(Attachment: Cases of Insomnia, Liver Qi and Liver Yang, Headache and Dizziness)
Zhu You, less than one hundred days postpartum, presented with alternating deficiency-cold and deficiency-heat, milder in the morning and heavier in the evening, persisting for a month. Appetite was poor, stools were loose, body was emaciated, complexion was sallow, accompanied by cough, spontaneous sweating (daytime sweating without exertion), and night sweats (sweating during sleep). The pulse was soggy, slippery, and powerless; the tongue coating was pale white. This indicates that Wei Qi (Defensive Qi) is deficient and fails to protect the exterior, Ying Blood (Nutritive Blood) is deficient and fails to guard the interior. Spleen Earth is weak and fails to generate Lung Metal; deficient Yang forces fluids to leak outward. Postpartum exhaustion is gradually worsening, and full recovery may be difficult. Tentatively prescribe Huangqi Jianzhong Tang (Astragalus Decoction to Establish the Middle) combined with Erjia Longgu Tang (Two-Added Dragon Bone Decoction) with modifications.

Second visit: After administering Huangqi Jianzhong Tang and Erjia Longgu Tang previously, the chills and fever lightened, and spontaneous and night sweats decreased. Although this is a favorable sign, the previous medications were too yin-nourishing, damaging the Middle Jiao (Spleen and Stomach). Clear Qi failed to ascend, resulting in loose stools, poor appetite, sallow complexion, and dull abdominal pain. The left pulse was fine and weak; the right pulse was soggy and slow. Yang has sunk into Yin, and Mingmen Fire (Gate of Life Fire) is declining. The *Maijue* states: "When Yang sinks into Yin, essence and blood are weak; even with white hair, one worries like a youth." This is indeed concerning. Again adhering to the original intent, add herbs that benefit Fire to generate Earth, hoping to strengthen the Middle Jiao. Firming the stool is key.

Third visit: Chills and fever lightened, deficiency sweating decreased, and loose stools showed signs of forming. However, there is much cough and phlegm, reduced food intake, emaciation, sallow complexion, a bare tongue without coating, and a soggy, fine pulse. Fortunately, no critical pulse signs are present. Spleen Earth is weak and fails to generate Lung Metal; Lung Qi is deficient and lacks irrigation authority. Still plan to establish Middle Qi and nourish Spleen Earth. If food intake increases and no complications arise, crisis may turn to safety.

Song You, illness originated from depression. Emotional discomfort led to Qi stagnation and Blood stasis. Wood overacts on Earth (Liver overacting on Spleen), Stomach lacks transformation and transportation, no Blood flows down to Chong and Ren meridians. Amenorrhea for one year. Poor appetite, emaciation, evening chills and fever, copious cough with frothy phlegm. Pulse: left deficient and wiry, right soggy and choppy. As stated in the classics: "Diseases of the Two Yang originate in Heart and Spleen; hidden grievances cause amenorrhea in women." Progression leads to Fengxiao (wind wasting), then Xiben (lung accumulation). If shortness of breath occurs, it is incurable. Tentatively prescribe Xiaoyao San (Free and Easy Wanderer Powder) combined with Guipi Tang (Restore the Spleen Decoction) and Dahuang Zhechong Wan (Major Rhubarb and Ground Beetle Pill), compounded treatment.

Follow-up: Evening chills and fever have lightened, but cough persists. Amenorrhea and poor appetite remain. Tongue is bare without coating. Pulse: left wiry, right choppy. This indicates dryness of the Blood Chamber, Wood-Fire attacking Metal (Liver fire invading Lung), and Spleen-Stomach lacking transformation authority. Must regulate emotions to assist medicinal power. Now plan to strengthen Earth to generate Metal, nourish Blood and unblock menstruation. However, results will not be immediate.

Cai Zuo, cough caused by dryness pathogen in mid-autumn, unresolved into winter. Added sore throat, dryness, tonsillar swelling hindering swallowing, internal heat, poor appetite. Pulse soggy and rapid, fortunately not surging and large. Tongue red with yellow coating. Constitutionally Yin deficient. Dryness transformed into fire, attacking Lung Metal above, consuming Kidney Water below. Water fails to rise, floating fire blazes. Fear hemoptysis leading to consumption. Urgently prescribe clearing dryness and moistening Lung, descending floating fire.

Fang Zuo, recurrent hemoptysis, chronic cough. Exertion causes Qi rebellion, sore throat, loss of voice, emaciation, tidal fever, dry mouth. Pulse wiry, large, and rapid. Wiry indicates consumption; rapid indicates disease progression. Yin fluids exhausted, Wood-Fire invades Lung. Lung lobes damaged, "broken metal does not ring" (metaphor for lung damage). Pulmonary tuberculosis is evident. Tentatively strengthen the source of Water (Kidney) to soften Liver Wood, clear and nourish Lung Qi, and tonify the source of transformation. This is merely doing one's best.

Hou Zuo, Lung deficiency causes cough and chills/fever; Spleen deficiency causes poor appetite and loose stools; Heart deficiency causes fine pulse and fatigue; Kidney deficiency causes seminal emission; Liver deficiency causes dizziness. All five deficiencies are present, making treatment difficult. Only suitable to strengthen Earth to generate Metal, benefit Kidney and nourish Liver. If diarrhea stops, food intake increases, chills/fever cease, and cough reduces, deficiency can be treated.
