Heart.〕 Baizhu (Atractylodis Rhizoma), Gancao (Glycyrrhizae Radix et Rhizoma) [When Wood Qi is excessive, Earth Qi becomes deficient; these are used to support the element that is overcome.] Danpi (Moutan Cortex), Heishanzhi (Gardeniae Fructus nigrococtus) [Used to clear and drain fire. Each used in a dosage of one qian.]
This is a treatment method for draining Liver and calming Spleen. It should be further supplemented with Renshen (Ginseng Radix) and Juhong (Citri Reticulatae Pericarpium Rubrum) to combine with Yigong San (Yi Gong Powder) to tonify Spleen Qi, so that digestion will return to normal. If there are symptoms such as Wind-Heat and Blood Dryness in the Liver Meridian, remove Danggui (Angelicae Sinensis Radix) and Baizhu, add Yin-nourishing herbs, and use Sangzhi (Mori Ramulus) decoction as the vehicle.
Dihuang Yinzi (Rehmannia Decoction for the Kidney) Treats tongue aphasia (inability to speak due to tongue stiffness) and foot febi (paralysis/inability to walk).
Shudi (Rehmanniae Radix Praeparata), Yurou (Corni Fructus), Wuweizi (Schisandrae Fructus) [To strengthen the source of Water.] Bajitian (Morinda Officinalis Radix), Roucongrong (Cistanches Herba), Fuzi (Aconiti Lateralis Radix Praeparata), Guangui (Cinnamomi Cortex) [To benefit the source of Fire.] Yuanzhi (Polygalae Radix) [Tonifies both Water and Fire.] Shichangpu (Acori Tatarinowii Rhizoma) [Its aromatic nature can unblock and open the orifices.] Shihu (Dendrobii Caulis) [Strengthens the Kidney, treats bi-syndrome and weakness of the legs and knees.] Fuling (Poria) [When Kidney Qi rebels, phlegm accumulates like surging tides. Poria descends to subdue the Kidney and drain overflowing phlegm. Moreover, since turbid Yin has rebelled upward, the lower orifices must be blocked. Poria guides the turbid Yin back down through the lower orifices, also utilizing the penetrating momentum of Gui and Fu to drive out turbid Yin and guide it out.] Maidong (Ophiopogonis Radix) [Equal parts.] Bohe (Menthae Haplocalycis Herba) [Half dose. When Yin-Fire flares upward, the chest and diaphragm become a land of raging fire; thus, these two herbs cool the diaphragm and clear the Heart. Additionally, adding Lizhi (Pyri Fructus Juice), Renru (Human Milk), and Zhuli (Bambusae Caulis in Taenia Succus) follows the method of treating heat with cold.] The reference includes Jiang (Zingiberis Rhizoma) and Zao (Jujubae Fructus), plus Renru, Lizhi, and Zhuli.
This formula tonifies the Kidney to benefit the source of Fire. According to the classics: "Internal depletion leading to Jue (reversal/collapse) results in Yin (aphasia) and Fei (paralysis); this is Kidney deficiency." Internal depletion refers to the loss of Essence. Loss of Essence leads to loss of Qi and subsequent Jue. Jue means rebellion/reversal. Qi rebellion causes chaos, hence aphasia above and paralysis below. The root cause is a great deficiency of Primordial Yang, located in the Kidney. This is because the Kidney meridian ascends to flank the base of the tongue and descends to the sole of the foot. Only when there is indeed a decline of Fire within the Kidney should this formula be considered. However, cases of disease caused by Fire deficiency in the world are rare, one or two in a thousand. Therefore, most cases of Yin and Fei belong to deficiency of Water (Yin/Essence) in the Kidney. One should use Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) added with Niuxi (Achyranthis Bidentatae Radix), Duzhong (Eucommiae Cortex), Shihu, Guijia (Testudinis Carapax et Plastrum), Renru, Lizhi, and Zhuli. Harsh acrid-hot herbs are strictly contraindicated. Furthermore, the classics state: "The disease of Fei is characterized by no pain in the body, inability to control the four limbs, slight confusion of mind but not severe. If speech is faint, it is treatable; if speech is lost entirely, it is untreatable." Thus, knowing that Yin and Fei appearing together is originally a dangerous condition.
