(Heat Diseases, Yin Syndromes, Yang Syndromes)
For all heat diseases, puncture Xiagu (Xiagu, 陷谷) acupoint. If the feet are initially cold, and the cold sensation ascends to the knees, then withdraw the needle. The body feels numb, with shivering and aversion to cold. Distension and pain in the hypochondriac region. For early-stage heat diseases, puncture the Jing-Well and Ying-Spring acupoints. If heat enters the interior causing shortness of qi and cold reversal, use moxibustion. After the heat subsides, moxa three cones at Yongquan (Yongquan, 涌泉). Irritability and loss of appetite; moxa Yongquan. After heat subsides, if there is cold reversal, wheezing, and shortness of breath: hemiplegia/stroke. Sweating with a clear/cold feeling. All these cases should select Xiaxi (Xiaxi, 侠溪) acupoint.
For all warm diseases, if the body has been hot for five days or more without sweating, puncture Taiyuan (Taiquan, 太泉/Taiyuan, 太渊) acupoint and retain the needle for one shichen (two hours). If less than five days have passed, puncturing is prohibited. For those who exhibit abnormal Taiyuan pulse qualities, loss of appetite, alternating chills and fever, sweating, nausea upon onset which subsides after vomiting, select Gongsun (Gongsun, 公孙) and the Jing-Well and Shu-Stream acupoints. Excess patterns manifest as severe cutting pain in the intestines with cold reversal, swelling of the head and face, irritability, mania, polydipsia, and insomnia. Deficiency patterns manifest as abdominal distension like a drum, fullness, heat, and pain in the abdomen, loss of appetite. Cholera is governed by Gongsun. For heat diseases with irritability, cold and clear feet with excessive sweating, first select Ran Gu (Rangu, 然谷), then select Taixi (Daxi, 大溪/Taixi, 太溪). The artery between the big toe should be tonified first. Warm diseases can be treated by puncturing fifty-nine acupoints. Additionally, there are six hundred and five acupoints on the body; thirty-six are harmful to moxa, and seventy-nine are disastrous to puncture. Physicians in Jiangnan secretly preserve Zhang Zhongjing's essential formulas and do not transmit them. Niyan (Niyao, 颞厌) acupoint: acupuncture and moxibustion treat warm diseases. (see Jaundice). *Zhi Mi Fang* records methods for moxa treating Yin-Du Cold Damage. Its symptoms include lack of restlessness. Take about ten raw green onions, remove roots and coarse skin, roll them upside down in paper into a cylinder two cun wide, not too tight, allowing airflow. Cut with a sharp knife into cakes half a cun high. Place on the center of the navel. Use an iron to heat-moxa. Replace when soft. Not exceeding ten times, the patient will revive. Then administer medicines to support healthy qi. Method for moxa treating Jie-Xiong Cold Damage: Symptoms include chest fullness, shortness of breath, pain upon pressure, vomiting/reversal of qi, chest oppression, or constipation. When all medicines fail, take seven Baidu beans (with skin) and seven cun of thick Huanglian (Coptidis Rhizoma, 黄连), remove whiskers, pound together into one pill. Place on the navel. Press firmly with hand. Moxa a cone larger than a soap bean on top of the medicine. Severe cases resolve in three to five cones. Follow with tonic medicines for one to two days. Even if ill for half a month with faint qi, it can be cured. Treating Qi deficiency and Yang collapse: Body cold, no pulse, breathing nearly ceased, unconsciousness. Also for Cold Damage Yin-cold reversal where hundred medicines fail, use Green Onion Heat-Moxa method. Bind green onions with rope to cup size, cut off roots and leaves, keep only white part about two cun long. Like a large cake. Heat one side with fire until hot, ensure moxa does not burn person. Place hot side on patient's navel extending below. Fill iron with fire and moxa above. Let heat penetrate muscles. Prepare three to four cakes beforehand. Replace when one is damaged. After a while, patient gradually awakens, hands and feet warm, sweating indicates recovery. Then administer Sini Tang (Four Reversal Decoction) type medicines to warm the interior. No worries. My elder brother suffered from Cold Damage, unconscious for eight days. Limbs hard and cold as stone, medicine could not enter. Used this method and recovered. Hu Quanfu, Scholar of Jixian Academy, used this method to save people in crisis, countless times. (From *Liang Fang*) At initial onset, either headache and alternating chills/fever, or shivering desiring warmth/fire, or stiff back and waist. Face looks like drunkenness. This is one to two days of initial Cold Damage. Only need to apply fire moxa to three points below the heart. First point one cun below heart, named Juque (Juque, 巨阙). Second point two cun below heart, named Shangwan (Shangwan, 上脘). Third point three cun below heart, named Weiwuan (Weizhong/Stomach Wan, 胃管/中脘). Moxa fifty cones each. Adults fifty cones, children three cones. Adjust according to age. (From *Qianjin Fang*) If illness lasts three to four days, first moxa twenty cones on upper chest. Measure rope from nose tip to hairline, fold in half. From hairline into hair, moxa at rope end, named Tiancong (Tiancong, 天聪). Also moxa bilateral Niyan (Niyao, 颞厌). Also moxa bilateral Fengchi (Fengchi, 风池). Also moxa Ganshu (Ganshu, 肝俞) one hundred cones. Other areas twenty cones each. Also moxa Taichong (Taichong, 太冲) thirty cones. Divine efficacy.
