The white membrane falls off, a sawing sound is heard in the throat, hoarseness or aphonia occurs, the throat is dry with no saliva, the eyes stare fixedly, phlegm is obstructed causing panting, symptoms persist for seven days without improvement, both the face and lips turn cyanotic, medicines cannot be swallowed, bowel movements are blocked after taking medicine, while diarrhea occurs when no medicine is taken, and the diarrhea continues incessantly.

At the onset, use Kudzu Root (Puerariae Radix) [Fenge], Silkworm (Bombyx Batryticatus) [Jiangcan], and Cicada Slough (Cicadae Periostracum) [Chantui] to disperse wind-heat; use Burdock Fruit (Arctii Fructus) [Niuzi], Forsythia Fruit (Forsythiae Fructus) [Lianqiao], Honeysuckle Flower (Lonicerae Japonicae Flos) [Jinyinhua], and Smilax Glabra Rhizome (Smilacis Glabrae Rhizoma) [Tufuling] to reduce swelling and eliminate toxins; use Rehmannia Root (Rehmanniae Radix) [Shengdi], Scutellaria Root (Scutellariae Radix) [Huangqin], Scrophularia Root (Scrophulariae Radix) [Yuanshen], Gardenia Seed (Gardeniae Fructus) [Zhiren], Isatis Root (Isatidis Radix) [Dougen], Ophiopogon Tuber (Ophiopogonis Tuber) [Maidong], and Gypsum Fibrosum (Gypsum) [Shigao] to clear heat; use Akebia Caulis (Akebiae Caulis) [Mutong], Alisma Rhizome (Alismatis Rhizoma) [Zexie], and Plantago Seed (Plantaginis Semen) [Cheqianzi] to guide heat downward. In severe cases, add Puffball (Lycoperdon) [Mabo] and Gentian Root (Gentianae Radix) [Longdancao]. Mabo is an essential herb for throat diseases. Externally, crush fresh live-rooted Achyranthes Bidentata Root (Achyranthis Bidentatae Radix) [Niuxidou] and decoct it in water; take it frequently like tea. Alternatively, crush fresh Qingxiang Grass (Rumex japonicus) [Wannianqing] to extract juice; ingest it, hold it in the mouth, or rinse the mouth frequently, which is excellent. For mild throat conditions, treat primarily with Chuwen Huadu San (Eliminate Plague and Transform Toxin Powder); for moderate severity, use Shengong Pixie San (Divine Merit Expel Evil Powder); for severe cases, use Shenxian Huoming Tang (Immortal Life-Saving Decoction). Mild cases require one to two doses daily; severe cases require three to four doses daily. The goal is to draw the epidemic toxin from the Upper Jiao (Upper Burner) to the Middle Jiao (Middle Burner), and then from the Middle Jiao to the Lower Jiao (Lower Burner), expelling it via the large intestine. Diarrhea indicates that fire-toxin is moving downward, which is an auspicious sign. If there is constipation, add a small amount of Cooked Rhubarb (Rhei Radix et Rhizoma) [Shu Dahuang] and a small amount of Mirabilite (Natrii Sulfas) [Yuanmingfen]. If constipation persists, switch to Raw Rhubarb (Rhei Radix et Rhizoma) [Sheng Dahuang]; remove it if diarrhea occurs. Mild cases heal after more than ten doses; severe cases may require twenty to thirty doses. The treatment continues until the white spots completely disappear. The addition or subtraction of herbs and dosages, as well as the number of doses, must be adapted to the specific clinical presentation. Generally, these herbs suffice; there is no need to change to other formulas. Take them urgently over consecutive days, and full recovery will naturally occur; this has been tested repeatedly with success. After the white spots have cleared, use cooling agents, primarily treating with Qingxin Dipai Tang (Clear Heart and Purify Lung Decoction), one dose daily, to eliminate residual toxins. Avoid excessive use of tonics, lest the residual toxins remain. This requires flexible application based on clinical judgment.

The above treatments for White Throat Disease (Baihou) apply generally to Single Tonsillar Swelling (Dan E), Double Tonsillar Swelling (Shuang E), as well as Pharyngeal Abscess (Houyong), Pharyngeal Obstruction (Houbi), Winding Wind Throat (Chanhoufeng), Running Horse Wind Throat (Zouma Houfeng), wind-heat affecting the throat, and redness and swelling within the throat. All these conditions can be treated according to this method using the aforementioned decoctions. For throat insufflation powders, Yuanzhu Dan (Cinnabar Pill) is considered the first miraculous pill.
