In patients where the Qi of the vessels is excessive while the Blood is deficient, needling may cause the depletion of Qi. Depletion of Qi leads to collapse and falling down. (If one bitterly drains the Qi, both Yin and Yang Qi will be depleted, thus causing collapse.)
When both Qi and Blood are abundant, and Qi is predominant, the Blood flows smoothly; needling causes the Blood to spurt out. If Yang Qi accumulates and remains stagnant for a long time without being drained, the Blood becomes dark and turbid, and therefore cannot spurt. (Prolonged stagnation of Yang Qi without drainage causes the Yin Blood to wither day by day.)
Freshly ingested fluids seep into the collaterals before harmonizing with the Blood. Consequently, when bleeding occurs, the Blood and fluid separate (Blood and fluid mix in equal parts). In those who have not recently drunk, if there is water retention in the body, it will eventually lead to edema. When Yin Qi accumulates in the Yang aspect and stagnates in the collaterals, upon needling, the Qi moves before the Blood comes out, resulting in swelling. (Yin stagnating in Yang is difficult to dissipate.)
When Yin and Yang Qi have newly interacted but are not yet harmonized (the Qi and Blood are just beginning to regulate), applying draining methods will cause both Yin and Yang Qi to deplete, leading to a separation of the exterior and interior. This results in a pale, ashen complexion (a color indicating decline and danger). If needling causes significant blood loss but the complexion does not change, accompanied by vexation, it is because the collateral was needled but the vessel was inadvertently emptied. Emptying the vessel belonging to Yin leads to Yin depletion, hence the vexation.
When Yin and Yang Qi interact and combine to form Bi syndrome (obstruction), this is due to pathogenic factors overflowing into the vessels and pouring into the collaterals. In such cases, both Yin and Yang are in excess; even if a large amount of blood is released, the condition cannot be considered vacuous.
If heat attaches to the needle, the needle becomes hot, causing the flesh to adhere to it, making it feel firm. (Adhesion means that after insertion, the needle feels tight,涩 [rough/stuck], and difficult to manipulate or remove.)
Excess Yin Qi manifests in dreams of crossing large bodies of water with fear. (Because Yin overcomes Yang, the dreams reflect Yin imagery.) Excess Yang Qi manifests in dreams of great fire burning. (Because Yang overcomes Yin, the dreams reflect Yang imagery.) When both Yin and Yang are excessive, one dreams of killing each other. (Both being strong leads to conflict.) Excess Qi in the upper part manifests in dreams of flying. (Yang predominance associates with the upper.) Excess Qi in the lower part manifests in dreams of falling. (Yin predominance associates with the lower.) Extreme hunger manifests in dreams of taking or seeking food. (Due to deficiency.) Extreme fullness manifests in dreams of giving. (Due to excess.)
Excess Liver Qi manifests in dreams of anger. (The emotion associated with the Liver is anger.) Excess Lung Qi manifests in dreams of fear, crying, or flying. (The emotion associated with the Lung is sorrow/worry, hence fear and crying; the Lung governs Qi, hence flying.) Excess Heart Qi manifests in dreams of joy, laughter, fear, or dread. (The emotion associated with the Heart is joy; its transformation involves worry/sorrow.) Excess Spleen Qi manifests in dreams of singing and music, with a heavy body unable to move. (The sound associated with the Spleen is song; it delights in music and governs the muscles.) Excess Kidney Qi manifests in dreams of the waist and spine feeling detached or broken. (The waist is the external mansion of the Kidney, hence the sensation of disconnection.) These twelve types of excess conditions, when present, can be immediately resolved by draining them. (Yang excess implies surplus in the Fu organs, Yin excess implies surplus in the Zang organs; simply identify the location of the pathogen and drain it with needles.)
Jue Qi (Rebellious Qi) lodging in the Heart manifests in dreams of hills, mountains, and smoke/fire. (Jue Qi in humans is neither purely Yang excess nor purely Yin excess; rather, it arises from the isolation of the Five Zang organs and the scattering of Spirit, leading to delusional dreams. The Heart belongs to Fire, hence dreams of smoke and fire.) Lodging in the Lung, it manifests in dreams of flying and seeing strange objects like gold, iron, or metal. (The Lung belongs to Metal.) Lodging in the Liver, it manifests in dreams of forests and trees. (The Liver belongs to Wood.) Lodging in the Spleen, it manifests in dreams of hills, large marshes, ruined houses, wind, and rain. (The Spleen belongs to Earth and governs dampness.) Lodging in the Kidney, it manifests in dreams of approaching abysses and drowning in water. (The Kidney belongs to Water.) Lodging in the Bladder, it manifests in dreams of traveling. (The Bladder belongs to the exterior of the Three Yang.) Lodging in the Stomach, it manifests in dreams of eating and drinking. (The Stomach is the sea of water and grain.) Lodging in the Large Intestine, it manifests in dreams of fields. (The Large Intestine is the official of transmission, its twists and turns resembling fields.) Lodging in the Small Intestine, it manifests in dreams of crowded cities and thoroughfares. (The Small Intestine is the official of reception and transformation, where things gather, resembling city streets.) Lodging in the Gallbladder, it manifests in dreams of fighting, litigation, and self-mutilation. (The Gallbladder governs decision-making and has a rigid nature.) Lodging in the genitalia, it manifests in dreams of sexual intercourse. (This is where desires focus.) Lodging in the vertex, it manifests in dreams of decapitation. (This is where terror strikes.) Lodging in the shins, it manifests in dreams of walking but being unable to advance, or dwelling in deep underground gardens. (The Jue rebellious pathogen is in the lower part.) Lodging in the thighs and arms, it manifests in dreams of performing rituals and bowing. (This is caused by fatigue and exhaustion.) Lodging in the bladder/intestines, it manifests in dreams of urination and defecation. (Bladder refers to the urinary bladder, intestines refer to the Large Intestine. Anteriorly, it manifests as urine; posteriorly, as stool.) These fifteen types of deficiency conditions, when present, can be immediately resolved by tonifying them. (One should tonify according to the specific meridian involved.)
