Among the five zang organs, the Heart serves as the sovereign. Due to excessive use of Kidney Qi, Kidney Water is depleted, failing to nourish Fire; consequently, Heart Yang is disturbed, and Ying (Nutritive) Blood fails to remain within. This leads to lower back pain and hemoptysis, followed by the onset of Jing Ju (Well Carbuncle). The external ulceration has persisted for a long time, accompanied by frequent epigastric discomfort. Zheng (Upright) Qi is depleted, and the Middle Jiao is deficient. Treatment should double the efforts to harmonize Ying and Wei (Defensive) Qi, while simultaneously nourishing the Heart.
Prescription: Codonopsis Radix (Dangshen), Poria Cocos (Fuling), Codonopsis Radix Tendril (Renshu), Polygala Tenuifolia Rhizome (Yuanzhi), Ostreae Concha (Muli), Atractylodis Macrocephalae Rhizoma (Tianzhushu), Dioscoreae Hypoglaucae Rhizoma (Huaiyao), Angelicae Sinensis Radix (Guishen), Paeoniae Radix Alba (Baishao), Citri Reticulatae Pericarpium (Chenpi), Biota Semen (Baiziren), Rehmanniae Radix Recens (Shengdi), Carthami Flos (Honghua).
Liver Qi,夹杂着 Phlegm, stagnates, manifesting as Tan Ju (Phlegm Carbuncle) at the Bingu area (sternum/xiphoid process). It has already ulcerated, but one end remains swollen and hard without resolving, and is about to break open again. The pulse at both Chi positions is Wiry and Rapid, indicating that Ying Blood is depleted, and Phlegm-Qi in the Yangming Meridian is not yet cleared. The treatment should nourish Ying and harmonize Wei, while simultaneously resolving Phlegm and softening hardness.
Prescription: Angelicae Sinensis Radix (Danggui), Pinelliae Rhizoma Preparatum (Qingbanxia), Citri Reticulatae Pericarpium (Chenpi), Poria Cocos (Fuling), Trichosanthis Semen (Gualouren), Curcumae Radix (Guangyujin), Fritillariae Thunbergii Bulbus (Dabei), Ostreae Concha (Zuomuli), Imperatae Rhizoma (Sugen), Bergamot Fruit (Foshou); externally apply Haifushi (Magmatic Rock/Scoria).
The swelling and hardness of the Bingu carbuncle have loosened slightly, and the pus is thicker. Continue with the method of harmonizing Ying and dissolving hardness.
Remove Sugen from the previous prescription, add Ze Lan (Lycopus Herba), Lianqiao (Forsythiae Fructus), and Ou Jie (Nymphaeae Nelumbinis Rhizomatis Nodus).
For the Tan Ju at Bingu, where pus has formed and is about to ulcerate, the treatment should clear heat and promote suppuration/drainage.
Prescription: Adenophorae Radix (Nanshashen), Fritillariae Thunbergii Bulbus (Dabei), Citri Reticulatae Pericarpium (Chenpi), Forsythiae Fructus (Lianqiao), Glycyrrhizae Radix et Rhizoma (Gancao), Dendrobii Caulis (Chuanshihu), Angelicae Sinensis Radix (Danggui), Poria Cocos (Fuling), Trichosanthis Semen (Gualouren), Nymphaeae Nelumbinis Rhizomatis Nodus (Ou Jie).
Shi Ju (Stone Carbuncle) is a malignant disease caused by the freezing coagulation of Qi and Blood. It begins as a hard nodule in the left axilla, gradually spreading to the breast, shoulder, arm, neck, and nape. The area is numb and swollen, with pulling pain day and night. With both Qi and Blood severely deficient, it is a difficult-to-treat condition. Considering the patient traveled from afar, a prescription is tentatively provided hoping for a chance of recovery.
