Liver Heat Disease presents with urine turning yellow first, abdominal pain, excessive sleeping, and body heat. When pathogenic heat contends with healthy qi, there is incoherent speech and fright, distension and pain in the hypochondrium, restlessness of the hands and feet, and inability to lie down calmly. The condition worsens on Geng-Xin days (Metal days) and resolves with profuse sweating on Jia-Yi days (Wood days). If Qi rebels, death occurs on Geng-Xin days; treatment involves needling the Foot-Jueyin (Liver) and Foot-Shaoyang (Gallbladder) meridians. If Qi rebels, there is dizziness and headache due to meridian Qi rushing upward to the head.
Heart Heat Disease begins with a sense of unhappiness, followed by fever after several days. When pathogenic heat contends with healthy qi, there is sudden heart pain, vexation, frequent vomiting, headache, red face, and absence of sweat. The condition worsens on Ren-Gui days (Water days) and resolves with profuse sweating on Bing-Ding days (Fire days). If Qi rebels, death occurs on Ren-Gui days; treatment involves needling the Hand-Shaoyin (Heart) and Hand-Taiyang (Small Intestine) meridians.
Spleen Heat Disease begins with heavy head and cheek pain, mental distress, greenish complexion, desire to vomit, and body heat. When pathogenic heat contends with healthy qi, there is lower back pain preventing flexion and extension, abdominal fullness, diarrhea, and pain in both mandibles. The condition worsens on Jia-Yi days (Wood days) and resolves with profuse sweating on Wu-Ji days (Earth days). If Qi rebels, death occurs on Jia-Yi days; treatment involves needling the Foot-Taiyin (Spleen) and Foot-Yangming (Stomach) meridians.
Lung Heat Disease begins with chills, erection of hair follicles, aversion to wind and cold, yellow tongue coating, and body heat. When pathogenic heat contends with healthy qi, there is wheezing and coughing, pain radiating to the chest, epigastrium, and back, inability to take deep breaths, unbearable headache, sweating followed by chills. The condition worsens on Bing-Ding days (Fire days) and resolves with profuse sweating on Geng-Xin days (Metal days). If Qi rebels, death occurs on Bing-Ding days; treatment involves needling the Hand-Taiyin (Lung) and Hand-Yangming (Large Intestine) meridians. Bleeding like the size of a soybean stops the symptoms immediately.
Kidney Heat Disease begins with lower back pain, bitter thirst with frequent drinking, and body heat. When pathogenic heat contends with healthy qi, there is neck pain and stiffness, chills with tremors, heat in the soles of the feet, and reluctance to speak. If Qi rebels, there is dizziness and vague neck pain. The condition worsens on Wu-Ji days (Earth days) and resolves with profuse sweating on Ren-Gui days (Water days). If Qi rebels, death occurs on Wu-Ji days; treatment involves needling the Foot-Shaoyin (Kidney) and Foot-Taiyang (Bladder) meridians. In all cases of sweating in heat diseases, sweating occurs on the day that overcomes the disease's associated element.
In Liver Heat Disease, the left cheek becomes red first. In Heart Heat Disease, the forehead becomes red first. In Spleen Heat Disease, the nose becomes red first. In Lung Heat Disease, the right cheek becomes red first. In Kidney Heat Disease, the chin becomes red first. Even if the disease has not yet fully manifested, if these areas show redness, needling should be performed; this is called "treating disease before it arises" (Zhi Wei Bing). Heat diseases originating from specific facial regions resolve by their corresponding time; if needling is contraindicated or incorrect, resolution takes three cycles; severe violation leads to death. All heat diseases requiring sweating will resolve with profuse sweating on their overcoming day.
All treatments for heat diseases involve having the patient drink cold water, then performing needling; the patient must wear light clothing and reside in cool places until the body cools. For heat disease starting with chest/hypochondriac pain and restlessness of limbs, needle the Foot-Shaoyang (Gallbladder) meridian and tonify the Foot-Taiyin (Spleen) meridian; for severe cases, use the Fifty-Nine Acupoints for Heat. For heat disease starting with arm pain, needle the Hand-Yangming (Large Intestine) and Hand-Taiyin (Lung) meridians to induce sweating. For heat disease starting with head pain, needle the Taiyang points in the nape to induce sweating. For heat disease starting with leg pain, needle the Foot-Yangming (Stomach) meridian to induce sweating. For heat disease starting with heavy body, bone pain, deafness, and desire to close eyes, needle the Foot-Shaoyin (Kidney) meridian; for severe cases, use the Fifty-Nine Acupoints for Heat. For heat disease starting with vertigo, fever, and chest/hypochondriac fullness, needle the Foot-Shaoyin (Kidney) and Foot-Shaoyang (Gallbladder) meridians.
If the color of the Foot-Taiyang (Bladder) meridian appears on the zygomatic bone, it indicates heat disease. If this redness has not yet intersected with other facial colors, it indicates that sweating will occur today, and one should wait for the appropriate time for resolution. If it appears simultaneously with the Foot-Jueyin (Liver) meridian signs, death will occur within three days, as the heat disease internally connects to the Kidney and shows Shaoyang meridian color. If the color of the Foot-Shaoyang (Gallbladder) meridian appears on the front of the cheek, it indicates heat disease. If this redness has not yet intersected with other facial colors, it indicates that sweating will occur today, and one should wait for the appropriate time for resolution. If it appears simultaneously with the Foot-Shaoyin (Kidney) meridian signs, death will occur within three days.
Acupoints for Heat Diseases: The space below the third thoracic vertebra treats heat in the chest; the space below the fourth thoracic vertebra treats heat in the diaphragm; the space below the fifth thoracic vertebra treats Liver heat; the space below the sixth thoracic vertebra treats Spleen heat; the space below the seventh thoracic vertebra treats Kidney heat. This redness reflects on the zygomatic bone. The third vertebra in the neck refers to the depression. Below the cheek, against the zygoma, indicates large accumulation (Da Jia); towards the lower jaw indicates abdominal fullness; behind the zygoma indicates hypochondriac pain; above the cheek indicates upper diaphragm pathology.
