Sudden syncope accompanied by deafness, and unilateral ear blockage with obstruction, is caused by the sudden upward rush of internal Qi. This is not a disease resulting from wind-stroke affecting the interior or exterior, but rather one where pathogenic factors remain stagnant, leading to emaciation and chronicity. Headache, tinnitus, and dysfunction of the nine orifices (eyes, ears, nose, mouth, etc.) are generated by disorders of the stomach and intestines.
The Yellow Emperor asked: The chapter "Stitching Joints" (Ci Jie) mentions the method of "Releasing the Blindfold" (Fa Meng), which involves needling Fu-organ acupoints to eliminate Fu-organ diseases. How does this work? Qibo replied: To perform this method, it must be done during the day. One should needle the Tinggong (SI19) acupoint in front of the ear (another version says Tinghui GB2), ensuring the needle sensation reaches the eyeball, so that sound can be heard externally. This demonstrates the efficacy of this acupoint. Asked: What is meant by "sound can be heard externally"? Replied: After needling, firmly press both nostrils with hands, forcing the Qi to descend urgently; the resulting resonant sound will necessarily correspond internally. For tinnitus, select the arterial pulse in front of the ear. If ear pain precludes needling, and there is pus in the ear, or hard cerumen (earwax), hearing is lost. For deafness, select the point at the corner of the nail on the little finger (Taisu notes this refers to the ring finger) where the nail meets the flesh (Guanchong SI1), first selecting Hand-Shaoyang points, then Foot-Shaoyang points. For tinnitus, select the area above the nail of the middle finger (Zhongzhu SJ3), treating left-sided symptoms with right-sided points and vice versa, first selecting Hand-Shaoyang points, then Foot-Shaoyang points. For deafness without pain, select Foot-Shaoyang Gallbladder channel points; for deafness with pain, select Hand-Yangming Large Intestine channel points.
For tinnitus, Baihui (GV20), Hanyan (GB6), Luxi (TE18), Tianchuang (SI16), Daling (PC7), Pianli (LI6), Qiangu (LI15), and Houxi (SI3) all govern these conditions. For ear pain, deafness, and tinnitus, Shangguan (ST7) governs, but needling must not be deep. For deafness and tinnitus, Xiaguan (ST7), Yangxi (LI5), Guanchong (SI1), Yimen (TE14 - *Note: Text says Yimen, likely typo for Yifeng or similar, but translating as written*), and Yanggu (SI4) govern. For deafness, tinnitus, and headache/jaw pain, Ermen (TE21) governs. For heavy head, jaw pain, and pulling sensations with buzzing in the ear, Heliao (LI18) governs. For deafness, if there is a rushing water or wind sound in the ear like an epileptic fit, Tinghui (GB2) governs. For deafness feeling full and obstructed as if unable to hear, with buzzing like cicadas, and ringing in the cheek area, Tinggong (SI19) governs. Select points below the jaw, akin to breaking the sound obstruction; needling here (which corresponds to the "Fa Meng" mentioned in the Nine Scrolls/Jiu Juan) is effective. For deafness, Yifeng (TE17), Huizong (TE7), Xiaguan (ST7) govern. For complete deafness, Tianchuang (SI16) govern. For deafness with no audible sound despite buzzing, Tianrong (SI17) govern. For tinnitus with no hearing, Jianzhen (SI9) and Wangu (GB12) govern. For wind sensation in the ear, intermittent tinnitus and deafness where hearing is lost, Shangyang (LI1) govern. For deafness with ear blockage, Hegu (LI4) govern. For deafness with pain in both temporal regions, Zhongzhu (SJ3) govern. For muffled hearing, Waiguan (TE5) govern. For sudden Qi-blockage deafness, Sidu (TE10) govern.
