The treatment focuses primarily on nourishing the Lung (Fei), nourishing Blood (Xue), tonifying Qi, and clearing Metal (Jin).
Tongsheng San (Universal Rescue Powder) mixed with slightly stir-fried earthworm mud and honey applied externally is excellent.
If the nodules arise from the abdomen, it indicates internal heat manifesting externally; Tongsheng San may still be administered internally.
One must first administer tonic medicines to generate Qi and Blood, primarily using Ginseng Radix et Rhizoma (Renshen), Atractylodis Macrocephalae Rhizoma (Baizhu), Angelica Sinensis Radix (Danggui), and Chuanxiong Rhizoma (Chuanxiong), in large doses. Externally, use powder of Aconiti Lateralis Radix Praeparata (Fuzi) mixed with saliva to form cakes as thick as a coin, and moxa cones are used for moxibustion. For larger ulcers/nodules, larger moxa cones are used; for smaller ones, smaller cones. The area should only be warmed slightly, avoiding pain; replace when dry.
If the nodule persists, grind it into powder, re-mix, and continue moxibustion. Stop if the patient feels fatigued. Repeat the procedure the next day until the flesh levels out, which marks efficacy. Alternatively, slices of Aconiti Lateralis Radix Praeparata can be used for moxibustion, while continuing to apply pastes made from the aforementioned Qi- and Blood-nourishing herbs.
Decoct Galla Chinensis (Wubeizi), Natrii Sulfas (Puxiao), Taxillus Sinicus (Sangjisheng), and Nelumbinis Plumula (Lianfang) for fumigation and washing. For swelling, grind Momordicae Semen (Mubiezi) and Galla Chinensis (Wubeizi) into fine powder and apply topically.
Ulcerative conditions (Lou) are treated primarily with cooling herbs.
Formula for Hemorrhoidal Fistula:
Ginseng Radix et Rhizoma (Renshen), Astragali Radix (Huangqi), Angelica Sinensis Radix (Danggui), Chuanxiong Rhizoma (Chuanxiong), Cimicifugae Rhizoma (Shengma), Aurantii Fructus (Zhiqiao), Scutellariae Radix (Tiaohuangqin/Huangqin), and Sophorae Flavescentis Semen (Huaijiao).
Treat as damp-heat and food stagnation. If complicated by wind pathogen, it is difficult to cure.
External packing medicines for fistulas: Calamina (Lu-gan-shi processed with urine) and Ostreae Concha Pulvis (Muli Fen).
Nodules may appear on the neck, nape, body, or arms. If they are swollen toxins, they are often phlegm accumulations forming hard knots that do not dissipate. This treats single nodules behind the ears and at the vertex.
Bombyx Batryticatus (Jiangcan, stir-fried), Indigo Naturalis (Qingdai), Arisaematis Cum Bile (Danning), and Rheum Palmatum Radix et Rhizoma (Dahuang processed with wine).
Grind the above into powder, make pills with honey, and dissolve slowly in the mouth.
For phlegm nodules under the jaw and cheeks, use Erchen Decoction (Two-Cured Decoction) added with stir-fried Rheum Palmatum Radix et Rhizoma (Dahuang), Forsythiae Fructus (Lianqiao), Platycodonis Radix (Jiegeng), and Bupleuri Radix (Chaihu).
To treat painful nodules in the arm:
Forsythiae Fructus (Lianqiao), Saposhnikoviae Radix (Fangfeng), Chuanxiong Rhizoma (Chuanxiong), Scutellariae Radix (Huangqin processed with wine), Atractylodis Rhizoma (Cangzhu), and Gleditsiae Sinensis Spina (Zaocao Ci).
To treat pain at the Huantiao acupoint and prevent the formation of osteomyelitis (Gu-ben Yong):
Use Atractylodis Rhizoma (Cangzhu) to assist the pungency of Phellodendri Cortex (Huangbai), combined with Citri Reticulatae Pericarpium (Qingpi) to move Qi. Add Cinnamomi Ramulus (Guizhi) in winter, and Scutellariae Radix (Tiaohuangqin) in summer. For deficient constitutions, add Achyranthis Bidentatae Radix (Tu Niuxi), and use Glycyrrhizae Radix et Rhizoma (Gancao) as an envoy herb. Decoct in large quantities, add fresh ginger juice, and drink warm before meals. For severe cases, add Huangbai and Guizhi. If after more than ten doses the mass does not move, add one dose of Rheum Palmatum Radix et Rhizoma (Dahuang); if it still does not move, an abscess may be forming. In this emergency, dig a pit in the ground, heat bricks or iron until red, pour urine over them, sit naked on the hot surface, and wrap the lower body with blankets. The rising heat will steam the body, opening the pores and promoting smooth Qi and Blood flow, leading to recovery.
