Xu Fan (Deficiency Heat/Restlessness) refers to what the formula treatises call Heart deficiency with vexation and oppression. Generally speaking, Yin deficiency generates internal heat, while Yang excess generates external heat. External heat is termed "Zao" (Dryness), and internal heat is termed "Fan" (Vexation); this distinction is subtle yet significant. After a major Shanghan (Cold Damage) illness, when the body has not returned to its normal state, or after Huoluan (Colic with vomiting and diarrhea), these conditions often cause Heart deficiency with vexation and oppression. Women after childbirth frequently suffer from this disease. The symptoms include internal vexation without feeling hot on the body, dizziness and headache, dry mouth and throat without thirst, and wakefulness with inability to sleep; all are manifestations of Xu Fan. There is a notion in formula examples that Xu Fan is close to Shanghan, which must be carefully distinguished. Additionally, spontaneous sweating in healthy individuals, frequent urination accompanied by seminal emission and white turbid discharge, are all caused by excessive worry and vexation. Shanghan belongs to external causes, worry and vexation belong to internal causes, and Huoluan involves both internal and non-internal causes. Scholars should analyze and treat accordingly.
