome, Sanshu Zhixue Tang (Disperse Summerheat and Stop Bleeding Decoction) is also miraculous:
Dahuang (Rhei Radix et Rhizoma) Shengdi (Raw Rehmanniae Radix) Shigao (Gypsum Fibrosum) (9g each)
Decoct and take orally.
A person expects blood threads in phlegm, less during the day, more at night, with incessant coughing and insomnia. This is Kidney Fire rushing into the throat, unable to descend to the Mingmen (Gate of Life), causing fire to boil phlegm upward. Heart Fire bullies weak Lung Metal, creating a mix of Water and Fire, resulting in blood threads in phlegm. Use Huasi Tang (Transform Threads Decoction):
Shudi (Prepared Rehmanniae Radix) (30g) Maidong (Ophiopogonis Radix) (15g) Beimu (Fritillariae Bulbus) (3g) Xuanshen (Scrophulariae Radix) (15g) Fuling (Poria) (9g) Suzi (Perillae Fructus) (3g) Digupi (Lycii Cortex) (9g)
Shashen (Glehniae Radix) (9g) Chaohei Jingjie (Stir-fried black Schizonepetae Herba) (3g) Decoct in water and take orally. After one dose, blood threads decrease; after two doses, they cease.
This formula treats Lung, Kidney, and Heart meridians simultaneously, combined with phlegm-resolving and fire-clearing agents, eliminating pathology invisibly, hence its speed. Without tonics, merely clearing phlegm and fire may increase phlegm and blood coagulation. After recovery, discontinue this formula and use Yiyin Dihuang Wan (Nourish Yin Rehmannia Pill). Formula:
Shudi (Prepared Rehmanniae Radix) (500g) Shanyao (Dioscoreae Rhizoma) (400g) Maidong (Ophiopogonis Radix) (500g) Bei Wuwei (Schisandrae Fructus) (150g) Shanzhuyu (Corni Fructus) (400g) Danpi (Moutan Cortex) (300g) Fuling (Poria) (300g)
Digupi (Lycii Cortex) (500g) Zexie (Alismatis Rhizoma) (200g) Honey for pills. Take for one year to prevent recurrence permanently.
For this syndrome, Huanyuan Tang (Return to Source Decoction) is also good:
Shudi (Prepared Rehmanniae Radix) (30g) Shanzhuyu (Corni Fructus) (15g) Chaohei Jingjie (Stir-fried black Schizonepetae Herba) (9g) Digupi (Lycii Cortex) (15g) Maidong (Ophiopogonis Radix) (9g) Tiandong (Asparagi Radix) (6g)
Gancao (Licorice Root) Beimu (Fritillariae Bulbus) (1g each) Jiegeng (Platycodonis Radix) (1.5g) Decoct in water and take orally. Cured after thirty doses.
A person with chronic hematemesis fails all attempts to stop it because blood enters turbid pathways. If Fire is not excessive and Qi does not reverse, blood is not vomited. Know that Qi reversal stems from Excess Fire; to treat Qi reversal, one must descend Fire. However, prolonged Excess Fire diminishes, and prolonged Qi reversal intensifies. It seems easier to purge Fire than descend Qi, but purging Fire causes Qi to descend with it. Long-standing Fire becomes Deficiency Fire, which requires guiding; however, guiding-fire herbs are often acrid-warm, risking aiding reversal. Better to strengthen Water to suppress Yang Fire. Use Zhuangshui Tang (Strengthen Water Decoction):
Shudi (Prepared Rehmanniae Radix) (60g) Shengdi (Raw Rehmanniae Radix) (30g) Chaohei Jingjie (Stir-fried black Schizonepetae Herba) (6g) Sanqi Gen Mo (Notoginseng Root Powder) (9g) Decoct in water, mix powder, and take orally. Stops bleeding after one dose; ceases after two doses, never recurring.
Using Shudi and Shengdi together contains the subtlety of stopping bleeding while tonifying essence. Jingjie guides blood to meridians; Sanqi cuts off the pathway, preventing re-emergence. Fire消 (dissipates) with Water; Qi descends with Water. This holds profound truth.
