Slow convulsion syndrome refers to a condition occurring after an illness, or following vomiting and diarrhea, or due to medicinal damage to the Spleen and Stomach. Symptoms include cold limbs, faint breath from the mouth and nose, twitching of the hands and feet, lethargy with eyes half-open (exposing the pupils). This is a syndrome of wind generated by Spleen deficiency and absence of Yang.
Slow convulsion belongs to the Spleen; it is a disease characterized by deficiency and insufficiency of Middle Qi. (Slow convulsion is a Yin syndrome; treatment should focus on securing the interior and awakening the Spleen. Xingpi San [Awakening the Spleen Powder] is indicated.)
Xingpi San [Awakening the Spleen Powder]: Treats persistent vomiting and diarrhea in children progressing to slow convulsion, where the Spleen is困 (trapped/dull), consciousness is clouded, the child is silent, and has no appetite.
Ingredients: Ren Shen [Ginseng Radix et Rhizoma] (remove rhizome), Bai Zhu [Atractylodis Macrocephalae Rhizoma] (remove rhizome), Fu Ling [Poria], Mu Xiang [Aucklandiae Radix], Quan Xie [Scorpio] (remove toxicity), Tian Ma [Gastrodiae Rhizoma], Bai Fu Zi [Typhonii Rhizoma] (baked), Jiang Can [Bombyx Batryticatus] (stir-fried; all equal parts), Gan Cao [Glycyrrhizae Radix et Rhizoma] (honey-roasted, half dose).
Preparation: Coarsely crush the herbs, add three slices of Sheng Jiang [Zingiberis Rhizoma Recens] and one Da Zao [Jujubae Fructus], decoct with water, and take warm. Another prescription removes Tian Ma and Jiang Can, adds Nan Xing [Arisaematis Rhizoma Praeparatum], Ban Xia [Pinelliae Rhizoma Praeparatum], and 200 grains of Chen Cang Mi [Oryza Sativa Var. Indica Preparata], decocted and taken orally, with repeated efficacy.
Huang Qi Tang [Astragalus Decoction]: A miraculous medicine for treating slow convulsion in children.
Ingredients: Huang Qi [Astragali Radix] (2 qian, stir-fried with honey water), Ren Shen [Ginseng Radix et Rhizoma] (3 qian), Zhi Gan Cao [Glycyrrhizae Radix et Rhizoma Preparata] (5 fen), plus Bai Shao [Paeoniae Radix Alba] (stir-fried, 1 qian).
Preparation: Coarsely crush one dose, decoct with water, and take well after meals.
Treating Spleen convulsion caused by vomiting and diarrhea in children (suitable for ages 1-2):
Ingredients: Zhu Sha [Cinnabaris] (5 li, 1 fen for over 2 years old, 4-5 fen for over 3 years old), Quan Xie [Scorpio] (one, remove legs, wings, and toxicity; one for age 1, two for age 3).
Preparation: Grind into fine powder, mix with milk, and administer.
For any case of slow convulsion where Yuan Qi [Original Qi] deficiency leads to coma, urgently moxa the Bai Hui acupoint [GV20]. If one waits until phlegm descends and the condition remains unhealed before applying moxibustion, the Yuan Qi will scatter and become incurable. This occurs because the transmission between Zang-Fu organs has reached its extreme, ultimately returning to deficiency, specifically affecting the Spleen. There is no wind to expel and no convulsion to treat; this apparent phlegm is actually due to Spleen deficiency failing to control saliva.
Zijin Ding Zi [Purple Gold Pellets]: Treats acute and slow convulsions, flooding of saliva causing spasms, vomiting or diarrhea, loss of appetite, mental fatigue, and weak Qi.
Ingredients: Ren Shen [Ginseng Radix et Rhizoma] (remove rhizome), Bai Zhu [Atractylodis Macrocephalae Rhizoma] (remove rhizome), Fu Ling [Poria], Fu Shen [Poria cum Radice] (remove wood), Shan Yao [Dioscoreae Rhizoma] (stir-fried), Ru Xiang [Olibanum], Chi Shi Zhi [Halloysit Rubrum] (quenched in vinegar seven times), Chen Sha [Cinnabaris] (all 3 qian each), She Xiang [Moschus] (1 qian).
Preparation: Grind into powder, mix with 1 liang of cake flour to form pills the size of a bullet, coat with Jin Bo [Gold Leaf]. Take one pill at a time, dissolve in Bo He Tang [Menthae Haplocalycis Decoction], and ingest.
A child developed Dan [erysipelas] due to food stagnation. After taking exterior-releasing formulas, the child experienced hand and foot spasms. After taking Bao Long Wan [Embracing Dragon Pills], the child's eyes became dull and phlegm increased significantly. I reasoned that this was a transformation into slow convulsion due to Spleen and Stomach damage. Since there was no wind to dispel and no phlegm to purge, only warming and tonifying Stomach Qi was appropriate. Thus, Liu Jun Zi Tang [Six Gentlemen Decoction] was used with added Fu Zi [Aconiti Lateralis Radix Preparata], resulting in cure after one dose. (Formula see "Tonification" section.)
Another child presented with spasms and spontaneous flow of saliva/phlegm. The use of convulsion-treating medicines exacerbated the condition. Observing the yellowish-white complexion, I used Liu Jun Zi Tang [Six Gentlemen Decoction] and Bu Zhong Yi Qi Tang [Tonify the Middle and Augment the Qi Decoction] to tonify Spleen Qi, leading to recovery. (Both formulas see "Tonification" section.)
Hun Yuan Dan [Primordial Elixir Pill]: Treats pediatric acute and slow convulsions, cough with phlegm, wheezing, heat, vomiting, diarrhea, abdominal distension, and has great efficacy for various pediatric diseases. (Formula see "General Treatment" section.)