Prescription: Codonopsis Radix (Dangshen), Rehmanniae Radix Preparata (Dashudi), Cinnamomi Cortex (Shangrougui), Atractylodis Macrocephalae Rhizoma (Jiaobaizhu), Paeoniae Radix Alba (Dabaishao), Angelicae Sinensis Radix (Quandanggui), Ligustici Chuanxiong Rhizoma (Chuanxiong), Poria Cocos (Fuling), Cyperi Rhizoma (Xiangfu), Glycyrrhizae Radix et Rhizoma (Zhigancao), Mori Cortex (Sangpi), Red Date (Hongzaotou).
Depression injures the Liver, and overthinking injures the Spleen, damaging the Ying of the Liver and Spleen. Qi moves turbulently within, and Wood-Fire carries Phlegm upward. Shaoyang Meridian Qi stagnates in the left neck, manifesting as Shi Ju. The mass is hard as stone, accompanied by soreness and distension in the shoulders and neck, trismus (lockjaw), and throat obstruction. The pulse is fine, and the spirit is emaciated. The disease has sunk deep, making rescue difficult. Considering the patient traveled from afar, a prescription is provided for home conditioning.
Prescription: Codonopsis Radix (Dangshen), Atractylodis Macrocephalae Rhizoma (Dongzhu), Angelicae Sinensis Radix (Danggui), Ligustici Chuanxiong Rhizoma (Chuanxiong), Paeoniae Radix Alba (Baishao), Cyperi Rhizoma (Xiangfu), Fritillariae Thunbergii Bulbus (Dabei), Pinelliae Rhizoma Preparatum (Qingbanxia), Citri Reticulatae Pericarpium (Chenpi), Poria Cocos (Fuling), Glycyrrhizae Radix et Rhizoma (Gancao), Zingiberis Rhizoma Preparatum (Weijiang), Red Date (Hongzao).
Heart and Liver Qi are depressed and unfulfilled, transforming into Fire. Fire rises with Phlegm, causing Shi Ju in the left neck. The mass is hard, swollen, and red, and is likely to ulcerate externally; if it does, healing will be difficult. Tentatively prescribe nourishing Ying and clearing Liver to dissolve hardness.
Prescription: Glehniae Radix (Beishashen), Ligustici Chuanxiong Rhizoma (Chuanxiong), Paeoniae Radix Alba (Baishao), Scrophulariae Radix (Yuanshen), Cyperi Rhizoma (Xiangfu), Pinelliae Rhizoma Preparatum (Qingbanxia), Fritillariae Thunbergii Bulbus (Dabei), Angelicae Sinensis Radix (Danggui), Forsythiae Fructus (Lianqiao), Rehmanniae Radix Recens (Zhongshengdi), Ostreae Concha (Zuomuli), Citrus Reticulata 'Chachi' Pericarpium (Juye).
Liver and Spleen depression causes Qi and Phlegm stagnation, forming Shi Ju with hard swelling. Symptoms include swollen throat, throat obstruction, trismus, neck soreness, and neck distension. Jueyin and Shaoyin are disharmonious, and meridians are blocked. This is a disease caused by severe emotional injury, and regulation/treatment is not easy. The Spleen and Stomach are weak, with loose stools and vomiting after eating. One must guard against Spleen failure. Tentatively prescribe strengthening Earth and harmonizing the Middle, hoping for normal appetite as a sign of luck.
Prescription: Atractylodis Macrocephalae Rhizoma (Jiaobaidongzhu, stir-fried with 2 fen of Immature Bitter Orange/Zhishi), Euphorbiae Humifusa Herba (Peilan), Aucklandiae Radix (Muxiang), Aurantii Fructus Immaturus (Zhiqiao), Amomi Fructus (Sharen), Citri Reticulatae Pericarpium (Chenpi), Codonopsis Radix (Lu Dangshen, stir-fried with Pogostemonis Herba/Huoxiang), Pinelliae Rhizoma (Banxia), Curcumae Radix (Yujin), Hordeum Vulgare Sprout (Guyagen), Glycyrrhizae Radix et Rhizoma (Zhigancao), Poria Cocos (Fuling), Citrus Nobilis Pericarpium (Jinyej).
This is a formula for treating vomiting due to deficiency consumption.
