Use a four-flame lamp (a lamp equipped with four wicks in one holder), add Cinnabar (Zhusha) into the lamp holder, and once lit, the parturient will calm down. "Four flames" refers to using four lamp heads within a single lamp.
Take raw Pinelliae Rhizoma (Sheng Banxia), sized like a bean, grind it into powder, and blow it into the nostrils; if sneezing occurs, the infant will survive.
If medical help is unavailable for a moment, use half a cup of White Honey (Baimi) and half a cup of Sesame Oil (Mayou), boil until half remains, and administer orally for immediate effect. However, this is not as effective as Jiawei Xionggui Tang (see previous section).
Dystocia often results from the mother panicking and taking the birthing position too early, or when the amniotic sac has ruptured but the fetus has not yet turned, or when the fetus has turned but not into a proper position, leading to the mother pushing forcefully, causing the feet to present first (known as breech presentation/nichan), hands to present first (known as transverse lie/hengsheng), or shoulders, ears, or forehead to present first (known as shoulder presentation/cichan), or when the umbilical cord is entangled preventing descent (known as obstructed delivery/aichan). In such emergencies, where two lives are at stake, there must be no unnecessary noise or panic, and midwives are strictly forbidden from acting recklessly. The primary principle is calmness. Regardless of whether it is breech, transverse, or obstructed presentation, gently push the fetus back inside, administer Foshou San (Buddha's Hand Powder) mentioned earlier, which is the most stable and effective method, and refer to the following methods for rescue.
Have the parturient lie supine in a straight posture, ensuring her mind and spirit are calm and free from fear. Request an experienced midwife to trim her fingernails and moisten her hands with Sesame Oil (Mayou). Gently insert the fetal feet, then push the fetal body upward so that it necessarily turns upright. Wait until the body is straight and the head is aligned, then administer the previously mentioned labor-inducing medicine. If thirsty, drink honey water; if hungry, eat thin porridge. Then assist the mother to rise slightly, apply force, and the infant will be delivered. This depends on the skill of the midwife; rough or incompetent persons must absolutely not be used. Furthermore, pricking the sole of the foot with needles must be avoided, as fetal pain may cause it to grip tightly, endangering the mother's life.
For all other cases, follow the above methods: gently insert the fetal hand again and push upward. Locate the fetal shoulder and gradually straighten it, after which the infant will descend. Avoid using needle pricks.
Similarly, have the parturient lie supine, following the previous methods. The midwife should examine carefully under lamplight to accurately identify whether the shoulder, forehead, left side, or right side is presenting. Use manual manipulation to gently adjust and straighten the position. Administer medication and food as before, assist the mother to rise and exert force, and the infant will descend.