Shiquan Dabu Tang (Ten-Agent Great Tonifying Decoction) Treats pseudo-stroke (Lei Zhongfeng), where Qi and Blood are both deficient, failing to nourish sinews and bones, resulting in generalized body pain.
Renshen, Huangqi (Astragali Radix), Baizhu, Fuling, Gancao [Using Sijunzi Tang (Four Gentlemen Decoction), adding Huangqi to tonify Qi.] Shudi, Danggui, Baishao (Paeoniae Radix Alba), Chuanxiong (Ligustici Chuanxiong Rhizoma) [Using Siwu Tang (Four Substance Decoction) to tonify Blood. Sijunzi Tang combined with Siwu Tang constitutes Bazhen Tang (Eight Treasure Decoction).] Rougui (Cinnamomi Cortex) [Those with Qi deficiency often have accompanying Cold; Cold is treated with Heat, hence Rougui is used. Moreover, Qi-tonifying herbs gain Wei-Qi benefits from it, and Blood-tonifying herbs gain Ying-Blood regulation from it.] Add Qinjiao (Gentianae Macrophyllae Radix) [Activates collaterals and relaxes sinews.] Xuduan (Dipsaci Radix) [Opens passages and benefits joints.] Duzhong [Eliminates waist and knee pain, benefits whole-body joints.] Hubone (Pantheris Os) [Treats limb spasms and pain.] Lujiaojiao (Colla Cornus Cervi) [Treats limb soreness and pain.]
This formula treats conditions of dual deficiency of Qi and Blood, and dual weakness of Yin and Yang, which is indeed valuable. However, many people suffer from Yin deficiency, and pseudo-stroke is often caused by Yin decline. The Rougui in this formula is acrid and hot, and Chuanxiong is acrid, aromatic, and ascending; both can damage Yin. This formula should only be administered if the condition truly belongs to Deficiency-Cold; otherwise, it is contraindicated. If body pain is due to disharmony of meridians, using herbs to harmonize Qi and Blood will be effective. If limb soreness and pain occur after illness, drinking cow's milk is recommended to tonify Blood, benefit Yin, and clear heat, with rapid efficacy.
Whenever the thumb and index finger are numb or immobile, there is a risk of stroke within three years, and prevention is necessary. Regulate diet, restrain the Seven Emotions (Joy, Anger, Worry, Thought, Sadness, Fear, Fright), and avoid sexual activity; these are of utmost importance. For Kidney Water deficiency, use Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) or Zuogui Wan (Left Restoring Pill); for True Fire deficiency, use Bawei Dihuang Wan (Eight-Ingredient Rehmannia Pill) or Six-Saint Powder (Sisheng San). For Spleen deficiency, use Guipi Tang (Restore the Spleen Decoction) or Liujunzi Tang (Six-Gentlemen Decoction); these provide benefit. Using Wind-dispelling herbs would merely attract Wind and invite Stroke.
In stroke, a slow and floating pulse indicates a favorable prognosis, while a firm, large, rapid, and urgent pulse indicates an unfavorable prognosis.
Mouth opening indicates Heart Qi exhaustion; hand relaxation (open palm) indicates Spleen Qi exhaustion; eye closing indicates Liver Qi exhaustion; urinary incontinence indicates Kidney Qi exhaustion; snoring sound indicates Lung Qi exhaustion. [If these five signs do not all appear, urgently administer a large dose of Dushen Tang (Single Ginseng Decoction); survival is possible. If Niu Huang Wan (Calculus Bovis Pill) or Suhexiang Wan (Styrax Pill) etc. are used, death occurs immediately upon ingestion.] Saliva spitting, fixed gaze, muscle wasting, tendon pain, stiff upright hair, head shaking and jerking upward, ruddy complexion like makeup, sweating like pearls, continuous panting—all indicate untreatable conditions. In any stroke case, whether conscious or unconscious, sudden vomiting of purple-red blood indicates inevitable death subsequently.