Vomiting has stopped, diet has increased, and Shi Ju swelling has receded somewhat, showing signs of turning around. However, trismus has not loosened, throat obstruction has not relieved, and the pulse is Deep, Wiry, and Choppy, indicating Yin injury, Liver depression, and Phlegm-Qi coagulation and blockage. The upper body is still in a dangerous state, so reliance on this may not be secure. Tentatively continue with the original method.
Prescription: Codonopsis Radix (Dangshen), Atractylodis Macrocephalae Rhizoma (Dongzhu), Ligustici Chuanxiong Rhizoma (Chuanxiong), Angelicae Sinensis Radix (Danggui), Pinelliae Rhizoma (Banxia), Amomi Fructus (Sharen), Citri Reticulatae Pericarpium (Chenpi), Aurantii Fructus Immaturus (Zhiqiao), Euphorbiae Humifusa Herba (Peilan), Curcumae Radix (Guangyujin), Paeoniae Radix Alba (Baishao), Citrus Reticulata 'Chachi' Pericarpium (Juye).
Shi Ju swelling and hardness have slightly loosened. Diseases caused by severe emotional injury are ultimately difficult to dissipate. However, since diet has increased and spirit has slightly recovered recently, tentatively continue with the original prescription.
Prescription: Codonopsis Radix (Dangshen), Angelicae Sinensis Radix (Danggui), Pinelliae Rhizoma Preparatum (Qingbanxia), Euphorbiae Humifusa Herba (Peilan), Atractylodis Macrocephalae Rhizoma (Dongzhu), Paeoniae Radix Alba (Baishao), Citri Reticulatae Pericarpium (Chenpi), Glycyrrhizae Radix et Rhizoma (Zhicao), Ligustici Chuanxiong Rhizoma (Chuanxiong), Poria Cocos (Fuling), Fritillariae Thunbergii Bulbus (Dabei), Old Ginger (Laojiang), Citrus Reticulata 'Chachi' Pericarpium (Juye).
Spirit and diet have doubled recently, and Shi Ju hardness and swelling are also contracting, which is a great fortune. Xiangbei Yangrong Tang should be the main prescription.
Prescription: Codonopsis Radix (Dangshen), Angelicae Sinensis Radix (Danggui), Paeoniae Radix Alba (Baishao), Citri Reticulatae Pericarpium (Chenpi), Atractylodis Macrocephalae Rhizoma (Baizhu), Ligustici Chuanxiong Rhizoma (Chuanxiong), Poria Cocos (Fuling), Pinelliae Rhizoma Preparatum (Qingbanxia), Fritillariae Thunbergii Bulbus (Dabei), Cyperi Rhizoma (Xiangfu), Glycyrrhizae Radix et Rhizoma (Zhigancao), Ostreae Concha (Muli), Red Date (Hongzao), Citrus Reticulata 'Chachi' Pericarpium (Juye).
The illness shows daily improvement; follow the previous method.
Prescription: Rehmanniae Radix Recens (Stir-fried with Polygoni Multiflori Folii Pulvis/Puhuang), Angelicae Sinensis Radix (Danggui), Citri Reticulatae Pericarpium (Chenpi), Fritillariae Thunbergii Bulbus (Dabei), Paeoniae Radix Alba (Baishao), Codonopsis Radix (Lu Dangshen), Ligustici Chuanxiong Rhizoma (Chuanxiong), Poria Cocos (Fuling), Cyperi Rhizoma (Xiangfu), Pinelliae Rhizoma Preparatum (Qingbanxia), Ostreae Concha (Muli), Polygala Tenuifolia Rhizome (Yuanzhi), Chrysanthemi Indici Flos (Jinyej), Red Date (Hongzao), Ginger (Jiang).
Shi Ju swelling increases slightly with distending pain, indicating Liver Fire rising again. Harmonize Ying and dissolve hardness, while soothing Liver depression.
Remove Shengdi and Yuanzhi from the previous prescription, add Prunellae Spica (Xiakucao).
