There are cases where sores and toxins appear on the left and right sides of the scrotum, known as Bian Du (Perineal Abscess). When they occur below the renal scrotum and in front of the anal canal, they are called Nang Yong (Scrotal Carbuncle). Among these two conditions, Bian Du is easier to treat, while Nang Yong is difficult to cure. This is because the area below the scrotum is called Xuan Yong (Hanging Perineum), and its skin and flesh differ from other parts. In other areas, the skin and flesh grow either horizontally or vertically, making them easy to heal; however, in the Xuan Yong area, the texture is a mix of horizontal and vertical fibers, so once damaged, it does not easily recover. Nevertheless, with proper treatment methods, it is not necessarily difficult. Such conditions often arise because young people indulge in alcohol and sexual desires, frequenting brothels for unrestrained pleasure... leading to such sores. This is what is referred to as "when desire is unfulfilled, essence transforms into pus and blood." The treatment method must focus on greatly tonifying deficiency and assisting with herbs that resolve toxins. Since toxins form due to deficiency, if deficiency is not treated, how can the carbuncle heal? Use Zhu Xie Zhi Shen Dan (Eliminate Evil Supreme Divine Decoction): Jin Yin Hua (Lonicerae Japonicae Flos, 4 liang), Pu Gong Ying (Taraxaci Herba, 2 liang), Dang Gui (Angelicae Sinensis Radix, 2 liang), Da Huang (Rhei Radix et Rhizoma, 5 qian), Ren Shen (Ginseng Radix, 1 liang), Gan Cao (Glycyrrhizae Radix, 1 liang), Tian Hua Fen (Trichosanthis Radix, 2 qian). Decoct in water and take orally. After one dose, the toxin dissipates; after two doses, recovery occurs; for ulcerated cases, three doses suffice for healing. This formula uses large doses of Jin Yin Hua and Pu Gong Ying to assist Dang Gui, Ren Shen, and Da Huang, which might seem overly aggressive. Moreover, using Da Huang to purge in a case of severe deficiency seems inappropriate. However, who knows that the toxic qi is very strong? Taking advantage of the initial stage when righteous qi has not yet declined, employing both great tonification and purging is appropriate. If one delays treatment or fears treating it, waiting until pus and blood flow out and righteous qi becomes depleted before starting to use Ren Shen to tonify qi, it often takes several jin of herbs without full recovery. Why not use it early during toxin resolution, preserving righteous qi while facilitating toxin dispersion? This is the method of guiding according to circumstances, which must be understood.
Secret Formula:
Zhu Xie Shen Dan contains four liang of Silver Flower,
Two Pu Gong Ying, Dang Gui, and five Da Huangs.

Ba Sheng Dan (Eight Sages Decoction) is also effective: Jin Yin Hua (Lonicerae Japonicae Flos, 4 liang), Gui Wei (Caulis Cinnamomi Cum Cortece, 1 liang), Ren Shen (Ginseng Radix, 2 liang), Chai Hu (Bupleuri Radix, 3 qian), Huang Qin (Scutellariae Radix, 3 qian), Huang Bo (Phellodendri Cortex, 3 qian), Bei Mu (Fritillariae Bulbus, 3 qian), Zhi Zi (Gardeniae Fructus, 3 qian). Decoct in water and take orally. Symptoms lighten after one dose, effectiveness appears after two doses. Once toxins have exited, stop taking it.

Some individuals engage in intercourse after drinking alcohol, preventing the release of essence. By midnight, they experience irritability, heat, thirst, dysuria with red urine, and excessive phlegm-saliva. On the first day, the scrotum swells and hurts; by the next day, the flesh of the scrotum rots, and the part of the penis adhering to the scrotum also rots. People attribute this to alcohol toxicity, but who knows it is Liver Fire exacerbated by Damp-Heat from alcohol? How can alcohol dampness cause rotting? Because fire wine is extremely hot. Excessive consumption leads to drunken death, and corpses often decompose due to the toxicity of the fire wine... The treatment method involves resolving alcohol toxicity and tonifying Qi and Blood, thereby dispelling Damp-Heat and promoting the growth of healthy tissue, leading to recovery. Use Jiu Fu Tang (Rescue Rotting Decoction): Ren Shen (Ginseng Radix, 1 liang), Bai Zhu (Atractylodis Macrocephalae Rhizoma, 1 liang), Dang Gui (Angelicae Sinensis Radix, 2 liang), Huang Qi (Astragali Radix, 2 liang), Fu Ling (Poria, 5 qian), Yu Mi (Corni Fructus, 5 qian), Bai Shao (Paeoniae Radix Alba, 1 liang), Huang Bo (Phellodendri Cortex, 2 qian), Ze Xie (Alismatis Rhizoma, 2 qian), Ge Gen (Puerariae Lobatae Radix, 2 qian), Zhi Zi (Gardeniae Fructus, 2 qian). Decoct in water and take orally. After four doses, necrotic tissue falls off and new tissue grows; continue for another four doses until the scrotum and penis completely flatten and recover. Alcohol toxicity forms due to suppression; soothing the Liver, draining Fire, and promoting diuresis to resolve toxins is feasible. Why use Ren Shen, Dang Gui, and Bai Zhu to tonify Qi and Blood? Generally, those with abundant Qi and Blood can withstand alcohol; even heavy drinking causes no harm. Rotting after consuming fire wine and forming toxins occurs because Qi and Blood are weak and cannot overcome the alcohol's toxicity. Thus, two Fires combine, burning the body and rotting flesh. Without urgent tonification of Qi and Blood, alcohol toxicity will not dissipate, and how can necrotic tissue quickly regenerate muscle and flesh?
Secret Formula:
One liang each of Ren Shen and Bai Zhu, two Dang Gui,
Five qian of Fu Ling and Yu Mi combined.
Bai Shao at two liang, taken over two to four doses,
Huang Bo, Ze Xie, Ge Gen, and Zhi Zi, two qian each.
Yi Bao San (Rare Treasure Powder) is also effective: Fu Ling (Poria, 1 liang), Bing Pian (Borneolum Syntheticum, 3 fen), Huang Bo (Phellodendri Cortex, 5 qian), Wa Cao (Herba Siegesbeckiae, 5 qian), Qing Tai (Moss, 5 qian), Er Cha (Catechu, 5 qian), She Xiang (Moschus, 8 fen), Zhang Nao (Camphora, 3 qian), Ru Xiang (Olibanum, 2 qian), Mo Yao (Myrrha, 2 qian). Grind into powder and sprinkle on the affected area. Once fluid discharge stops, further rotting ceases.
Secret Formula:
Another rare treasure sprinkles on scrotal carbuncles,
One liang of Fu Ling, three fen of Bing Pian.
Huang Bo and Wa Cao, five qian each,
Qing Tai and Er Cha are identical in amount.
Eight fen of She Xiang, three qian of Zhang Nao,
Ru Xiang and Mo Yao, two qian each, follow suit.