For this syndrome, using only 9g Sanqi Gen Mo mixed with one bowl of Tongbian (Child Urine) and taken orally stops bleeding immediately.
A person vomits blood due to great anger, with purple blood and reversed Qi, distension and pain in both hypochondria. Anger injures Blood, preventing storage and causing vomiting. The Liver stores Blood; anger causes Liver lobes to open, losing storage capacity. Liver Qi is inherently urgent; anger makes it more urgent, making blood hard to retain, thus gushing out, sometimes profusely. The Liver contains Dragon-Lei Fire; anger agitates this fire, burning wood and forest, causing blood to rush upward. With blood surging, the Liver lacks nourishment, causing hypochondriac pain and distension. Treatment urgently soothes Liver Qi with cooling herbs. Once anger subsides, Dragon-Lei Fire recedes, and blood disorders heal. Using only hemostatics contradicts the nature of Heat-Fire. Use Pinggan Zhixue San (Soothe Liver and Stop Bleeding Powder):
Baishao (Paeoniae Radix Alba) (60g) Danggui (Angelicae Sinensis Radix) (30g) Chaohei Jingjie (Stir-fried black Schizonepetae Herba) (9g) Chao Zhizi (Stir-fried Gardeniae Fructus) (6g) Gancao (Licorice Root) (3g) Danpi (Moutan Cortex) (6g)
Decoct in water and take orally.
After one dose, Liver Qi soothes; after two doses, hematemesis stops; after three doses, Qi does not reverse, and distension/pain vanish.
Shaoyao soothes the Liver and benefits Liver Qi/Blood. Combined with Danggui, it generates and invigorates blood miraculously. Danpi and Zhizi slightly cool blood to clear fire, facilitating Jingjie’s meridian guidance and Gancao’s spasm-relief.
For this syndrome, Duanhong Yin (Stop Red Drink) is also miraculous:
Baishao (Paeoniae Radix Alba) Danggui (Angelicae Sinensis Radix) (30g each) Chaohei Jingjie (Stir-fried black Schizonepetae Herba) (9g) Sanqi Gen Mo (Notoginseng Root Powder) (9g) Decoct in water, mix powder, and take orally. Stops bleeding after one dose.
A person with hemoptysis does not bleed suddenly but first coughs incessantly, feeling stuck Qi below the throat, finding relief only after expelling blood. People think this is Lung Qi reversal, but it is actually Kidney Qi reversal. Kidney Qi here refers to Deficiency Fire in the Kidney. Excessive Deficiency Fire arises from True Water decline, failing to control Fire, causing Fire to reverse upward and bleed heavily. Why cough first? Because Lung Qi obstructs it. The Lung is the Mother of the Kidney; Kidney Water is the obedient child, Kidney Fire is the arrogant child. The Lung generates Kidney Water but not Kidney Fire, detesting the arrogance. The arrogant child, seeing the Mother favor Kidney Water, attacks and steals Lung Metal blood. The Lung refuses to give readily, causing tug-of-war and hemoptysis. Use Liuwei Dihuang Tang (Six-Ingredient Rehmannia Pill):
Shudi (Prepared Rehmanniae Radix) (30g) Shanzhuyu (Corni Fructus) (9g) Shanyao (Dioscoreae Rhizoma) (9g) Maidong (Ophiopogonis Radix) (30g) Wuweizi (Schisandrae Fructus) (3g)
Fuling (Poria) Zexie (Alismatis Rhizoma) Danpi (Moutan Cortex) (6g each)
Decoct in water and take orally. After four doses, hemoptysis stops; after one month, full recovery.
Liuwei Tang greatly tonifies Kidney Water; Maidong and Wuweizi greatly benefit Lung Metal, sufficient to control Excess Fire, preventing recurrent hemoptysis. This shows treating Water does not require purging Fire.