Shi Ju swells again, accompanied by vomiting. The chest and heart feel stifled and restless, with Liver and Stomach Qi rebelling upward. In a body of extreme deficiency, attacking and tonifying are both difficult. The condition is in a dangerous state. Tentatively prescribe Xiangsha Liujun Tang with modifications as the main prescription.
Prescription: Codonopsis Radix (Dangshen), Atractylodis Macrocephalae Rhizoma (Dongzhu), Angelicae Sinensis Radix (Danggui), Euphorbiae Humifusa Herba (Peilan), Citri Reticulatae Pericarpium (Guangpi), Poria Cocos (Fuling), Hordeum Vulgare Sprout (Guyagen), Aucklandiae Radix (Muxiang), Amomi Fructus (Sharen), Pinelliae Rhizoma Preparatum (Qingbanxia), Glycyrrhizae Radix et Rhizoma (Zhigancao), Curcumae Radix (Yujin), Fresh Ginger (Shengjiang), Jujube (Zao).
Phlegm, Qi, and Blood accumulate in the Liver collaterals, located on both sides of the lower abdomen, forming Shi Ju with hard swelling. Wood Qi loses sensation/pain perception. Tentatively prescribe warming and dissipating methods, hoping it does not ulcerate, which would be auspicious.
Prescription: Angelicae Sinensis Radix (Danggui), Paeoniae Radix Rubra (Chishao), Persicae Semen (Taoren), Poria Cocos (Fuling), Cinnamomi Cortex (Rougui), Pinelliae Rhizoma Preparatum (Qingbanxia), Citri Reticulatae Pericarpium (Chenpi), Glycyrrhizae Radix et Rhizoma (Gancao), Corydalis Rhizoma (Yanhusu), Uncaria Rhynchophylla/Galbulus (Lengzi - note: context suggests a specific local herb or typo for another, kept as literal translation), Fresh Ginger (Shengjiang).
Dampness and Blood stasis coagulate in the meridians. At Weiyang Acupoint, Shi Ju is hard and swollen, purple in color, and the inner calf is numb and swollen. There is fever and pain at night. Convergence will be difficult later; urgently resolve dampness and dissolve coagulation to aim for contraction.
Prescription: Polygoni Multiflori Preparati Radix (Shengshouwu), Angelicae Sinensis Radix Cauda (Guiwei), Glycyrrhizae Radix et Rhizoma (Gancao), Myrrha (Meyao), Forsythiae Fructus (Lianqiao), Dioscoreae Hypoglaucae Rhizoma (Chuanbi), Paeoniae Radix Rubra (Chishao), Persicae Semen (Taoren), Phellodendri Cortex (Huangbai), Lycopus Herba (Zelan), Achyranthis Bidentatae Radix (Huainiuxi), Citri Reticulatae Pericarpium (Guangpi), Mori Ramulus (Sangzhi).
Depression and anger injure the Liver, causing Qi stagnation in the collaterals, leading to collateral blood stasis. Chest and hypochondrium hurt, followed by hard swelling at Rugen, manifesting as a major Shi Ju. The pulse is Wiry and Strong. Movement causes panting, spontaneous sweating, and night sweats. Liver Yin is injured, and Kidney Qi fails to consolidate. The symptoms are extremely severe. Prescribe nourishing Yin and softening the Liver to dissolve hard knots.
Prescription: Glehniae Radix (Beishashen), Ostreae Concha (Muli), Angelicae Sinensis Radix (Danggui), Fritillariae Thunbergii Bulbus (Dabei), Paeoniae Radix Alba (Baishao), Polygala Tenuifolia Flesh (Yuanzirou), Lycopus Herba (Zelan), Poria Cum Pachyrama Stem (Fushen), Salviae Miltiorrhizae Radix (Danshen), Citri Reticulatae Pericarpium (Guangpi), Citrus Reticulata 'Chachi' Pericarpium (Juye), Trichosanthis Fructus (Gualouzi), Nymphaeae Nelumbinis Rhizomatis Nodus (Ou Jie).
