前歙州歙县尉杨元操序。
The *Huangdi Bashiyi Nan Jing* (Yellow Emperor's Classic of Eighty-One Difficulties) is a work authored by Qin Yueren of Bo Hai. Yueren received secret medical arts from Master Sang, thereby thoroughly comprehending the Way of Medicine, to the extent that he could see through organs and viscera, dissect intestines, and remove hearts. Because his abilities were similar to those of Bian Que during the time of Xuanyuan (the Yellow Emperor), he was given the title Bian Que. (Note: In the *Zhengyi* commentary on the *Records of the Grand Historian*, "nai" is written as "ren".) He also resided in the State of Lu, and thus was called "Physician Lu." Some people mistakenly regard "Lu" and "Bian" as two different individuals; this is indeed an error.
According to records, the Yellow Emperor possessed the *Neijing* (Inner Canon) in two fascicles, each containing nine chapters. Its principles are profound and subtle, almost impossible to fully explore. Qin Yueren selected its essence, excerpted its core teachings, and compiled eighty-one chapters from these two classics into a structured text to elaborate and expound upon its doctrines. (Note: The original text erroneously uses "shou"; corrected to "zhang" or "section" based on Hua Shou's interpretation.) By exploring the subtle and uncovering the hidden, he transmitted it to future generations, naming it the *Eighty-One Difficulties*. This name reflects the fact that its reasoning and interest are profound and not easily understood at first glance. Having magnified and clarified the words of the Sages, it begins with the reference to the Yellow Emperor. It is indeed the marrow of medical classics and the pivot for curing diseases, comparable to extracting pure material from ivory and rhinoceros horn, or gathering fine feathers from kingfishers.
Later, Lü Guang, the Imperial Physician of Wu, provided annotations for it, which aligned with the fundamental origins and were sufficient to serve as instructive examples. However, his annotations covered less than half of the text, with the remainder missing. I have a natural fondness for medical prescriptions and tireless pursuit of medical knowledge. The structure and sentences of this classic were specially transmitted to me by my master. For ten years, I have been deeply engaged in research without slackening. Although I have not yet reached its ultimate source, I have grasped its general framework. The rise of this teaching has spanned many ages; consequently, not only are there errors in the textual phrasing, but the narrative threads are also disjointed. It is difficult for later readers to comprehend. Therefore, I have systematically organized and arranged the content, grouping similar categories together, dividing it into thirteen sections while retaining the original eighty-one chapters. I have annotated the parts omitted by Lü Guang and expanded upon his incomplete explanations. Additionally, I have separately compiled phonetic and semantic glosses to clarify its purport. In the past, Huangfu Mi synthesized three major works into the *Jiayi Jing* (Systematic Classic of Acupuncture and Moxibustion), and in recent times, Tao Hongjing of Huayang expanded the *Zhouhou Beiji Fang* (Handbook of Prescriptions for Emergencies) into the *Baiyi Fang*. Both acted with deep care and utmost deliberation to benefit and nurture all living beings. My current elaboration aims to emulate high virtue and benevolence from afar and follow great morality close at hand. I only regret that my mediocre understanding is limited, while the Sage’s intent is boundless; like using a short rope to draw water from a deep well, it is inherently difficult to exhaustively capture everything. This is the preface by Yang Yuancan, Vice Magistrate of She County, Xizhou Prefecture.
