(Jueyin Wood and Ministerial Fire govern the qi. Therefore, it corresponds to the transition between the third and fourth lunar months. During the third month when Yang qi is abundant, yet Jueyin governs the qi, it is thus termed Yin within Yang. Here, "pathogen" refers to Yin qi. The Jueyin qi resides internally but fails to fully emerge externally, thereby causing hernia (Shan) and abdominal swelling. Zhang Zhaohuang stated: There are cases where Yang qi, while ascending, is suppressed by seasonal factors; there are also cases where seasonal qi is at its peak while simultaneously being governed by Yin qi. One must understand the Yin and Yang of the extraordinary (Qiheng), and how they deviate from the four seasons to cause disease.)
Regarding the statement that waist and spine pain prevents flexion and extension: In the third month, Yang qi vibrates and stimulates growth, and all things flourish. Leaves and branches hang downward, exhibiting only flexion without extension. As humans are the spirit of all things, their waist and spine also suffer pain and inability to flex or extend.
Regarding the statement about strangury (Long), hernia (Shan), and cutaneous distension: It indicates that Yin qi is also abundant, causing vessel distension and obstruction, hence termed strangury and hernia. "Yin qi is also abundant" means the Jueyin qi is abundant externally. When Yin is excessive and vessels are distended and obstructed, strangury and cutaneous distension occur. Strangury and hernia refer to swelling of the genital organs accompanied by an inability to urinate. Note: This section serves as Bo Gao's continuation of the previous chapter, explaining the pulse qi of extraordinary diseases through a self-posed question and answer format. Thus, the character "Yue" (said) is added at the end to clarify this rhetorical structure. Zhang Zhaohuang noted: The phrase "Yue Suowei" uses "Yue" as a hypothetical question marker, with the following text serving as the answer; hence, the addition of "Yue" distinguishes the question from the answer.)
Regarding the statement that severe cases manifest as dry throat and internal heat: This occurs because Yin and Yang contend, generating heat, which leads to a dry throat. "Severe" refers to the extreme abundance of Yang qi. When Jueyin qi contends with excessive Yang qi, Yin qi transforms into heat. Severe heat results in a dry throat and internal heat. The Yin and Yang of human viscera and bowels correspond with the Qi of Heaven and Earth and the four seasons, with cold and heat alternating in sequence. However, the patterns of the extraordinary (Qiheng) begin every sixty days, opposing the four seasons and causing disease. Therefore, the sages mastered the art of diagnosis by first discerning the order of Yin and Yang, and subsequently identifying anomalies in Yin and Yang. By examining the beginning and end of diseases and understanding both routine and variable conditions, the Dao of diagnosis becomes complete, allowing for mastery in clinical practice.)
