If the patient has weak Qi and is unable to take food, Jiahe San (Jiahe Powder) should be administered, with additional Baidoukou Ren (Amomi Fructus Rotundus) added in appropriate dosage. Previously, a nobleman suffered severely from a carbuncle on the back. The physician's medication sequence was disordered, leading to a prolonged illness and resulting in extreme weakness and complete anorexia. I intended to administer Jiahe San, but other physicians argued: "It contains Dingxiang (Caryophylli Flos), which is heating, and thus cannot be used." They failed to realize that Dingxiang is one of the ingredients for treating carbuncles, especially those arising from anger. Moreover, the herbs in Jiahe San are all mild, benefiting the Spleen and Stomach, and descending Qi. After failing to win the debate and delaying for several days, other medications proved ineffective. Finally, Jiahe San was used, and the patient was able to eat. From then on, whenever Ginseng and Poria Decoction (Shenling Baizhu San or similar formulation implied by context, here translated as Ginseng-Poria Soup) was suspended in the morning, a dose of Jiahe San was invariably given until the disease resolved.
Family-inherited Bu Huan Jin Zhengqi San (Bu Huan Jin Rectifying Qi Powder): Treats carbuncles on the back caused by initial contraction of Wind-Cold, Disharmony of Spleen Qi, presenting with alternating chills and fever, and vomiting.
Cangzhu (Atractylodis Rhizoma): Select yellow, unrotten rhizomes. Soak first in rice water for half a day in summer/autumn, or one day in spring/winter. Wash clean, then soak overnight in fresh well water. Scrape off black skin, slice, dry-roast, and stir-fry with wheat bran until yellow and fragrant. Remove bran. Use 4 liang of purified herb.
Houpo (Magnoliae Officinalis Cortex): Select purple bark. Scrape off rough outer bark and the inner dark core resembling paper. Slice into three-cun lengths and cut into thin slices. For every 5 liang of purified Houpo, use 5 liang of purified Shengjiang (Zingiberis Rhizoma Recens). Slice Shengjiang thinly with skin, pound into paste, and soak with Houpo overnight. Roast over alternating Wen Huo (Wen Fire/Low Heat) and Wu Huo (Wu Fire/High Heat) five to seven times until the ginger residue is scorched black. Scrape off ginger; do not use it. Only use finely sliced Houpo, taking 4 liang of purified herb. Then, use another 4 liang of Shengjiang, slice thinly with skin, pound into paste, mix with Houpo, soak overnight again, and stir-fry in a pot over Wen and Wu Huo until dry.
Gancao (Glycyrrhizae Radix et Rhizoma): Roasted, scraped of red skin, finely sliced. Take 2 liang of purified herb.
Chenpi (Citri Reticulatae Pericarpium): Purchase genuine tangerine peel. Soak in water for three shichen (6 hours). Wash away black impurities. Scrape off pulp, white pith, and membranes. Slice and dry-roast. Take 3 liang of purified herb.
The above four ingredients are placed together, stir-fried lightly once over Wen Huo, then wrapped in paper and placed on a wooden board to dissipate fire toxicity.
Banxia (Pinelliae Rhizoma): Soak in boiling water seven times. Scrape off black umbilicus. Finely slice and dry-roast. Grind into fine powder. Mix with natural juice of Shengjiang to form thin cakes. Roast over Wen Huo until yellow and cooked through. Take 2 liang of purified herb.
Huoxiang (Agastaches Herba): Remove stems, use leaves only. Wash with water to remove dust. Dry in sunlight if available, otherwise dry-roast over low heat. Take 1 liang of purified herb.
Renshen (Ginseng Radix): 1 liang. Remove crown, finely slice and dry-roast.
Muxiang (Aucklandiae Radix): 1 liang. Wrap in wet paper and bake/stew until paper is dry. Remove paper, finely slice.
Baifuling (Poria): 1 liang. Remove black skin, slice.
The aforementioned four previously stir-fried ingredients are then combined with the latter five ingredients (Banxia, Huoxiang, etc., which are not stir-fried). All are ground together into a fine powder. For administration, take 1 qian of the powder, add 1 zhan of water, 3 slices of Shengjiang, and 1 jujube. Decoct until reduced to 8 fen. Add a small amount of salt while hot. Take warm at any time.