Generally treating Cold Damage, only Yin syndromes can use moxa; others should be punctured. Thus *Qianjin Fang* only mentions puncturing, and *Suwen* also states for severe disease, fifty-nine punctures. Less than five days, prohibit puncturing. (From *Qianjin Fang*) However, *Qianjin Fang* for headache and alternating chills/fever, moxas Juque, Shangwan, and Zhongwan. Could this also be a Yin syndrome? The symptom described is "shivering desiring warmth." Physicians must distinguish this.
Tongli (Tongli, 通里) acupoint treats heat disease. Sudden mental distress and regret. Frequent yawning and stretching. Sadness and fear. Dizziness, headache. Facial redness and heat. Palpitations. Elbow and arm pain. Excess: limb swelling. Deficiency: inability to speak. Bitter vomiting. Throat obstruction. Shortness of qi, urinary incontinence. (From *Tongren Shuxue Zhenjiu Tujing*) Qimen (Qimen, 期门) acupoint treats women with Cold Damage passing the meridian cycle without resolution. Should puncture Qimen to prevent transmission. Zhongwan (Zhongwan, 中脘) acupoint treats epidemic Cold Damage. (From *Mingtang*) Quze (Quze, 曲泽) acupoint treats Cold Damage with warm-dampness. Body heat, dry mouth. Ge Shu (Geshu, 膈俞) acupoint governs Cold Damage with somnolence, lethargy unwilling to move. Body constantly damp, unable to eat. Initially unhappy for several days. (From *Qianjin Fang*) Yuji (Yuji, 鱼际) and Yanggu (Yanggu, 阳谷) acupoints govern heat disease. Shivering, jaw quivering. Abdominal fullness, impotence. Color unchanged. Jinze (Jinze, 尽泽 - likely Jingqu or variant) acupoint governs Qi obstruction, liking to vomit. Jaw quivering. No sweat. Shenshu (Shenshu, 肾俞) acupoint governs head and body heat/redness. Shivering. Lumbago and limb soreness/weakness. Desire to vomit. Sanjian (Sanjian, 三间) acupoint governs Qi heat. Body heat, wheezing. (*Jia Yi Jing* says: Chills/fever, dry mouth. Body heat, wheezing. Eyes urgent/painful. Easily frightened.) Yuji (Yuji, 鱼际) acupoint treats heat disease with chills, jaw quivering. Abdominal fullness, impotence. Color unchanged. Zhongchong (Zhongchong, 中冲), Shaochong (Shaochong, 少冲), Guanchong (Guanchong, 关冲), Laogong (Laogong, 劳宫), Tailing (Taoling, 太陵), Yangxi (Yangxi, 阳溪), Tianrong (Tianrong, 天容 - inferred from context/missing char) acupoints govern heat disease with irritability. Heart stuffiness, no sweat. Palm heat. Heart pain, body heat like fire. Root of tongue pain. Jianshi (Jianshi, 间使) acupoint governs heat disease with irritability. Likelihood of hiccups. Quze (Quze, 曲泽) acupoint governs Cold Damage and warm diseases. Body heat, irritability, dry mouth. Zhengzheng (Zhengzheng, 支正) and Shaohai (Shaohai, 少海) acupoints govern heat disease starting with lumbar/calf soreness. Likelihood of thirst, frequent drinking. Body heat, neck stiffness/pain. Tianjing (Tianjing, 天井) acupoint governs chills, neck pain. Weizhong (Weizhong, 委中) acupoint governs heat disease with spinal pain. Fengmen (Fengmen, 风门) acupoint treats Cold Damage neck stiffness. Eye closure/darkening. (*Mingtang* has nasal congestion). Frequent sneezing, rhinorrhea, clear nasal discharge. (From *Tongren Shuxue Zhenjiu Tujing*)
Yin-Du sinking and fatigue, medicines difficult to work. Only moxa the navel three hundred cones. Moxa cone size half a date. If hands and feet are not warm, incurable. (From *Benshi Fang*) Or mental confusion, deafness, unresponsive to calls, caused by eating cold things. I saved several people with Lizhong Tang (Regulate Middle Decoction). If thirsty, decoct Wuling San (Five-Ling Powder) to drink, or decoct Renshen Tang (Ginseng Decoction) to drink, both effective. (*Qianjin Fang* says: Cold Damage mostly acquired from wind-cold. Initially wind-cold in the exterior, entering interior does not dissipate. Without warming covering...)