For this syndrome, Shengshu Er Di Tang (Raw and Prepared Two Earths Decoction) is also wonderful:
Shengdi (Raw Rehmanniae Radix) Shudi (Prepared Rehmanniae Radix) (60g each) Decoct in water and take orally. Cured after ten doses.
A person with hemoptysis bleeds due to coughing. This syndrome often results from labor injury depleting Kidney Water. Water fails to distribute to other Zang organs, and imprudence in sexual activity further dries Water. Dry Water forces Lung Metal to seek generation, draining Lung Qi, while Kidney Water is daily depleted. The poor child steals from the mother, causing maternal pain. Seeking help from the Stomach, but the Stomach is bullied by Liver Fire, so the Stomach dares not generate Lung Metal. Liver Wood generates Fire, making Heart Fire prosperous. Prosperous Heart Fire attacks the Lung. Bullied Lung calls the Child for aid, but Kidney Child Water is weak, unable to control Fire. Fire bullies useless Water, bullying Lung severely. Lung tries to flee to the Child’s home, but the Kidney home is barren and dry, offering no shelter. Forced to return to its own palace, coughing and hemoptysis occur. Treatment should save the Lung, but merely saving the Lung leaves Kidney dry, so Lung fluid still attends to the Kidney, injuring the Lung. Thus, treating the Lung requires tonifying the Kidney. When Kidney Water is sufficient, Liver Wood is calm, Heart Fire rests, and the Lung is peaceful without direct treatment. Use Jiuhuo Tang (Save Desiccation Decoction):
Maidong (Ophiopogonis Radix) (60g) Shudi (Prepared Rehmanniae Radix) (60g) Digupi (Lycii Cortex) (30g) Danpi (Moutan Cortex) (30g) Baijiezi (Brassicae Semen Album) (9g)
Decoct in water and take orally. After one dose, cough lightens; after two doses, wheezing lightens; after ten doses, cough ceases and bleeding heals.
Maidong and Shudi together treat Lung and Kidney. Digupi and Danpi have subtle meaning: Hemoptysis damages Yin; Yin deficiency causes Fire prosperity. This Fire is Yin Fire, not Yang Fire. Digupi and Danpi clear internal heat in bone marrow, relieving Kidney simmering, so it doesn’t demand from Lung Metal. Lung becomes moist, allowing Clear Turbid Qi to descend to Kidney. Mother and Child are at peace; Kidney Water moistens, nourishing Liver Wood and controlling Heart Fire. External evils don’t invade, family harmony prevails, avoiding depletion. Baijiezi simply resolves membrane/phlegm phlegm; no deep meaning, just taking advantage of its non-depletion of True Yin for Yin-deficiency cough with phlegm.
For this syndrome, Maidong Shudi Tang (Maidong and Shudi Decoction) is also good:
Shudi (Prepared Rehmanniae Radix) (60g) Maidong (Ophiopogonis Radix) (30g) Decoct in water and take orally. Full recovery after ten doses.
A person with epistaxis (nosebleed) lasting months or years, intermittently healing or not, is lighter than oral hematemesis, but ignoring it or treating incorrectly can be fatal. Hematemesis affects the Stomach (Turbid Pathway); Epistaxis affects the Lung (Clear Pathway). Affecting the Turbid Pathway reverses all Five Zang; affecting the Clear Pathway reverses only the Lung Meridian. Qi reversal causes diverse complications, hence both can be fatal. Treatment should regulate Lung Qi reversal, caused by Lung Meridian Fire. Lung belongs to Metal, originally having no Fire; Lung Meridian Fire is actually Kidney Water Fire. Due to Heart Fire invasion, Kidney Water saves the Mother (Lung) and dries up, sending Kidney Fire to assist, leading to Fire vs. Fire battle, causing blood to run wild and exit from the nose. Thus, regulating Qi relies on regulating the Kidney, which means tonifying Water to control Fire. Use Zini Tang (Stop Epistaxis Decoction):
Shengdi (Raw Rehmanniae Radix) (30g) Maidong (Ophiopogonis Radix) (90g) Xuanshen (Scrophulariae Radix) (60g) Decoct in water and take orally. Stops after one dose.
Maidong directly treats Lung Metal deficiency; Shengdi and Xuanshen resolve suppressed Fire in the Kidney. As Fire retreats, Qi smooths, and blood returns to meridians. If afraid of heavy dosage and reducing it, blazing Fire won’t be controlled, and efficacy will fail.
For this syndrome, Maidong Sanqi Tang (Maidong and Notoginseng Decoction) is also miraculous:
Maidong (Ophiopogonis Radix) (90g) Sanqi Gen Mo (Notoginseng Root Powder) (9g) Decoct in water, mix powder, and take orally. Stops after two doses.
A person with bleeding from the ears, dripping continuously, dies if it lasts three days without stopping. This disease is rare but real. The Ear is the Orifice of the Kidney; ear bleeding is naturally a Kidney Deficiency disease. However, in Kidney Deficiency, blood doesn’t go to the Stomach or exit the mouth, but exits the ear, for a specific reason. Pericardium Fire guides it. Pericardium Fire connects with Mingmen Fire; boiling of both Fires causes blood to bypass the Stomach and go to the ear. The Stomach is the Child of the Pericardium; fearing Kidney Fire harming the Heart and Stomach, it guides Fire upward to the ear. Along all meridians passing through, soil rolls up, so blood follows. Though ear orifices are small compared to the Stomach opening, continuous dripping isn’t sustainable. Urgent stopping is needed. Use Tinqiao Zhifen Tang (Fill Orifice and Stop Smoke Decoction):
Maidong (Ophiopogonis Radix) (30g) Shudi (Prepared Rehmanniae Radix) (60g) Changpu (Acori Tatarinowii Rhizoma) (3g) Decoct in water and take orally. Effective like an echo after one dose.
Shudi fills Kidney Meridian Water; Maidong extinguishes Pericardium Flame. With Fire of both meridians extinguished, if the ear orifice remains open, there’s a hole to drill through; temporary stopping might not prevent re-bleeding. Thus, Changpu guides the two herbs directly into the ear, guiding Ear Fire back to the Pericardium. Fire returns, ear orifice closes. Such divine medication is truly inconceivable.
For this syndrome, Jieliu Tang (Intercept Flow Decoction) is also miraculous:
Shudi (Prepared Rehmanniae Radix) (60g) Shengdi (Raw Rehmanniae Radix) Maidong (Ophiopogonis Radix) (30g each) Sanqi Gen Mo (Notoginseng Root Powder) (9g) Changpu (Acori Tatarinowii Rhizoma) (3g) Decoct in water and take orally. Stops bleeding after one dose.
A person with unceasing bleeding from the tongue has a red, rotten tongue, with red traces appearing in cracks, blood flowing from these traces. Though not instantly fatal, daily worsening leads to incurable states. This syndrome is excessive Heart Fire without Kidney Water assisting. The Heart must be nourished by Water; Evil Water invading the Heart causes death; True Water nourishing the Heart causes life. Thus, Heart and Kidney seem antagonistic but are actually mutually generating. Now, Kidney Water doesn’t communicate with the Heart, seeking Kidney nourishment is impossible, so it borrows from Lianquan (Spring under the tongue), constantly drawing fluids, causing the tongue to deteriorate and dry up. Lianquan water can irrigate Five Zang, but when Kidney Water is sufficient, Lianquan water is sufficient; when Kidney Water dries, Lianquan water dries, like rivers full making wells full. Now, Kidney Water doesn’t assist the Heart, how can it rise above the Heart to lips/mouth? Lianquan wants to nourish the square-inch tongue but cannot, let alone relieve Heart Heat. Thus, spring veins break and wells crack, failing to assist the Heart and rotting the tongue. Once the tongue rots, clear spring mud flows red water becoming blood. Treatment must greatly tonify Heart and Kidney, enabling mutual assistance, so tongue bleeding stops spontaneously. Use Hushedan (Protect Tongue Elixir):
